    Peritonitis encapsulante or sclerosing encapsulating peritonitis is diffuse peritoneal fibrosis may develop into a true sclerosis, or a hull sheathing peritoneal intestinal loops commonly found in a cocoon during laparoscopy or laparotomy. His diagnosis but especially pre-operative enables the surgeon to treat the patient optimally. The main etiologies are peritoneal dialysis,intraperitoneal chemotherapy, infections. We report a case of peritonitis encapsulating complicated of occlusion to a patient monitoring for tuberculosis peritoneale and discuss its main clinicaland etiopathogenic.La péritonite encapsulante ou sclérosante est une fibrose péritonéale diffuse pouvant évoluer vers une véritable sclérose voire une coque péritonéale engainant les anses intestinales dans un cocon découvert fréquemment lors des laparoscopies ou laparotomie. Son diagnostic surtout pré- mais aussi opératoire permet au chirurgien de traiter le patient de façon optimale. Les principales étiologies sont la dialyse péritonéale, la chimiothérapie intra péritonéale, les infections. Nous rapportons une observation d’une péritonite encapsulante compliquée d’occlusion chez un patient traité pour tuberculose péritonéale et nous discutons ses principaux aspects cliniques et étiopathogéniques

    LIPOSARCOME DU SEIN. A PROPOS D’UN CAS

    Liposarcoma of the breast is an unfrequent tumor that can be found in 45-55 year old nwomen, usually with benign clinical and radiological characteristics. We report the case of a 51-year-old patient. Symptomatology began since 1 month by an increase from the left breast with general conservation of the state. The cytoponction evoke the diagnosis of liposarcoma. Four years and half of follow up after mastectomy alone, the evolution is well. No relapse was observed. Beyond the not very frequent character of this pathology, our observation underlines the interest of the early diagnosis of this type of tumour, by screening of breast cancers without mammography among menopausal women.Le liposarcome du sein est une lésion rare, pouvant toucher la femme de 45 à 55 ans en moyenne et qui présente les caractéristiques cliniques et radiologiques d'une lésion bénigne. Nous rapportons ici le cas d'une patiente de 51 ans ménopausée. La symptomatologie débutait depuis un  mois par une augmentation du sein gauche sans signes associés avec conservation de l’état général. La ponction biopsie anatomopathologique a permis d’évoquer le diagnostic de liposarcome du sein. 4 ans et demi de recul après mastectomie seule, l'évolution est satisfaisante. Au-delà du caractère peu fréquent de cette pathologie, notre observation souligne l’intérêt du  diagnostic précoce de ce type de tumeur, grâce aux mammographies systématiques annuelles chez les femmes ménopausées

    Automatic tuning of communication protocols for vehicular ad hoc networks using metaheuristics

    The emerging field of vehicular ad hoc networks (VANETs) deals with a set of communicating vehicles which are able to spontaneously interconnect without any pre-existing infrastructure. In such kind of networks, it is crucial to make an optimal configuration of the communication protocols previously to the final network deployment. This way, a human designer can obtain an optimal QoS of the network beforehand. The problem we consider in this work lies in configuring the File Transfer protocol Configuration (FTC) with the aim of optimizing the transmission time, the number of lost packets, and the amount of data transferred in realistic VANET scenarios. We face the FTC with five representative state-of-the-art optimization techniques and compare their performance. These algorithms are: Particle Swarm Optimization (PSO), Differential Evolution (DE), Genetic Algorithm (GA), Evolutionary Strategy (ES), and Simulated Annealing (SA). For our tests, two typical environment instances of VANETs for Urban and Highway scenarios have been defined.     Parental Discontent with Infant Sleep During the First Two Years of Life

    Background: Problematic sleep in infants can have a high impact on families. We examined parental discontent with infant sleep in the first six months of life and parent-perceived problematic sleep during the second year of life. Methods: We used Sarphati Cohort data of 1471 children. During periodic youth health care visits in the first six months of life, professionals registered parental discontent with infant sleep. In the second year of life, parents filled out the Brief Infant Sleep Questionnaire (BISQ), from which we defined parent-perceived problematic sleep and BISQ-defined problematic sleep. We examined the association of parental discontent with infant sleep during the first six months with both BISQ-derived outcomes up to age two, using multivariable logistic regression analysis. Results: 26% of parents were discontented with infant sleep during the first six months of life. During the second year of life, 27% of the parents perceived their child’s sleep as problematic, and 9% of the infants had BISQ-defined problematic sleep. Early parental discontent with infant sleep was associated with parent-perceived problematic sleep [adjusted OR 2.50 (95% CI 1.91–3.28)], and BISQ-defined problematic sleep [adjusted OR 1.88 (1.11–3.17)]. Conclusions: Early registered parental discontent with infant sleep was a predictor of parent-perceived problematic sleep in early toddlerhood. Registering parental discontent during infancy might enable professionals to identify a group of infants at risk for later problematic sleep.
