    CHANGE IN MUSCLE ACITIVITY DURING SKIING USING 2 SIMULATORS

    This research analyzed the change in lower muscle activity during skiing using two simulators. Participants in this study were 8 male adults who held a certificate issued by the Korea Ski Instructors Association. Kinematic factors when skiing on two types of simulators were compared and analyzed. Most subjects maintained a higher value of %MVIC while repeating the positions on the Pro Ski Simulator than Skiers Edge. Efficient skiing on ski-simulators requires the maximum use of skiing time on each foot and the ability to quickly flex and extend the hip and the knee joints. Furthermore, differences between individuals exist depending on the level of ski technique and the type of simulator and it is believed that the development of an additional adjustment apparatus in the existing equipment is necessary to enable safe and effective motions

    Fade Lighting Control Method for Visual Comfort and Energy Saving

    This study proposes a fade lighting control method to ensure the visual comfort of indoor occupants through gradual illuminance control while saving energy. The illuminance sensor measures the indoor illuminance and calculates the required illuminance for achieving a reference illuminance of 500 Lux. The control illuminance for each lighting is derived based on the required illuminance, and it is confirmed to fall within the threshold range of 20%. The illuminance values and time intervals for fade lighting control are calculated, ensuring that the amount of illuminance adjustment is divided by the size of the threshold range or less. In the performance evaluation, the proposed method (experimental group) was compared with the influence-based control method (control group). The result shows that this fade lighting control method minimizes the visual discomfort of occupants caused by sudden changes in lighting, and the same energy-saving of 11-42% is achieved as the control group

    Data-Reserved Periodic Diffusion LMS With Low Communication Cost Over Networks

    In this paper, we analyze diffusion strategies in which all nodes attempt to estimate a common vector parameter for achieving distributed estimation in adaptive networks. Under diffusion strategies, each node essentially needs to share processed data with predefined neighbors. Although the use of internode communication has contributed significantly to improving convergence performance based on diffusion, such communications consume a huge quantity of power in data transmission. In developing low-power consumption diffusion strategies, it is very important to reduce the communication cost without significant degradation of convergence performance. For that purpose, we propose a data-reserved periodic diffusion least-mean-squares (LMS) algorithm in which each node updates and transmits an estimate periodically while reserving its measurement data even during non-update time. By applying these reserved data in an adaptation step at update time, the proposed algorithm mitigates the decline in convergence speed incurred by most conventional periodic schemes. For a period p, the total cost of communication is reduced to a factor of 1/p relative to the conventional adapt-then-combine (ATC) diffusion LMS algorithm. The loss of combination steps in this process leads naturally to a slight increase in the steady-state error as the period p increases, as is theoretically confirmed through mathematical analysis. We also prove an interesting property of the proposed algorithm, namely, that it suffers less degradation of the steady-state error than the conventional diffusion in a noisy communication environment. Experimental results show that the proposed algorithm outperforms related conventional algorithms and, in particular, outperforms ATC diffusion LMS over a network with noisy links.11Ysciescopu

    Successful Rescue Therapy with Pumpless Extracorporeal Carbon Dioxide Removal in a Patient with Persistent Air Leakage due to Empyema

    A young metastatic lung cancer patient developed empyema due to an infection with carbapenem-resistant Acinetobacter baumannii. Hydropneumothorax was detected and managed by a tube thoracotomy. However, persistent air leakage through the chest tube was observed due to the presence of a bronchopleural fistula (BPF). As hypercapnic respiratory failure had progressed and the large air leak did not diminish by conservative management, a pumpless extracorporeal lung assist (pECLA) device was inserted. The pECLA allowed the patient to be weaned from mechanical ventilation and the BPF to heal. The present case shows the effective application of pECLA in a patient with empyema complicated with BPF and severe hypercapnic respiratory failure. pECLA enabled us to minimize airway pressure to aid in the closure of the BPF in the mechanically ventilated patient

    Impact of Toxocariasis in Patients with Unexplained Patchy Pulmonary Infiltrate in Korea

