Comparative efficacy of long-acting bronchodilators for COPD - a network meta-analysis
Background: Clinicians are faced with an increasingly difficult choice regarding the optimal bronchodilator for
patients with chronic obstructive pulmonary disease (COPD) given the number of new treatments. The objective of this study is to evaluate the comparative efficacy of indacaterol 75/150/300 ÎĽg once daily (OD), glycopyrronium
bromide 50 ÎĽg OD, tiotropium bromide 18 ÎĽg/5 ÎĽg OD, salmeterol 50 ÎĽg twice daily (BID), formoterol 12 ÎĽg BID,
and placebo for moderate to severe COPD.
Methods: Forty randomized controlled trials were combined in a Bayesian network meta-analysis. Outcomes of interest were trough and post-dose forced expiratory volume in 1 second (FEV1), St. George’s Respiratory Questionnaire (SGRQ) score and responders (≥4 points), and Transition Dyspnea Index (TDI) score and responders
(≥1 point) at 6 months.
Results: Indacaterol was associated with a higher trough FEV1 than other active treatments (difference for indacaterol 150 ÎĽg and 300 ÎĽg versus placebo: 152 mL (95% credible interval (CrI): 126, 179); 160 mL (95% CrI:
133, 187)) and the greatest improvement in SGRQ score (difference for indacaterol 150 ÎĽg and 300 ÎĽg versus
placebo: -3.9 (95% CrI -5.2, -2.6); -3.6 (95% CrI -4.8, -2.3)). Glycopyrronium and tiotropium 18 ÎĽg resulted in the
next best estimates for both outcomes with minor differences (difference for glycopyrronium versus tiotropium
for trough FEV1 and SGRQ: 18 mL (95% CrI: -16, 51); -0.55 (95% CrI: -2.04, 0.92).
Conclusion: In terms of trough FEV1 and SGRQ score indacaterol, glycopyrronium, and tiotropium are expected to
be the most effective bronchodilators
LHC Searches for Non-Chiral Weakly Charged Multiplets
Because the TeV-scale to be probed at the Large Hadron Collider should shed
light on the naturalness, hierarchy, and dark matter problems, most searches to
date have focused on new physics signatures motivated by possible solutions to
these puzzles. In this paper, we consider some candidates for new states that
although not well-motivated from this standpoint are obvious possibilities that
current search strategies would miss. In particular we consider vector
representations of fermions in multiplets of with a lightest neutral
state. Standard search strategies would fail to find such particles because of
the expected small one-loop-level splitting between charged and neutral states.Comment: 16 pages, 9 figure

Antipsychotic dose escalation as a trigger for Neuroleptic Malignant Syndrome (NMS): literature review and case series report
Background: “Neuroleptic malignant syndrome” (NMS) is a potentially fatal idiosyncratic reaction to any medication which affects the central dopaminergic system. Between 0.5% and 1% of patients exposed to antipsychotics develop the condition. Mortality rates may be as high as 55% and many risk factors have been reported. Although rapid escalation of antipsychotic dose is thought to be an important risk factor, to date it has not been the focus of a published case series or scientifically defined.
<p/>Aims: To identify cases of NMS and review risk factors for its development with a particular focus on rapid dose escalation in the 30 days prior to onset.
<p/>Methodology: A review of the literature on rapid dose escalation was undertaken and a pragmatic definition of “rapid dose escalation” was made. NMS cases were defined using DSM-IV criteria and systematically identified within a secondary care mental health service. A ratio of titration rate was calculated for each NMS patient and “rapid escalators” and “non rapid escalators” were compared.
<p/>Results: 13 cases of NMS were identified. A progressive mean dose increase 15 days prior to the confirmed episode of NMS was observed (241.7mg/day during days 1-15 to 346.9mg/day during days 16-30) and the mean ratio of dose escalation for NMS patients was 1.4. Rapid dose escalation was seen in 5/13 cases and non rapid escalators had markedly higher daily cumulative antipsychotic dose compared to rapid escalators.
