A Rydberg Quantum Simulator
Following Feynman and as elaborated on by Lloyd, a universal quantum
simulator (QS) is a controlled quantum device which reproduces the dynamics of
any other many particle quantum system with short range interactions. This
dynamics can refer to both coherent Hamiltonian and dissipative open system
evolution. We investigate how laser excited Rydberg atoms in large spacing
optical or magnetic lattices can provide an efficient implementation of a
universal QS for spin models involving (high order) n-body interactions. This
includes the simulation of Hamiltonians of exotic spin models involving
n-particle constraints such as the Kitaev toric code, color code, and lattice
gauge theories with spin liquid phases. In addition, it provides the
ingredients for dissipative preparation of entangled states based on
engineering n-particle reservoir couplings. The key basic building blocks of
our architecture are efficient and high-fidelity n-qubit entangling gates via
auxiliary Rydberg atoms, including a possible dissipative time step via optical
pumping. This allows to mimic the time evolution of the system by a sequence of
fast, parallel and high-fidelity n-particle coherent and dissipative Rydberg

Mining and analysis of audiology data to find significant factors associated with tinnitus masker
Objectives: The objective of this research is to find the factors associated with tinnitus masker from the literature, and by using the large amount of audiology data available from a large NHS (National Health Services, UK) hearing aid clinic. The factors evaluated were hearing impairment, age, gender, hearing aid type, mould and clinical comments.
Design: The research includes literature survey for factors associated with tinnitus masker, and performs the analysis of audiology data using statistical and data mining techniques.
Setting: This research uses a large audiology data but it also faced the problem of limited data for tinnitus.
Participants: It uses 1,316 records for tinnitus and other diagnoses, and 10,437 records of clinical comments from a hearing aid clinic.
Primary and secondary outcome measures: The research is looking for variables associated with tinnitus masker, and in future, these variables can be combined into a single model to develop a decision support system to predict about tinnitus masker for a patient.
Results: The results demonstrated that tinnitus maskers are more likely to be fit to individuals with milder forms of hearing loss, and the factors age, gender, type of hearing aid and mould were all found significantly associated with tinnitus masker. In particular, those patients having Age<=55 years were more likely to wear a tinnitus masker, as well as those with milder forms of hearing loss. ITE (in the ear) hearing aids were also found associated with tinnitus masker. A feedback on the results of association of mould with tinnitus masker from a professional audiologist of a large NHS (National Health Services, UK) was also taken to better understand them. The results were obtained with different accuracy for different techniques. For example, the chi-squared test results were obtained with 95% accuracy, for Support and Confidence only those results were retained which had more than 1% Support and 80% Confidence.
Conclusions: The variables audiograms, age, gender, hearing aid type and mould were found associated with the
choice of tinnitus masker in the literature and by using statistical and data mining techniques. The further work in this research would lead to the development of a decision support system for tinnitus masker with an explanation that how that decision was obtained
Microscopic observation of magnon bound states and their dynamics
More than eighty years ago, H. Bethe pointed out the existence of bound
states of elementary spin waves in one-dimensional quantum magnets. To date,
identifying signatures of such magnon bound states has remained a subject of
intense theoretical research while their detection has proved challenging for
experiments. Ultracold atoms offer an ideal setting to reveal such bound states
by tracking the spin dynamics after a local quantum quench with single-spin and
single-site resolution. Here we report on the direct observation of two-magnon
bound states using in-situ correlation measurements in a one-dimensional
Heisenberg spin chain realized with ultracold bosonic atoms in an optical
lattice. We observe the quantum walk of free and bound magnon states through
time-resolved measurements of the two spin impurities. The increased effective
mass of the compound magnon state results in slower spin dynamics as compared
to single magnon excitations. In our measurements, we also determine the decay
