    Post-Orchiectomyanaemia in patients with advanced carcinoma of the prostate

    To study postorchiectomy anaemia in black Nigerian patients who had bilateral orchiectomy for advanced carcinoma of the prostate. Anaemia has been observed in similar studies done on Caucasians and non-black patients in non-tropical environments : Consecutive patients with advanced carcinoma of the prostate who were managed by the authors over a four-year period, and who were treated with bilateral orchiectomy were prospectively studied.They also satisfied the set criteria for inclusion in the study.Thirty-six patients were studied. The mean age was 71 years (SD=7.6). Preoperative white blood cell count with blood film examination, and serum urea and creatinine were within normal limits and remained so for the duration of the study.     Correlation between prostate specific antigen, digital rectal examination and histology in patients with prostate cancer

    Introduction: Prostate cancer remains a health concern worldwide with an increasing global incidence. In Nigerian men it is the most common diagnosed cancer. Diagnosis of prostate cancer is made through biopsy and histology which in turn is dependent on prostate specific antigen and digital rectal examination finding.Objective: This study sought to look at the correlation between PSA , DRE and histology in patient who had prostate biopsy: (This objective is slightly at variance with what the title of the work says)Method: It was a prospective study of all patients who presented to our clinic and had prostate biopsy. Data on age of patient, size of prostate, PSA, DRE finding of benign or suspicious for cancer of the prostate and the final histology were collated and there correlation analysed using SPSS and Microsft Excel 2013.Results: The mean age, prostate volume and PSA were 70.99+ 9.1years, 97.6+ 88.1ml and 70.13 + 73.2ng/ml respectively. The positive predictive value, negative predictive value and overall diagnostic accuracy are 55.61, 66.67, 55.77 respectively for PSA above 4ng/ml , 71.97, 73.68, 72.60 respectively for DRE alone and 55.59, 0.00, 55.29 respectively for a combination of PSA above 4ng/ml and DRE.Conclusion: PSA and DRE singly or in combination have a poor PPV (including PPV of 55.61 ), NPV and ODA to help counseling of patients prior to prostate biopsy.Keyword: PSA, DRE, biopsy histology, diagnostic accurac

    Accuracy of Prader orchidometer in measuring testicular volume

    Background: Seminiferous tubules comprise 80ā€‘90% of testicular mass. Thus, the testicular volume is believed to be an index of spermatogenesis. Therefore, accurate testicular volume is one way to assess testicular function.Objective: To determine the accuracy of Prader orchidometer for measuring the testicular volume by comparing the resultant measurement with the actual testicular volume in humans.Materials and Methods: The testicular volumes of 121 testes from 62 patients with prostate cancer (mean age 72.74 Ā± 9.38 years) were measured using Prader orchidometer before therapeutic bilateral orchidectomy. The actual testicular volumes were then determined by water displacement of the testis.Results: The mean testicular volume of the 121 testes was 10.60 Ā± 3.5 ml and13.26 Ā± 5.2 ml for water displacement and Prader orchidometer measurements, respectively. A strong correlation was found between the actual testicular volume and volumes obtained by Prader orchidometer (r = 0.926, P = 0.0001). The Prader orchidometer however, overā€‘estimated the mean actual testicular volume by 2.66 Ā± 2.37 ml (25.10%).Conclusion: The result of this study has shown that measuring the testicular volume by Prader orchidometer overestimates the actual testicular volume.Key words: Accuracy, prader orchidometer, testicular volum

    Right colon interposition in corrosive esophageal long segment stricture: Our local experience

    Background: Aim of our study is to highlight technical details in pedicled right colon interposition locally in cases of long segment corrosiveĀ  esophageal stricture. Lesion results from cicatrization of burns wound inflicted by chemicals. Restoration of swallowing is of paramount importance.Materials and Methods: It was a retrospective study involving adult and adolescent patients who had surgery because of corrosive esophageal stricture between January 2001 and December 2010. Hospital records were reviewed for age, sex, radiological findings, detail of anesthesia, operative procedure, and follow up. The steps which included mobilization of colon with intact marginal blood supply, safeguarding the middle colic artery that forms the pedicle, and doing three anastomoses that were leak proof were ascertained.Result: There were 12 males (70.5%) and six females (29.4%) and age ranged from 14 to 45 years. Caustic soda caused the problem among 10 (58.8%) patients while liquid acid battery was involved in three (17.6%) patients. The rest were distributed equally between native concoction and unknown substance. All were done under general anesthesia and mean duration of the procedure was 306.8 ± 54.6 min with an average intra operative blood loss of 823.1 ± 428.5 m. Sixteen (94.1%) had successful pedicle isolation and 15 (82.3%) patients had neither dysphagia nor reflux at follow.up.Conclusion: Securing a pedicled right colon based on middle colic artery was a critical step. This surgical technique should be used to restoreĀ  swallowing among patients with long segment esophageal stricture.     ā€œOversizedā€ Penile Length In The Black People; Myth Or Reality

