    What is an ROC curve?

    The paper by Body et al is concerned with the evaluation of decision aids, which can be used to identify potential acute coronary syndromes (ACS) in the ED. The authors previously developed the Manchester Acute Coronary Syndromes model (MACS) decision aid, which uses several clinical variables and two biomarkers to ‘rule in’ and ‘rule out’ ACS. However, one of the two biomarkers (heart-type fatty acid bindingprotein, H-FABP) is not widely used so a revised decision aid has been developed (Troponin-only Manchester Acute Coronary Syndromes, T-MACS), which include a single biomarker hs-cTnT. In this issue, the authors show how they derive a revised decision aid and describe its performance in a number of independent diagnostic cohort studies. Decision aids (as well as other types of ‘diagnostic tests’) are often evaluated in terms of diagnostic testing parameters such as the area under the receiver operating characteristic (ROC) curve, sensitivity and specificity. In this article, we explain how the ROC analysis is conducted and why it is an essential step towards developing a test with the desirable levels of sensitivity and specificity

    An empirical method to cluster objective nebulizer adherence data among adults with cystic fibrosis

    Background: The purpose of using preventative inhaled treatments in cystic fibrosis is to improve health outcomes. Therefore, understanding the relationship between adherence to treatment and health outcome is crucial. Temporal variability, as well as absolute magnitude of adherence affects health outcomes, and there is likely to be a threshold effect in the relationship between adherence and outcomes. We therefore propose a pragmatic algorithm-based clustering method of objective nebulizer adherence data to better understand this relationship, and potentially, to guide clinical decisions. Methods to cluster adherence data: This clustering method consists of three related steps. The first step is to split adherence data for the previous 12 months into four 3-monthly sections. The second step is to calculate mean adherence for each section and to score the section based on mean adherence. The third step is to aggregate the individual scores to determine the final cluster (“cluster 1” = very low adherence; “cluster 2” = low adherence; “cluster 3” = moderate adherence; “cluster 4” = high adherence), and taking into account adherence trend as represented by sequential individual scores. The individual scores should be displayed along with the final cluster for clinicians to fully understand the adherence data. Three illustrative cases: We present three cases to illustrate the use of the proposed clustering method. Conclusion: This pragmatic clustering method can deal with adherence data of variable duration (ie, can be used even if 12 months’ worth of data are unavailable) and can cluster adherence data in real time. Empirical support for some of the clustering parameters is not yet available, but the suggested classifications provide a structure to investigate parameters in future prospective datasets in which there are accurate measurements of nebulizer adherence and health outcomes

    The Impact of Intra-Transaction Communication on Customer Purchase Behaviour in E-Commerce Context

    Online shopping has progressed from having customers passively browse through product pages to having them proactively engage in communication dialogs with product sellers via Live-Chat. Customers indicated that communication with seller is an important type of salesperson service, which begins from their information search and ends with their purchase decision. Building on Content and Style (C&S) framework, this study seeks to understand how the intra-transaction communication influences the customers’ purchase behaviour in environment of e-commerce. By collecting 1234 communication sessions and their following purchase orders from 85 customers, we have a preliminary test of the proposed conceptual hypotheses, and results indicated that informativeness, conversation topic, and emotional expression had significant positive impact on the likelihood of customers’ purchase. This study presents a theoretical-driven and empirically validated proposition to improve the intra-communication to aid customers’ online shopping

    The Effects of Air and Underwater Blast on Composite Sandwich Panels and Tubular Laminate Structures

    The resistance of glass-fibre reinforced polymer (GFRP) sandwich panels and laminate tubes to blast in air and underwater environments has been studied. Procedures for monitoring the structural response of such materials during blast events have been devised. High-speed photography was employed during the air-blast loading of GFRP sandwich panels, in conjunction with digital image correlation (DIC), to monitor the deformation of these structures under shock loading. Failure mechanisms have been revealed by using DIC and confirmed in post-test sectioning. Strain gauges were used to monitor the structural response of similar sandwich materials and GFRP tubular laminates during underwater shocks. The effect of the backing medium (air or water) of the target facing the shock has been identified during these studies. Mechanisms of failure have been established such as core crushing, skin/core cracking, delamination and fibre breakage. Strain gauge data supported the mechanisms for such damage. These studies were part of a research programme sponsored by the Office of Naval Research (ONR) investigating blast loading of composite naval structures. The full-scale experimental results presented here will aid and assist in the development of analytical and computational models. Furthermore, it highlights the importance of support and boundary conditions with regards to blast resistant design

    Effects of Physical Conditioning on Intercollegiate Golfer Performance

    This investigation was conducted to determine the effects of a physical conditioning program on clubhead speed, consistency, and putting distance control in 10 men and 6 women National Collegiate Athletic Association Division I golfers. Supervised strength, power, and flexibility training was performed 3 times per week for 11 weeks. Performance tests were conducted before and after the training period. Significant (p \u3c 0.05) increases were noted for all strength, power, and flexibility tests from pre- to posttraining of between 7.3 and 19.9%. Clubhead speed in- creased significantly (1.6%), equating to approximately a 4.9-m increase in driving distance. Putting distance control significantly improved for the men-only group (29.6%), whereas there was no significant difference in putting distance control for the total and women-only groups. Eleven weeks of golf-specific physical conditioning increased clubhead speed without a negative effect on consistency or putting distance control in intercollegiate men and women golfers