time of bound magnons, which is most likely limited by scattering on thermal
fluctuations in the system. Our results open a new pathway for studying
fundamental properties of quantum magnets and, more generally, properties of
interacting impurities in quantum many-body systems.Comment: 8 pages, 7 figure

[email protected] OBJECTIVES: Screening for type 2 diabetes has been recommended and targeted screening might be an efficient way to screen. The aim was to investigate whether diabetic patients identified by a targeted screening procedure differ from newly diagnosed diabetic patients in general practice with regard to the prevalence of macrovascular complications. DESIGN: Cross-sectional population-based study. SETTING: Population study, primary care. SUBJECTS: Diabetic patients identified by a population-based targeted screening procedure (SDM patients), consisting of a screening questionnaire and a fasting capillary glucose measurement followed by diagnostic testing, were compared with newly diagnosed diabetic patients in general practice (GPDM patients). Ischaemic heart disease and prior myocardial infarction were assessed by ECG recording. Peripheral arterial disease was assessed by the ankle-arm index. Intima-media thickness of the right common carotid artery was measured with ultrasound. RESULTS: A total of 195 SDM patients and 60 GPDM patients participated in the medical examination. The prevalence of MI was 13.3% (95% CI 9.3-18.8%) and 3.4% (1.0-11.7%) in SDM patients and GPDM patients respectively. The prevalence of ischaemic heart disease was 39.5% (95% CI 32.9-46.5%) in SDM patients and 24.1% (15.0-36.5%) in GPDM patients. The prevalence of peripheral arterial disease was similar in both groups: 10.6% (95% CI 6.9-15.9%) and 10.2% (4.7-20.5%) respectively. Mean intima-media thickness was 0.85 mm (+/-0.17) in SDM patients and 0.90 mm (+/-0.20) in GPDM patients. The difference in intima-media thickness was not statistically significant. Two-orbital SU(N) magnetism with ultracold alkaline-earth atoms
Fermionic alkaline-earth atoms have unique properties that make them
attractive candidates for the realization of novel atomic clocks and degenerate
quantum gases. At the same time, they are attracting considerable theoretical
attention in the context of quantum information processing. Here we demonstrate
that when such atoms are loaded in optical lattices, they can be used as
quantum simulators of unique many-body phenomena. In particular, we show that
the decoupling of the nuclear spin from the electronic angular momentum can be
used to implement many-body systems with an unprecedented degree of symmetry,
characterized by the SU(N) group with N as large as 10. Moreover, the interplay
of the nuclear spin with the electronic degree of freedom provided by a stable
optically excited state allows for the study of spin-orbital physics. Such
systems may provide valuable insights into strongly correlated physics of

Intra-gastric balloon as an adjunct to lifestyle support in severely obese adolescents; Impact on weight, physical activity, cardio-respiratory fitness and psychosocial wellbeing.
BACKGROUND: Severe adolescent obesity (BMI>99.6th centile) is a significant public health challenge. Current non-invasive treatments, including community-based lifestyle interventions, are often of limited effectiveness in this population, with NICE guidelines suggesting the use of bariatric surgery as the last line of treatment (NICE, 2013). Health professionals are understandably reluctant to commission bariatric surgery and as an alternative, the use of an intra-gastric balloon as an adjunct to a lifestyle programme might offer a reversible, potentially safer and less invasive option.
OBJECTIVES: Explore the use of an intra-gastric balloon as an adjunct to a lifestyle support programme, to promote weight loss in severely obese adolescents. Outcomes included Weight loss, Waist and Hip measurements, psychosocial outcomes including health related quality of life and physical self-perceptions, physical activity and cardiorespiratory fitness.
METHOD: Non-randomised pilot study. Results: 12 severely obese adolescents (5 males, 7 females; mean age 15yrs; BMI >3.5 s.d.; puberty stage 4 or more) and their families were recruited. Mean weight loss at 12 months (n=9) was 3.05 kg±14.69; d=0.002, P=0.550, and a BMI Z-score (n=12) change of 0.2 s.d.; d=0.7, P=0.002 was observed at 6 months with a large effect, but was not sustained at 12 months (mean change 0.1 s.d.; d=0.3, P=0.146 ) At 24 months (n=10) there was a weight gain from baseline of +9.9 kg±1.21 (d=0.4; P=0.433). Adolescent and parent HRQoL scores exceeded the minimal clinical important difference between baseline and 12 months for all domains but showed some decline at 24 months.
