Effectiveness of cognitive orientation to (daily) occupational performance (CO-OP) on children with cerebral palsy: A mixed design
Background. Cerebral palsy (CP) is the most common cause of physical disabilities during childhood. Therapeutic interventions mainly focus on impairment reduction to address motor-based difficulties. In contrast, Cognitive Orientation to daily Occupational Performance (CO-OP) is a cognitive approach, providing intervention at the level of activity and participation.
Aims. This study aims to determine whether the CO-OP approach improves motor skills and achievement in motor-based occupational performance goals in children with CP.
Methods and procedures. In this mixed design research (i.e., a multiple baseline single case experimental design and a one-group pretest-posttest design), five children with CP participated in 12 CO-OP intervention sessions. Repeated measures of motor skills for the multiple baseline single case experimental design were taken using the Bruininks-Oseretsky Test of Motor Proficiency (BOTMP); pre- and post-measures of parent/child perception of performance and satisfaction were identified using the Canadian Occupational Performance Measure (COPM); level of achievement was identified using Goal Attainment Scaling (GAS).
Outcomes and results. According to the BOTMP results, all children were able to engage in the CO-OP intervention to improve motor performance. Significant differences after treatment were found in both performance and performance satisfaction ratings using the COPM as rated by parents and children. The GAS results showed progress in achievement levels for all children; all goals were achieved or exceeded.
Conclusions and implications. CO-OP intervention can be helpful in improving motor skills and achieving self-identified, motor-based goals in children with CP
Cognitive orientation to occupational performance : effect on brain structure/function and motor outcomes in children with developmental coordination disorder
Children with developmental coordination disorder (DCD) have difficulty learning motor skills, which can affect their participation in activities of daily living and psychosocial well-being. Over 50% of children with DCD also have attention deficit hyperactivity disorder (ADHD), which further exacerbates their motor function and quality of life. Cognitive Orientation to Occupational Performance (CO-OP), a rehabilitation approach developed for children with DCD, uses problem-solving strategies to help children learn motor skills they wish to achieve. While this approach has been effective for children with DCD, few studies have examined its effectiveness for children with co-occurring ADHD. Further, the underlying mechanisms of this intervention are unknown.
In this randomized waitlist-controlled trial (ClinicalTrials.gov ID: NCT02597751), I used MRI and motor outcome measures to determine whether CO-OP intervention: (1) was effective in improving motor goals in children with DCD +/- ADHD; (2) induced changes in functional connectivity of the brain; and (3) promoted positive neuroplastic changes in white matter microstructure.
Thirty-seven children with DCD and 41 children with DCD+ADHD, aged 8-12 years old, were randomized to treatment or waitlist groups at their first MRI. The treatment group began the intervention after their MRI scan and pre-assessment, and returned for a post-treatment assessment/scan at 3 months, and a follow-up scan at 6 months; the waitlist group waited 3 months before their second MRI, received intervention, and then had a post-treatment assessment and MRI scan.
Results showed that CO-OP is effective for children with DCD +/- ADHD, in achieving functional motor goals. Neuroimaging results showed improved functional connectivity within the default mode network (DMN) as well as improved microstructural properties in the white matter underlying the DMN in children with DCD only. However, there was a lack of transfer to other motor skills and brain changes in children with DCD+ADHD.
Given the role of the DMN in self-regulation, emotion regulation, and attention regulation and in accordance with cognitive models of motor learning, I suggest that these cognitive processes may underlie motor skills improvement after CO-OP in children with DCD. مقایسه کارکردهای اجرایی کودکان اتیستیک با عملکرد بالا و همتایان عادی
مقدمه: فرضیههای متعددی در خصوص نقصان کارکردهای شناختی در کودکان مبتلا به اختلال طیف اتیسم وجود دارد که یکی از آنها فرضیه نقصان کارکردهای اجرایی است. هدف از این مطالعه، بررسی تفاوت دو گروه کودکان مبتلا به اتیسم و همتایان سالم در توجه پایدار، توجه انتخابی و مهار پاسخ به عنوان کارکردهای اجرایی بود.
مواد و روشها: در این مطالعه مقطعي، 16 کودک مبتلا به اتیسم با عملکرد بالا با 16 کودک همتای عادی مورد مقايسه قرار گرفتند. آنها به انجام آزمون استروپ رنگ- واژه (جهت ارزیابی توجه انتخابی) و آزمون برو نرو (جهت ارزیابی مهار) و آزمون عملکرد مداوم (جهت ارزیابی توجه پایدار) پرداختند. جهت آنالیز دادهها Independent t-test مورد استفاده قرار گرفت. از نرمافزار 18SPSS برای آناليز دادهها استفاده شد و سطح معنیداری 01/0 در نظر گرفته شد.