    Prevalence of dementia in ischaemic or mixed stroke populations: systematic review and meta-analysis

    An understanding of the epidemiology of poststroke dementia (PSD) is necessary to inform research, practice and policy. With increasing primary studies, a contemporary review of PSD could allow for analyses of incidence and prevalence trends. Databases were searched using a prespecified search strategy. Eligible studies described an ischaemic or mixed stroke cohort with prospective clinical assessment for dementia. Pooled prevalence of dementia was calculated using random-effects models at any time after stroke (primary outcome) and at 1 year (range: 6–18 months), stratified for inclusion of prestroke dementia. Meta-regression explored the effect of year of study. Sensitivity analyses removed low-quality or outlier studies. Of 12 505 titles assessed, 44 studies were included in the quantitative analyses. At any time point after stroke, the prevalence of PSD was 16.5% (95% CI 10.4% to 25.1%) excluding prestroke dementia and 22.3% (95% CI 18.8% to 26.2%) including prestroke dementia. At 1 year, the prevalence of PSD was 18.4% (95% CI 7.4% to 38.7%) and 20.4% (95% CI 14.2% to 28.2%) with prestroke dementia included. In studies including prestroke dementia there was a negative association between dementia prevalence and year of study (slope coefficient=−0.05 (SD: 0.01), p<0.0001). Estimates were robust to sensitivity analyses. Dementia is common following stroke. At any point following stroke, more than one in five people will have dementia, although a proportion of this dementia predates the stroke. Declining prevalence of prestroke dementia may explain apparent reduction in PSD over time. Risk of dementia following stroke remains substantial and front-loaded, with high prevalence at 1 year post event

    Promoting adherence to nebulized therapy in cystic fibrosis: poster development and a qualitative exploration of adherence

    Background: Cystic fibrosis (CF) health care professionals recognize the need to motivate people with CF to adhere to nebulizer treatments, yet little is known about how best to achieve this. We aimed to produce motivational posters to support nebulizer adherence by using social marketing involving people with CF in the development of those posters. Methods: The Sheffield CF multidisciplinary team produced preliminary ideas that were elaborated upon with semi-structured interviews among people with CF to explore barriers and facilitators to the use of nebulized therapy. Initial themes and poster designs were refined using an online focus group to finalize the poster designs. Results: People with CF preferred aspirational posters describing what could be achieved through adherence in contrast to posters that highlighted the adverse consequences of nonadherence. A total of 14 posters were produced through this process. Conclusion: People with CF can be engaged to develop promotional material to support adherence, providing a unique perspective differing from that of the CF multidisciplinary team. Further research is needed to evaluate the effectiveness of these posters to support nebulizer adherence

    Impact of ethnicity and extreme prematurity on infant pulmonary function.

    The impact of birth before 27 completed weeks of gestation on infant pulmonary function (PF) was explored in a multi-ethnic population in comparison to more mature preterm controls (PTC) and healthy fullterm infants. Plethysmographic lung volume (FRCpleth ) and forced expired volume (FEV0.5 ) were obtained at ∌12 months post-term age in 52 extremely preterm (EP) infants (median [range] gestational age [GA]: 26 [23-27] weeks; 40% White mothers; 79% with BPD), 41 PTC (GA:35 [30-36] weeks; 37% White mothers) and 95 fullterm infants (GA:40 [37-42] weeks; 86% White mothers). Using reference equations based on identical equipment and techniques, results were expressed as z-scores to adjust for age, sex and body size. FEV0.5 was significantly lower in EP infants when compared with PTC (mean difference [95% CI]: -1.02[-1.60; -0.44] z-scores, P < 0.001), as was forced vital capacity (FVC) but there were no significant differences in FRCpleth or FEV0.5 /FVC ratio. FEV0.5 , FVC, and FEV0.5 /FVC were significantly lower in both preterm groups when compared with fullterm controls. On multivariable analyses of the combined preterm dataset: FEV0.5 at ∌1 year was 0.11 [0.05; 0.17] z-scores higher/week GA, and 1.28 (0.49; 2.08) z-scores lower in EP infants with prior BPD. Among non-white preterm infants, FEV0.5 was 0.70 (0.17; 1.24) z-scores lower, with similar reductions in FVC, such that there were no ethnic differences in FEV0.5 /FVC. Similar ethnic differences were observed among fullterm infants. These results confirm the negative impact of preterm birth on subsequent lung development, especially following a diagnosis of BPD, and emphasize the importance of taking ethnic background into account when interpreting results during infancy as in older subjects.     EFFECTS OF TRYING TO GAIN DISTANCE ON GOLF DRIVING TECHNIQUE

    Previous biomechanical research of the golf swing has shown segment separation of the shoulders and hips aids ball velocity. The purpose of the current study was to examine how the swing may differ when “trying” to gain extra distance in a game specific situation and whether an increase in launch velocity was achievable. A single subject design was employed with kinematic data (200 Hz) and ball characteristics collected using Trackman Pro launch monitor (ISG A/S, Denmark). Segment separation of the shoulder and hips (X-Factor and X-Factor stretch) and maximum endpoint velocity of the left hip, shoulder and elbow showed a significant difference between conditions, although mean launch velocity did not. This finding shows although the subject was able to produce greater endpoint velocity, performance improvement did not occur