    Objective: Amongst the Caucasians and some non-Black races, there is the popular belief that the black people generally have longer penile sizes. Even the Blacks themselves tend to believe so. This study was carried out to test the veracity of this ascription of ā€œoversizedā€ penile length to people of the Black (Negro) race. Design: A prospective and comparative study Setting: The Nnamdi Azikiwe University Teaching Hospital, Nnewi, Nigeria. Subjects and Method: Full-stretch flaccid penile lengths, and flaccid penile lengths, were measured in 115 adult men of the Black race in Nigeria. The results were compared with reported similar main studies on people of other races, which were accessible to the authors. These studies were done in Italy, Greece, Korea, Britain, and the United States of America. Result: The mean full-stretch penile length of the Nigerian Blacks was 13.37cm and the mean flaccid length was 9.36cm. Similar studies reported full-stretch penile lengths of 12.50cm in Italians, 12.18cm in Greeks, 9.6cm in Koreans, 13cm in British Caucasians, and 12.45cm in the American Caucasians. The penile length for Nigerian Blacks was longer than those of the other races, but the differences were only statistically different in comparison with the Koreans.     Genitourinary tuberculosis in Nigeria; a review of thirty-one cases

    Objective: To report and discuss the characteristics of genitourinary tuberculosis as observed in a sub-Saharan African setting, where reports suggest its rarity despite reported high incidence and prevalence of pulmonary tuberculosis.Patients and Method: 31 consecutive patients with discharge diagnoses of genitourinary tuberculosis seen in a small community-based hospital over a five-year period were retrospectively studied.Result: There were 28 males and three females, with a mean age of 37.6 (SD 11.5) years. Forty-three sites were involved in seven organs: epididymis 25 (58.1%), kidney seven (16.3%), testis four (9.3%), bladder three (7.0%), ureter two (4.7%), prostate one (2.3%), and cord of the testis one (2.3%). The commonest presenting features were scrotal/testicular mass with or without pain/tenderness (80.6%), fever/headache (51.6%), and dysuria (22.6%). Other common features were back, loin, or abdominal pain/tenderness, hydrocoele, scrotal abscess, and haematuria. 26.9% had evidence of concurrent or previous pulmonary tuberculosis, and 9.1% were positive on HIV 1 & 11 screening.Conclusion: With the prevailing conditions in sub-Saharan Africa and most of the developing world and the slightly different characteristics of the disease in our environment, diagnosis of genito-urinary tuberculosis may be difficult. It is advised that patients with unexplained symptoms in the urinary tract should be investigated for tuberculosis.     Emergency transvesical prostatectomy: A review of fifty cases

    Objective: To retrospectively study patients with benign prostatic enlargement who had emergency transvesical prostatectomy in order to determine the patterns in their presentation, management and outcome. This may guide future decisions in the management of such cases in a resource poor environment.Patients and Method: Fifty patients who presented to the authors with benign prostatic enlargement and who had emergency transvesical prostatectomy over a fifteen-year period were studied retrospectively. The data collected included age, indications for surgery, associated medical conditions, investigations done, perioperative blood transfusions, weights and configurations of the enucleated prostate adenomata, duration of postoperative hospital stay, and postoperative complications.Result: The indications for the emergency transvesical prostatectomy were severe prostatic haematuria which was refractory, and recalcitrant clot and urinary retention from prostatic haematuria. The peri-operative blood transfusion was 3.40 (SD 0.49) units per patient. There were postoperative complications in 40% of patients, and 62% had associated medical conditionsConclusion: Emergency transvesical prostatectomy is expedient when precisely indicated and properly done.
