    Manifolds of classical probability distributions and quantum density operators in infinite dimensions

    The manifold structure of subsets of classical probability distributions and quantum density operators in infinite dimensions is investigated in the context of C∗-algebras and actions of Banach-Lie groups. Specificaly, classical probability distributions and quantum density operators may be both described as states (in the functional analytic sense) on a given C∗-algebraA which is Abelian for Classical states, and non-Abelian for Quantum states. In this contribution, the space of states S of a possibly infinitedimensional, unital C∗-algebra A is partitioned into the disjoint union of the orbits of an action of the group G of invertible elements of A . Then, we prove that the orbits through density operators on an infinite-dimensional, separable Hilbert space H are smooth, homogeneous Banach manifolds of G = GL(H), and, when A admits a faithful tracial state τ like it happens in the Classical case when we consider probability distributions with full support, we prove that the orbit through τ is a smooth, homogeneous Banach manifold for G .A.I. and G.M. acknowledge financial support from the Spanish Ministry of Economy and Competitiveness, through the Severo Ochoa Programme for Centres of Excellence in RD (SEV-2015/0554).     Soluble and insoluble dietary fibre intake and risk factors for metabolic syndrome and cardiovascular disease in middle-aged adults: The AWHS cohort

    Introduction: The Westernization of the Mediterranean lifestyle has led to a modification of certain dietary habits such as a decrease in the consumption of dietary fibre-rich foods. The impact of these changes on cardiovascular diseases (CVD) has been studied over the last few years and the effect of the different sources of fibre on cardiovascular risk parameters and coronary heart disease (CHD) continues to create controversy. Objective: To evaluate the association between the source of dietary fibre and the prevalence of metabolic syndrome (MetS) and other cardiovascular risk factors in a Spanish working population. Subjects and methods: The study was carried out in a sample of 1592 Spanish workers free of CVD (40-55 years old) within the Aragon Workers' Health Study (AWHS) cohort. Sociodemographic, anthropometric, clinical and biochemical data were collected. Fibre intake was assessed by means of a validated 136-items semiquantitative food-frequency questionnaire. MetS was defined by using the modified National Cholesterol Education Programme-Adult Treatment Panel III (NCEP-ATP III) definition. Results: After adjusting for possible confounding factors, we found an inverse association between insoluble fibre intake and systolic and diastolic blood pressure, total cholesterol, triglycerides, apolipoprotein B100 and ratio TG/HDL. Soluble fibre was inversely associated with triglycerides and apolipoprotein B100. Furthermore, prevalence of MetS was found to be lower (OR 0.62, 95% CI: 0.40-0.96) in those participants in the highest quartile of insoluble fibre intake. Conclusion: A higher intake of insoluble fibre could play an important role in the control and management of hypertension, lipid profile and MetS. Introducción: La occidentalización del estilo de vida mediterráneo ha dado lugar a una modificación de ciertos hábitos dietéticos, tales como una disminución en el consumo de alimentos ricos en fibra dietética. El impacto de estos cambios sobre las enfermedades cardiovasculares (ECV) se ha estudiado en los últimos años y el efecto de las diferentes fuentes de fibra en los parámetros de riesgo cardiovascular y en la enfermedad coronaria sigue creando controversia. Objetivo: Evaluar la asociación entre la fuente de fibra dietética y la prevalencia de síndrome metabólico (SM) y otros factores de riesgo cardiovascular en una población laboral española. Sujetos y métodos: El estudio se llevó a cabo en una muestra de 1592 trabajadores españoles libres de ECV (40-55 años) pertenecientes a la cohorte del Estudio de la Salud de los Trabajadores de Aragón (AWHS). Se recogieron datos sociodemográficos, antropométricos, clínicos y bioquímicos. La ingesta de fibra se evaluó por medio de un cuestionario semicuantitativo de frecuencia de consumo de alimentos de 136-items previamente validado. Para la definición de SM se siguieron los criterios del Programa Nacional de Educación del Colesterol en el marco del III Panel de Tratamiento de Adultos (NCEPATP III). Resultados: Se encontró una asociación inversa entre el consumo de fibra insoluble y la presión arterial sistólica y diastólica, colesterol total, triglicéridos, apolipoproteína B100 y la relación TG/HDL, tras ajustar por posibles factores de confusión. Así mismo, la fibra soluble se asoció inversamente con triglicéridos y apolipoproteína B100. Además, se encontró una menor prevalencia de SM (OR 0.62, IC del 95%: 0.40 a 0.96) en aquellos participantes en el cuartil más alto de consumo de fibra insoluble. Conclusión: Una mayor ingesta de fibra insoluble puede desempeñar un papel importante en el control y manejo de la hipertensión, el perfil lipídico y el SM

