Żylna choroba zakrzepowo-zatorowa i leczenie przeciwzakrzepowe w COVID-19
Szczególną cechą COVID-19, głównie o ciężkim przebiegu, jest zwiększona skłonność do występowaniaepizodów żylnej choroby zakrzepowo-zatorowej (ŻChZZ). Dość często przybieraona postać izolowanej zatorowości płucnej i bywa przyczyną nagłego zgonu chorych. EpizodyŻChZZ obserwowano pomimo standardowej profilaktyki przeciwzakrzepowej, dlatego zasadniczeznaczenie w przebiegu COVCID-19 ma dobór właściwej dawki leków stosowanych w profilaktycei leczeniu ŻChZZ. Lekami z wyboru są heparyny drobnocząsteczkowe (HDCz). W wielurandomizowanych badaniach klinicznych porównywano dawki profilaktyczne, pośrednie i leczniczeHDCz stosowane w profilaktyce ŻChZZ u chorych, zależnie od ciężkości przebiegu choroby.Niniejsze opracowanie przedstawia bieżące zalecenia grupy ekspertów International Societyon Thrombosis and Hememostasis na temat profilaktyki ŻChZZ w przebiegu COVID-19, którepowstały na podstawie wyników dostępnych obecnie badań klinicznych
Inhibitory czynnika XI — przełom w profilaktyce i leczeniu przeciwzakrzepowym?
W artykule omówiono dotychczasowe badania kliniczne nad zastosowaniem inhibitorówczynnika XI w profilaktyce żylnej choroby zakrzepowo-zatorowej i zakrzepicy tętniczej
Venous thromboembolism and antithrombotic therapy in COVID-19
A distinctive feature of critically ill COVID-19 patients is higher susceptibility to venous thromboembolism (VTE) often presenting as isolated pulmonary embolism that may lead to sudden death. VTE episodes have been observed to occur despite standard prophylactic anticoagulation. It is therefore of crucial importance to choose the appropriate drug dose for therapeutic and prophylactic antithrombotic therapy in patients with COVCID-19. The drugs of choice here are low molecular weight heparins (LMWHs). Several randomized clinical trials have compared the efficacy of prophylactic, intermediate and therapeutic dose LMWH in VTE prophylaxis in various clinical settings. The paper presents the most recent International Society on Thrombosis and Hemostasis expert recommendations for anticoagulant prophylaxis in COVID-19. The recommendations were based on the outcome of recent clinical trials
Factor XI inhibitors — a breakthrough in prevention and treatment of thromboembolic disorders
Available pharmacological antithrombotic prophylaxis is associated with a low but noticeable risk of bleeding. Hence, there is an ongoing search for effective antithrombotic medications which do not significantly increase bleeding risk. Factor XI inhibitors seem to fulfil these requirements. Results of presently available randomized clinical trials on the use of factor XI inhibitors in venous thromboembolism prophylaxis and prevention of arterial thrombosis are presented
Zasadniczym celem pracy jest szczegółowe przedstawienie nowych kryteriów klasyfikacyjnych APS, opracowanych przez American College of Rheumatology (ACR) i Alliance of Associations for Rheumatology (EULAR).
Increased expression of selected very late antigen integrin subunits on CD4 and CD8 blood T lymphocytes in patients with clinically stable asymptomatic atopic asthma
Recruitment of the inflammatory cells from blood to the airways in asthma is mediated by adhesive
molecules, e.g. selectins and integrins. The most important integrins in cells trafficking are molecules containing
and subunits. We hypothesized that also and integrins (both found by us on blood eosinophils. To assess the expression of selected very late antigen (VLA) subunits (, , and ) on blood CD4 and CD8
T lymphocytes from stable atopic asthmatic patients.
of asthmatic subjects) are important in asthma pathogenesis. The study was conducted on 25 adult atopic asthmatics (mild to moderate persistent asthma in a stable clinical condition) and 17 matched healthy controls using flow cytometry. Expression of and on CD4 T cells was significantly higher in asthma than in controls. The subunit
was absent from blood lymphocytes. The chain hardly detected on lymphocytes from healthy subjects was distinctively present in asthmatics. Surprisingly, in subjects suffering from asthma for longer than 4 years (n = 15),
the overexpression of , and was observed on both: CD4 and CD8 T cells. Expression of selected VLA subunits on blood T cells may depend on asthma duration. The biological
role of integrin in asthma is unknown, but as it was described as a stimulator of collagen accumulation in
the airways, integrin could be, at least in part, responsible for asthma airway remodelling