    Toxocariasis is one of the causes of pulmonary eosinophilic infiltrate that is increasing in Korea. This study was designed to identify the prevalence of toxocara seropositivity in patients with unexplained pulmonary patchy infiltrate and to evaluate associated factors. We evaluated 102 patients with unexplained pulmonary patchy infiltrate on chest computed tomography (CT) scan. As a control set, 116 subjects with normal chest CT were also evaluated. History of allergic disease, drug use, parasitic disease and raw cow liver intake were taken. Blood eosinophil count and total IgE level were measured. Specific serum IgG antibody to Toxocara canis larval antigen and specific IgG antibodies to 4 other parasites were measured by enzyme-linked immunosorbent assay (ELISA). In the infiltrate group, 66.7% subjects were toxocara seropositive whereas 22.4% of the control group were seropositive (p<0.001). In the infiltrate group, patients with a history of eating raw cow liver (odds ratio [OR], 7.8) and patients with eosinophilia (OR, 5.2) had a higher incidence of toxocara seropositivity. Thirty-five percent of toxocara seropositive patients with infiltrate exhibited migrating infiltrate and 48% had decreased infiltrate on the follow-up CT. We recommend that toxocara ELISA should be performed in patients with unexplained pulmonary patchy infiltrate, and that the eating of raw cow liver should be actively discouraged

    Kinematics Characteristic of Lower Extremity during Simulated Skiing Exercise

    We analyze the kinematic factors of sectional and total movements in healthy participants to providing group dependent information during simulated exercise. Participants in this study's experiment were 26 male adults, the elapsed times in each phase, the difference in the lower extremity angles, and muscle activity were computed through analysis of kinematic factors. We revealed that motions of the experts took shorter to perform than non-experts, and showed larger variation of lower limb joint angle in most events during simulated skiing. There were also significant group dependent differences in the peak and mean EMG values during simulated skiing. Referring to these results, such a non-expert's posture leads to enhance muscle activity to keep the lower body in balance. Non-experts should maintain appropriate ROM with lower-intensity exercise to improve muscle endurance initially, and it can be useful in providing preliminary data for future training and rehabilitation studies, as well as improvements in muscle strength

    Epidermal Growth Factor Receptor Mutation and Pathologic-Radiologic Correlation Between Multiple Lung Nodules with Ground-Glass Opacity Differentiates Multicentric Origin from Intrapulmonary Spread

    IntroductionNo standard guidelines detailing recommendations for the selection and treatment for multiple lung nodules with ground-glass opacity (GGO) have been established. For treatment decision, we analyzed epidermal growth factor receptor (EGFR)/K-ras somatic aberrations and pathologic-radiologic correlation in multiple lung nodules presented as GGO to differentiate multifocal lesions from intrapulmonary spread.MethodsTwenty-four patients with multiple lung nodules presented as GGO were identified to investigate somatic mutations of EGFR (exon 18–21) and K-ras (codons 2, 13, and 61). This series included 18 atypical adenomatous hyperplasias (AAH), 15 bronchioloalveolar carcinomas (BAC), and 23 adenocarcinomas (ADC) obtained from 24 patients.ResultsHigh frequency of discordant EGFR mutations (17 of 24, 70.8%) could discriminate tumor clonality (18 of 24, 75%) of multiple lung neoplastic nodules presented as GGO. EGFR mutations were common in AAH (38.9%), BAC (46.7%), and ADC (39.1%). In case 4, AAH and BAC had different mutational changes, and in case 10, the BAC lesion contains EGFR mutation that is not in the invasive ADC. In case 17, the BAC had more mutational changes than the carcinoma. The pure GGO appearance in the radiologic examination corresponded preinvasive pathologic change.ConclusionsThis study showed that synchronous BAC and/or ADC can have different EGFR or K-ras mutational profiles suggesting these lesions arise as independent events rather than intrapulmonary spread or systemic metastasis. This has significant implication in staging and treatment. These findings might be a clue to establish guidelines of the multiple neoplastic lung nodules with GGO