<p/>Conclusions: Rapid dose escalation occurred in less than half of this case series (n=5, 38.5%), although there is currently no consensus on the precise definition of rapid dose escalation. Cumulative antipsychotic dose – alongside other known risk factors - may also be important in the development of NMS
Strengthening confidence in climate change impact science
Aim: To assess confidence in conclusions about climate-driven biological change through time, and identify approaches for strengthening confidence scientific conclusions about ecological impacts of climate change. Location: Global. Methods: We outlined a framework for strengthening confidence in inferences drawn from biological climate impact studies through the systematic integration of prior expectations, long-term data and quantitative statistical procedures. We then developed a numerical confidence index (Cindex) and used it to evaluate current practices in 208 studies of marine climate impacts comprising 1735 biological time series. Results: Confidence scores for inferred climate impacts varied widely from 1 to 16 (very low to high confidence). Approximately 35% of analyses were not associated with clearly stated prior expectations and 65% of analyses did not test putative non-climate drivers of biological change. Among the highest-scoring studies, 91% tested prior expectations, 86% formulated expectations for alternative drivers but only 63% statistically tested them. Higher confidence scores observed in studies that did not detect a change or tracked multiple species suggest publication bias favouring impact studies that are consistent with climate change. The number of time series showing climate impacts was a poor predictor of average confidence scores for a given group, reinforcing that vote-counting methodology is not appropriate for determining overall confidence in inferences. Main conclusions: Climate impacts research is expected to attribute biological change to climate change with measurable confidence. Studies with long-term, high-resolution data, appropriate statistics and tests of alternative drivers earn higher Cindex scores, suggesting these should be given greater weight in impact assessments. Together with our proposed framework, the results of our Cindex analysis indicate how the science of detecting and attributing biological impacts to climate change can be strengthened through the use of evidence-based prior expectations and thorough statistical analyses, even when data are limited, maximizing the impact of the diverse and growing climate change ecology literature
Prioritisation of Clinical Research by the Example of Type 2 Diabetes: A Caregiver-Survey on Perceived Relevance and Need for Evidence
BACKGROUND: The Cochrane Collaboration aims at providing the best available evidence for interventions in health care. We wished to examine to which extent treatments considered relevant by caregivers in type 2 diabetes are covered by Cochrane systematic reviews. METHODOLOGY/PRINCIPAL FINDINGS: 130 different interventions in type 2 diabetes were identified based on a review of clinical practice guidelines and expert opinion (Table S1). 459 members of the German Diabetes Society (diabetologists, general practitioners, diabetic nurses, nutritionists, podologists, others) were surveyed via e-mail-list to rank a) the perceived clinical relevance and b) the perceived need for evidence of interventions, based on an internet survey. In the Cochrane Library, there were, at the time of this evaluation, 56 reviews on interventions in diabetes. Generally, coverage of topics by Cochrane reviews reflected the perceived clinical relevance and perceived need for evidence. As an example, highly ranked treatments such as lifestyle changes or oral antidiabetics were well covered, while low rank treatments such as complementary approaches were not covered. Discrepancies occurred with new treatments such as amylin-analogues (low relevance, high need for evidence, review not yet completed) and interventions with immediate and dramatic effects such as treating hypoglycemia (high relevance, low need for evidence, no review). Also, there was a relative scarcity of reviews concerning specific problems, in particular, treatment of late diabetic complications. CONCLUSIONS/SIGNIFICANCE: For most interventions, perceived relevance and perceived need for evidence are reflected by the evidence already available. Prioritizing should aim at improving immediacy and consideration of the treatment of complications
Spread Supersymmetry
In the multiverse the scale of SUSY breaking, \tilde{m} = F_X/M_*, may scan
and environmental constraints on the dark matter density may exclude a large
range of \tilde{m} from the reheating temperature after inflation down to
values that yield a LSP mass of order a TeV. After selection effects, the
distribution for \tilde{m} may prefer larger values. A single environmental
constraint from dark matter can then lead to multi-component dark matter,
including both axions and the LSP, giving a TeV-scale LSP lighter than the
corresponding value for single-component LSP dark matter.
If SUSY breaking is mediated to the SM sector at order X^* X, only squarks,
sleptons and one Higgs doublet acquire masses of order \tilde{m}. The gravitino
mass is lighter by a factor of M_*/M_Pl and the gaugino masses are suppressed
by a further loop factor. This Spread SUSY spectrum has two versions; the
Higgsino masses are generated in one from supergravity giving a wino LSP and in
the other radiatively giving a Higgsino LSP. The environmental restriction on
dark matter fixes the LSP mass to the TeV domain, so that the squark and
slepton masses are order 10^3 TeV and 10^6 TeV in these two schemes. We study
the spectrum, dark matter and collider signals of these two versions of Spread
SUSY. The Higgs is SM-like and lighter than 145 GeV; monochromatic photons in
cosmic rays arise from dark matter annihilations in the halo; exotic short
charged tracks occur at the LHC, at least for the wino LSP; and there are the
eventual possibilities of direct detection of dark matter and detailed
exploration of the TeV-scale states at a future linear collider. Gauge coupling
unification is as in minimal SUSY theories.
If SUSY breaking is mediated at order X, a much less hierarchical spectrum
results---similar to that of the MSSM, but with the superpartner masses 1--2