یافتهها: در آزمون استروپ در تعداد پاسخهای صحیح (05/0 P) کمتر از همتایان عادی بود. در آزمون برو نرو، میانگین زمان (05/0 P) در گروه عادی به طور معنیداری بیشتر از این میزان در گروه اتیسم بود.
نتیجه گیری: مهار پاسخ (غالب و حرکتی) و توجه پایدار از جمله کارکردهای اجرایی شناختی سالم در افراد اتیسم با عملکرد بالا است. سلامت مهار پاسخ در اتیسم میتواند به علت نقص این افراد در درک مطلب، مناطق مغزی درگیر و یا نوع تکالیف ارایه شده باشد. سلامت توجه پایدار در افراد اتیسم نیز حاکی از تمایل به تمرکز روی محرکهای خاص و رفتارهای تکراری است که از علایم شناخته شده اتیسم میباشد.

شواهد عصب شناختی نقصان برتری طرفی مغز در کودکان مبتلا به اختلال طیف اتیسم
مقدمه: منحصر به فردترین جنبه در سازمانبندی مغز انسان عدم تقارن مغزی است و اوتیسم یکی از اختلالاتی است که شاید بر این عدم تقارن و عملکرد نیمکرههای مغزی تأثیر میگذارد. هدف از این مطالعه، بررسی تفاوت عدم تقارن نیمکرهای در افراد اوتیسم نسبت به همتایان عادی و تعیین ارتباطات بین طرفی شدن دست و زبان بود.
مواد و روشها: در مطالعه حاضر 26 پسر مبتلا به اوتیسم با عملکرد بالا، با استفاده از ASSQ و 27 پسر همتای عادی (سن و هوشبهر) از دبستانهای پسرانه شهرستان نجف آباد انتخاب شدند. تمام نمونهها با استفاده از پرسشنامه برتری طرفی Edinburgh، آزمون ضربه زدن و آزمون شنود دو گوشی مورد ارزیابی قرار گرفتند. پس از جمعآوری دادهها، تحلیل با استفاده از نرمافزار SPSS نسخه 19 انجام شد.
یافتهها : 2/19 درصد نمونههای اوتیستیک برتری دست چپ داشتند و تقاوت معنیداری بین نمرات Edinburgh دستی دو گروه وجود دارد (006/0 = P). بین دو دست افراد اوتیستیک در آزمون ضربه زدن تفاوتی مشاهده نشد (186/0 = P) و تفاوت دو دست در دو گروه معنیدار بود (021/0 = P). در آزمون شنود دو گوشی مزیت گوش راست در افراد اوتیستیک مشاهده شد (001/0 > P)، ولی تفاوت معنیداری بین دو گوش در دو گروه نیز مشخص شد (007/0 = P). نتایج هیچ گونه همبستگی بین عوامل برتری دستی و طرفی شدن زبان آشکار نکرد.
نتیجهگیری : طرفی شدن طبیعی در هیچ یک از آزمونهای ضربه زدن و شنود دو گوشی و پرسشنامه برتری دستی Edinburgh در افراد اوتیستیک مشاهده نشد و عدم تقارن نیمکرهای طبیعی در افراد اوتیستیک از الگوی هنجاری پیروی نمیکند. شیوع بالای چپ دستی در افراد اوتیستیک نیز عاملی بر تأیید ناهنجاری در عملکرد بین نیمکرهای میباشد. به علاوه با وجود ارتباط نزدیک برتری دستی و طرفی شدن زبان، هیچ همبستگی مشخصی بین این عوامل مشاهده نشد.

Cognitive and General Health Status in Elderly With and Without History of Falling
Objectives: Falling is a main problem in all health systems and approximately 30% of elderly experience one or more falling every year. Cognitive function is known to be a main risk factor for elderly falling. In this study we aim to compare general health and general cognitive status in elderly with and without history of falling.
Methods & Materials: In this study 22 elderly without history of falling (15 M, 7 FM) were evaluated and compared with 31 elderly with history of falling (21 M, 10 FM) using general health questionnaire (GHQ) and MMSE. Independent t-test is used for data analysis.