    Towards multi-tenant cache management for ISP networks

    The decreasing cost of storage and the advent of virtualization technology can allow Internet Service Providers (ISPs) to deploy multi-tenant caching infrastructures and lease them to content producers and Content Delivery Networks (CDNs). Serving content requests directly from the ISP network does not only reduce the delivery time, but also allows the ISP to optimize the network resources by controlling the placement and routing of content items. In this paper, we introduce a multi-tenant cache management approach that significantly reduces the bandwidth utilization of ISPs networks by pro-actively allocating caching space, leased by content producers and/or CDNs, and intelligently routing content to the end users. Using real content request traces, we show that the optimal solution to this problem can increase the cache hit ratio by 70.64% while reducing the bandwidth usage by 57.17% on average, compared to a commonly used reactive cache management scheme. These results provide a benchmark for the development of novel multi-tenant cache management strategies

    Spatial Data Infrastructures for environmental e-government services: The case of water abstractions authorisations

    Environment-related authorisations are a relevant issue for environmental management. They require a considerable effort by the authorities, and this might result in substantial delays for the citizens. Implementing those authorisation processes by means of e-government services would improve efficiency and, consequently, citizen satisfaction. Environment-related authorisations usually require a variety of geospatial information, and have to deal with administrative areas which do not match physical and ecological ones. They also have to integrate heterogeneous information in different formats, data models and languages, and provided by distinct organisations, even from different countries. This paper discusses how Spatial Data Infrastructures (SDIs) can deal with these problems in the environmental domain, while improving the level of service provision in terms of e-government applications. This is even more relevant within the European Union where there is a legal mandate to establish an SDI to support environmental policies and activities with an impact on the environment. As a proof-of-concept, an application to request and manage water abstraction authorisations, based on an SDI, is demonstrated. This application is part of SDIGER, a cross-border inter-administration SDI to support the water framework directive information access for the Adour–Garonne and Ebro River basins, that was a pilot project for the EU INSPIRE Directive. The introduction of this transactional e-government service modifies the administrative process of granting authorisations: it allows to re-use the effort in data capture made by the applicants in their requests, facilitates the submission of more feasible applications and reduces the workload of the office staff

    Proactive Multi-tenant Cache Management for Virtualized ISP Networks

    The content delivery market has mainly been dominated by large Content Delivery Networks (CDNs) such as Akamai and Limelight. However, CDN traffic exerts a lot of pressure on Internet Service Provider (ISP) networks. Recently, ISPs have begun deploying so-called Telco CDNs, which have many advantages, such as reduced ISP network bandwidth utilization and improved Quality of Service (QoS) by bringing content closer to the end-user. Virtualization of storage and networking resources can enable the ISP to simultaneously lease its Telco CDN infrastructure to multiple third parties, opening up new business models and revenue streams. In this paper, we propose a proactive cache management system for ISP-operated multitenant Telco CDNs. The associated algorithm optimizes content placement and server selection across tenants and users, based on predicted content popularity and the geographical distribution of requests. Based on a Video-on-Demand (VoD) request trace of a leading European telecom operator, the presented algorithm is shown to reduce bandwidth usage by 17% compared to the traditional Least Recently Used (LRU) caching strategy, both inside the network and on the ingress links, while at the same time offering enhanced load balancing capabilities. Increasing the prediction accuracy is shown to have the potential to further improve bandwidth efficiency by up to 79%

