    Effectiveness of Balance Exercise Program in Total Knee Arthroplasty: Meta-Analysis

    Background: Balance is essential for maintaining postural stability while performing functional activities and for falls avoidance in patients with TKA.  There are very few RCTs done to assess functional improvement after TKA. Hence, this study was undertaken to find out evidence on physiotherapy treatment in TKA and analyze it.  Methods: We searched Medline, CINAHL, PEDro from the year 2001 to 2015 for RCT involving evaluations of physiotherapy exercise in adults with a knee replacement. Search criteria’s were pain, stiffness, adherence and attrition. The inclusion criteria were studies which included physiotherapy exercise for balance specific exercises which compared various functional training and functional training with balance exercise in TKA. Result: Searches identified 20 randomized trials related to TKA and physiotherapy. Only three were fulfilled the inclusion criteria. Functional training with balance exercises found to be effective in patients with total knee arthroplasty. Conclusion: After total knee replacement, interventions including physiotherapy along with balance exercises showed improvement in pain, stiffness and functional performance in mobility.     Approximating Tverberg Points in Linear Time for Any Fixed Dimension

    Let P be a d-dimensional n-point set. A Tverberg-partition of P is a partition of P into r sets P_1, ..., P_r such that the convex hulls conv(P_1), ..., conv(P_r) have non-empty intersection. A point in the intersection of the conv(P_i)'s is called a Tverberg point of depth r for P. A classic result by Tverberg implies that there always exists a Tverberg partition of size n/(d+1), but it is not known how to find such a partition in polynomial time. Therefore, approximate solutions are of interest. We describe a deterministic algorithm that finds a Tverberg partition of size n/4(d+1)^3 in time d^{O(log d)} n. This means that for every fixed dimension we can compute an approximate Tverberg point (and hence also an approximate centerpoint) in linear time. Our algorithm is obtained by combining a novel lifting approach with a recent result by Miller and Sheehy (2010).Comment: 14 pages, 2 figures. A preliminary version appeared in SoCG 2012.     The potential risk of HIV-1 infection following human bite although epidemiologically insignificant, but it is biologically possible. There are anecdotal reports of HIV transmission by human bites particularly if saliva is mixed with blood. The oral tissues support HIV replication and may serve as a previously unrecognized HIV reservoir. The HIV infected individuals have more viruses in blood than saliva, possibly due to the potent HIV-inhibitory properties of saliva. The case presented here is of a primary HIV infections following a human bite where in the saliva was not blood stained but it got smeared on a raw nail bed of a recipient. The blood and saliva of the source and blood of the recipient showed a detectable viral load with 91% sequence homology of C2-V3 region of HIV gp120 between the two individuals. The recipient did not receive PEP [post exposure prophylaxis] as his family physician was unaware of salivary transmission. The family physician should have taken PEP decision after proper evaluation of the severe and bleeding bite. Hence it is necessary to treat the HIV infected human bites with post exposure prophylaxis

    Physician and Patient Moves in the Hospital Do Not Move Patient Satisfaction: A Single-Center Retrospective Analysis

    Background. Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) is a standardized survey for measuring patient\u27s experiences at US hospitals. There is a shift towards geographically assigning patients and physicians. However, its impact on patient satisfaction scores has not been studied. Objective: Examine the correlation between patient experience and overall hospital rating with the number of physicians seen and the number of times the patient was moved during a hospitalization. Methods: A retrospective observational study was performed using select HCAHPS & Press Ganey survey questions to assess physician satisfaction scores and overall hospital rating and recommendation scores. Results: There was no significant difference across the select survey questions based on the number of times a patient was moved or the hospitalists seen during a hospitalization on most questions. A higher case mix index was associated with an increased likelihood of receiving the highest rating for the hospital (OR 1.39,95% CI 1.03-1.88), p 0.03), and two other physician communication questions. An increase in the length of stay was associated with a lower likelihood of receiving the highest rating for similar physician communication questions (OR 0.91, 95% CI 0.83 to 0.99,p 0.04) Conclusions: The number of hospitalists seen by the patient and the number of times a patient is moved during a hospitalization is not associated with physicians related patient satisfaction scores, overall care received in the hospital, or if the patient would recommend the hospital to others
