    The contribution of fatigue and sleepiness to depression in patients attending the sleep laboratory for evaluation of obstructive sleep apnea

    Purpose: A high prevalence of depressive symptomatology has been reported amongst sufferers of obstructive sleep apnea (OSA), but it remains unclear as to whether this is due to their OSA or other factors associated with the disorder. The current study aimed to assess the incidence and aetiology of depression in a community sample of individuals presenting to the sleep laboratory for diagnostic assessment of OSA. Methods: Forty-five consecutive individuals who presented to the sleep laboratory were recruited; of those, 34 were diagnosed with OSA, and 11 were primary snorers with no clinical or laboratory features of OSA. Nineteen control subjects were also recruited. Patients and controls completed the Beck Depression Inventory, the Profile of Mood States (POMS), and the Epworth Sleepiness Scale to assess their mood and sleepiness, prior to their polysomnography. Results: All patients reported significantly more depressive symptoms compared with healthy controls, regardless of their degree of OSA. There were no significant differences between OSA patients and primary snorers on any of the mood and self-rated sleepiness measures. Depression scores were not significantly associated with any of the nocturnal variables. Regression analysis revealed that the POMS fatigue subscale explained the majority of the variance in subjects' depression scores. Conclusions: Fatigue was the primary predictor of the level of depressive symptoms in patients who attended the sleep laboratory, regardless of the level of severity of sleep disordered breathing. When considering treatment options, practitioners should be aware of the concomitant occurrence of depressive symptoms and fatigue in patients presenting with sleep complaints, which may not be due to a sleep disorder

    Sleep quality and the treatment of intestinal microbiota imbalance in Chronic Fatigue Syndrome: A pilot study

    Chronic Fatigue Syndrome (CFS) is a multisystem illness, which may be associated with imbalances in gut microbiota. This study builds on recent evidence that sleep may be influenced by gut microbiota, by assessing whether changes to microbiota in a clinical population known to have both poor sleep and high rates of colonization with gram-positive faecal Streptococcus, can improve sleep. Twenty-one CFS participants completed a 22- day open label trial. Faecal microbiota analysis was performed at baseline and at the end of the trial. Participants were administered erythromycin 400 mg b.d. for 6 days. Actigraphy and questionnaires were used to monitor sleep, symptoms and mood. Changes in patients who showed a clinically significant change in faecal Streptococcus after treatment (responders; defined as post-therapy distribution<6%) were compared to participants who did not respond to treatment. In the seven responders, there was a significant increase in actigraphic total sleep time (p=0.028) from baseline to follow up, compared with non-responders. Improved vigour scores were associated with a lower Streptococcus count (ρ=-0.90, p=0.037). For both the responders and the whole group, poorer mood was associated with higher Lactobacillus. Short term antibiotic treatment appears to be insufficient to effect sustainable changes in the gut ecosystem in most CFS participants. Some improvement in objective sleep parameters and mood were found in participants with reduced levels of gram-positive gut microbiota after antibiotic treatment, which is encouraging. Further study of possible links between gut microorganisms and sleep and mood disturbances is warranted

    Hayes, Juarez, and Escoffery-Runnels (2014) analyzed the educational philosophies and pedagogical practices of two educators to understand how personal and professional experiences individually and collectively influenced their approach to teaching. Using oral histories, they presented an argument of why culturally relevant and social justice–oriented teaching has historically been an effective tool in educating students of color, and why it is necessary for teacher preparation in today’s so-called post-racial climate. We suggest that that the education system is merely a microcosm of society, and consequently, we must consider structures larger than individual best practices when discussing culturally relevant teaching. Bridges to benefits are networks of White privilege that flow between institutions, such as education, the economy, and the law, and that involve capitalizing on the misery of Blacks while simultaneously protecting White supremacy. We use the “bridges to benefits” concept to propose that scholars not focus solely on education but rather focus on how social institutions in general are created and designed such that they continually oppress and suppress Black and Brown Americans. We draw special attention to America’s criminal justice system, labor, and housing market

    Sleep and mental health among paramedics from Australia and Saudi Arabia : a comparison study

    Paramedics face many challenges while on duty, one of which is working different types of shifts. Shift work has been linked to a number of health issues such as insomnia, depression, and anxiety. Besides shift work, Saudi paramedics, a group that has not been investigated for sleep or mental health issues previously, may be facing more demands than Australian paramedics due to lower numbers of paramedics in comparison to the general population. The aim of this study was to investigate the prevalence of sleep and mental health disorders among paramedics in Saudi Arabia and Australia. Paramedics were invited to complete a survey to assess stress, post-traumatic stress disorder (PTSD), depression, anxiety, daytime sleepiness, insomnia, sleep quality, shift work disorder, obstructive sleep apnoea, fatigue, and general health. A total of 104 males Saudi paramedics (M age = 32.5 +/- 6.1 years) and 83 males paramedics from Australia (M age = 44.1 +/- 12.1 years) responded to the survey. Significantly higher rates of depression, PTSD, insomnia, and fatigue, along with significantly poorer physical functioning were observed among Saudi paramedics in comparison with Australian paramedics. However, Australian paramedics reported significantly poorer sleep quality and general health in comparison to Saudi paramedics. After removing the effect of driving and working durations, outcomes were no longer significant. The higher burden of depression and PTSD among Saudi paramedics may be explained by longer hours spent driving and longer work durations reported by this group. Taking into consideration the outcomes reported in this study, more investigations are needed to study their possible effects on paramedics' cognition, performance, and safety

    The Dynamic Universe: Realizing the Potential of Classical Time Domain and Multimessenger Astrophysics

    In parallel with the multi-messenger revolution, major advances in time-domain astronomy across multiple science disciplines relevant to astrophysics are becoming more urgent to address. Aside from electromagnetic observations of gravitational wave events and explosive counterparts, there are a number of “classical” astrophysical areas that require new thinking for proper exploration in the time domain. How NASA, NSF, ESA, and ESO consider the 2020 USA Decadal Survey within the astronomy community, as well as the worldwide call to support and expand time domain and multi-messenger astrophysics, it is crucial that all areas of astrophysics, including stellar, galactic, Solar System, and exoplanetary science participate in the discussion, and that it not be made into an exclusive preserve of cosmological, high-energy, explosive and transient science. Time domain astronomy is used to explore many aspects of astrophysics–particularly concerning ground- and space-based mission science goals of exploring how the Universe works, understanding how did we get here, and are we alone. Time domain studies are already built into the core operations of many currently operating and future space telescopes (e.g., Roman, PLATO) as well as current and planned large areal ground-based surveys (e.g., Rubin). Time-domain observations designed for one scientific purpose, also lead to great discoveries in many other science areas. The recent advent of user-friendly hardware, software, observational approaches, and online data infrastructure has also helped make time domain observations especially suitable and appealing for citizen science projects. We provide a review of the current state of TDAMM alerts and observational protocols, revealing a wide array of software and applications, much of which is incompatible. Any conversation regarding TDAMM astrophysics should include all aspects of the field, including those aspects seen as classical applications