CONCLUSION: An intra-gastric balloon as an adjunct to a lifestyle support programme represents a safe and well tolerated treatment approach in severely obese adolescents, with short-term effects on weight change. Improvements in psychosocial health, physical activity and cardiorespiratory fitness were maintained at 12 months, with varying results at 24 months
Overview of data-synthesis in systematic reviews of studies on outcome prediction models
Background: Many prognostic models have been developed. Different types of models, i.e. prognostic factor and outcome prediction studies, serve different purposes, which should be reflected in how the results are summarized in reviews. Therefore we set out to investigate how authors of reviews synthesize and report the results of primary outcome prediction studies. Methods: Outcome prediction reviews published in MEDLINE between October 2005 and March 2011 were eligible and 127 Systematic reviews with the aim to summarize outcome prediction studies written in English were identified for inclusion.
Characteristics of the reviews and the primary studies that were included were independently assessed by 2 review authors, using standardized forms. Results: After consensus meetings a total of 50 systematic reviews that met the inclusion criteria were included. The type of primary studies included (prognostic factor or outcome prediction) was unclear in two-thirds of the reviews. A minority of the reviews reported univariable or multivariable point estimates and measures of dispersion from the primary studies. Moreover, the variables considered for outcome prediction model development were often not reported, or were unclear. In most reviews there was no information about model performance. Quantitative analysis was performed in 10 reviews, and 49 reviews assessed the primary studies qualitatively. In both analyses types a range of different methods was used to present the results of the outcome prediction studies.
Conclusions: Different methods are applied to synthesize primary study results but quantitative analysis is rarely performed. The description of its objectives and of the primary studies is suboptimal and performance parameters of the outcome prediction models are rarely mentioned. The poor reporting and the wide variety of data synthesis strategies are prone to influence the conclusions of outcome prediction reviews. Psychiatric misdiagnoses in patients with chronic fatigue syndrome
OBJECTIVES: The aim of this study was to examine the accuracy of doctors at diagnosing co-morbid psychiatric disorders in patients with chronic fatigue syndrome (CFS). DESIGN: Case series comparing clinical diagnoses with a standardized structured psychiatric interview. SETTING: Secondary care specialist chronic fatigue syndrome clinic. PARTICIPANTS: One hundred and thirty-five participants of a randomized controlled trial of non-pharmacological treatments at one centre in the PACE trial. MAIN OUTCOME MEASURES: Current psychiatric diagnoses made by CFS specialist doctors, compared with current psychiatric diagnoses made independently using a structured psychiatric interview. RESULTS: Clinicians identified 59 (44%, 95% CI 39-56%) of patients as suffering from a co-morbid psychiatric disorder compared to 76 (56%, CI 53-69%) by structured interview. Depressive and anxiety disorders were most common. Clinicians were twice as likely to miss diagnoses (30 patients, 22%) than misdiagnose them (13, 10%). Psychiatrists were less likely to miss diagnoses than other clinicians, but were as likely to misdiagnose them. CONCLUSIONS: Doctors assessing patients in a chronic fatigue syndrome clinic miss psychiatric diagnoses more often than misdiagnosing them. Missed diagnoses are common. Variable selection under multiple imputation using the bootstrap in a prognostic study
Background: Missing data is a challenging problem in many prognostic studies. Multiple imputation
(MI) accounts for imputation uncertainty that allows for adequate statistical testing. We developed
and tested a methodology combining MI with bootstrapping techniques for studying prognostic
variable selection.
Method: In our prospective cohort study we merged data from three different randomized
controlled trials (RCTs) to assess prognostic variables for chronicity of low back pain. Among the
outcome and prognostic variables data were missing in the range of 0 and 48.1%. We used four
methods to investigate the influence of respectively sampling and imputation variation: MI only,
bootstrap only, and two methods that combine MI and bootstrapping. Variables were selected
based on the inclusion frequency of each prognostic variable, i.e. the proportion of times that the
variable appeared in the model. The discriminative and calibrative abilities of prognostic models
developed by the four methods were assessed at different inclusion levels.
Results: We found that the effect of imputation variation on the inclusion frequency was larger
than the effect of sampling variation. When MI and bootstrapping were combined at the range of
0% (full model) to 90% of variable selection, bootstrap corrected c-index values of 0.70 to 0.71 and
slope values of 0.64 to 0.86 were found.
Conclusion: We recommend to account for both imputation and sampling variation in sets of
missing data. The new procedure of combining MI with bootstrapping for variable selection, results
in multivariable prognostic models with good performance and is therefore attractive to apply on
data sets with missing values