    Association of single and joint metals with albuminuria and estimated glomerular filtration longitudinal change in middle-aged adults from Spain: The Aragon workers health study

    The nephrotoxicity of low-chronic metal exposures is unclear, especially considering several metals simultaneously. We assessed the individual and joint association of metals with longitudinal change in renal endpoints in Aragon Workers Health Study participants with available measures of essential (cobalt [Co], copper [Cu], molybdenum [Mo] and zinc [Zn]) and non-essential (As, barium [Ba], Cd, chromium [Cr], antimony [Sb], titanium [Ti], uranium [U], vanadium [V] and tungsten [W]) urine metals and albumin-to-creatinine ratio (ACR) (N = 707) and estimated glomerular filtration rate (eGFR) (N = 1493) change. Median levels were 0.24, 7.0, 18.6, 295, 3.1, 1.9, 0.28, 1.16, 9.7, 0.66, 0.22 μg/g for Co, Cu, Mo, Zn, As, Ba, Cd, Cr, Sb, Ti, V and W, respectively, and 52.5 and 27.2 ng/g for Sb and U, respectively. In single metal analysis, higher As, Cr and W concentrations were associated with increasing ACR annual change. Higher Zn, As and Cr concentrations were associated with decreasing eGFR annual change. The shape of the longitudinal dose-responses, however, was compatible with a nephrotoxic role for all metals, both in ACR and eGFR models. In joint metal analysis, both higher mixtures of Cu–Zn–As–Ba–Ti–U–V–W and Co–Cd–Cr–Sb–V–W showed associations with increasing ACR and decreasing eGFR annual change. As and Cr were main drivers of the ACR change joint metal association. For the eGFR change joint metal association, while Zn and Cr were main drivers, other metals also contributed substantially. We identified potential interactions for As, Zn and W by other metals with ACR change, but not with eGFR change. Our findings support that Zn, As, Cr and W and suggestively other metals, are nephrotoxic at relatively low exposure levels. Metal exposure reduction and mitigation interventions may improve prevention and decrease the burden of renal disease in the population

    Surgical treatment for colorectal cancer: Analysis of the influence of an enhanced recovery programme on long-term oncological outcomes-a study protocol for a prospective, multicentre, observational cohort study

    Introduction The evidence currently available from enhanced recovery after surgery (ERAS) programmes concerns their benefits in the immediate postoperative period, but there is still very little evidence as to whether their correct implementation benefits patients in the long term. The working hypothesis here is that, due to the lower response to surgical aggression and lower rates of postoperative complications, ERAS protocols can reduce colorectal cancer-related mortality. The main objective of this study is to analyse the impact of an ERAS programme for colorectal cancer on 5-year survival. As secondary objectives, we propose to analyse the weight of each of the predefined items in the oncological results as well as the quality of life. Methods and analysis A multicentre prospective cohort study was conducted in patients older than 18 years of age who are scheduled to undergo surgery for colorectal cancer. The study involved 12 hospitals with an implemented enhanced recovery protocol according to the guidelines published by the Spanish National Health Service. The intervention group includes patients with a minimum implementation level of 70%, and the control group includes those who fail to reach this level. Compliance will be studied using 18 key performance indicators, and the results will be analysed using cancer survival indicators, including overall survival, cancer-specific survival and relapse-free survival. The time to recurrence, perioperative morbidity and mortality, hospital stay and quality of life will also be studied, the latter using the validated EuroQol Five questionnaire. The propensity index method will be used to create comparable treatment and control groups, and a multivariate regression will be used to study each variable. The Kaplan-Meier estimator will be used to estimate survival and the log-rank test to make comparisons. A p value of less than 0.05 (two-tailed) will be considered to be significant. Ethics and dissemination Ethical approval for this study was obtained from the Aragon Ethical Committee (C.P.-C.I. PI20/086) on 4 March 2020. The findings of this study will be submitted to peer-reviewed journals (BMJ Open, JAMA Surgery, Annals of Surgery, British Journal of Surgery). Abstracts will be submitted to relevant national and international meetings. Trial registration number NCT04305314