Results: Findings showed that differences of general health (P=0.009), and subtest of anxiety and sleep disorder (P=0.004) between two groups are significant. Means comparison showed worst status in elderly with history of falling than those without in general health, sleep disorder, and anxiety. Significant difference between cognitive health score of faller and non-faller elders was seen (P=0.022). Comparing means showed more cognitive impairment in elderly with history of falling than those without history of falling.
Conclusion: This study confirms relationship between anxiety, sleep disorder and falling. Sleep disorder may cause falling due to dizziness. Anxiety and fear of falling may cause limited physical activity, balance, and increased risk of falling. Therapeutic methods in reducing anxiety would avoid falling. In addition, cognitive therapy for elderly can reduce the risk of falling
Comparing the effects of an aerobic exercise program and group-based play therapy on the balance of children with Attention Deficit Hyperactive Disorder (ADHD)
Introduction: Attention Deficit Hyperactivity Disorder (ADHD) is a developmental disorder usually characterized by poor coordination and clumsiness. It seems that rhythmic aerobic exercise has the same effect on affected children’s balance as play therapy. The aim of this study was to compare the effect of an aerobic exercise program with that of group play therapy on the balance abilities of children with ADHD.
Materials and Methods: In this study, 26 children with attention deficit hyperactivity disorder (4 girls and 22 boys) were assigned to 2 experimental groups (group-based play therapy and aerobic exercise groups) and one control group. Each experimental group received its respective intervention program 3 sessions a week, lasting 45 to 60 minutes each, over an 8-week period.
Results: Significant differences were observed between group-based play therapy group and aerobic group regarding dynamic balance. There was, however, no significant difference between experimental groups as long as static balance was concerned.
Conclusion: Although group-based play therapy had greater impact on improving the balance of children with ADHD, aerobic exercise program had significant effects on improving static balance. Therefore adding aerobic exercise to routine group play therapy may bring more benefits to children with ADHD.

مقایسه یادگیری حرکتی صریح و یادگیری حرکتی ضمنی در کودکان مبتلا به اوتیسم با عملکرد بالا و آسپرگر با همتایان عادی
مقدمه: اختلالات طیف اوتیسم با ناهنجاریهای مناطق قشری همراه میباشند که پایهای برای نقص یادگیری حرکتی ایجاد کردهاند. یادگیری حرکتی به دو طبقه اصلی صریح و ضمنی تقسیم میشود. هدف این پژوهش، تعیین تفاوت یادگیری حرکتی صریح با ضمنی و میزان تحکیم یادگیری بعد از 24 ساعت در پسران مبتلا به اختلالات طیف اوتیسم و همتایان عادی بود.
مواد و روشها: در این مطالعه، 30 پسر مبتلا به اختلالات طیف اوتیسم با 32 پسر همتای عادی مقایسه شدند. نمونههای سالم و مبتلا به اختلالات طیف اوتیسم با استفاده از تست ASSQ از دبستانهای پسرانه شهرستان نجف آباد انتخاب گردیدند. هر دو گروه در روز اول به انجام آزمون زمان عکسالعمل متوالی (10 بلوک) با دست راست پرداختند و روز بعد، آزمون را در 4 بلوک به منظور سنجش تحکیم انجام دادند. آزمون ANOVA با اندازهگیری مکرر و با استفاده از 19SPSS جهت تحلیل دادهها مورد استفاده قرار گرفت.
یافتهها: دو گروه در موضوع یادگیری صریح تفاوت معنیداری با یکدیگر داشتند که بیانگر نقص یادگیری صریح در افراد مبتلا به اختلالات طیف اوتیسم بود (009/0 = P). نتایج همچنین عدم تفاوت معنیدار را در یادگیری ضمنی بین دو گروه نشان داد (385/0 = P)، به علاوه یافتهها حاکی از این است که تحکیم یادگیری ضمنی در گروه اختلالات طیف اوتیسم (160/0 = P) نیز بینقص و تحکیم یادگیری صریح دچار اختلال (046/0 = P) میباشد.
نتیجهگیری: نقص یادگیری حرکتی صریح و تحکیم آن در گروه اختلالات طیف اوتیسم میتواند به علت اختلال عملکرد نیمکره چپ، تخصصی شده در یادگیری صریح، باشد. به علاوه تحکیم یادگیری صریح وابسته به خواب است و در گروه اختلالات طیف اوتیسم اختلالات خواب وجود دارد، بنابراین این نقص دور از انتظار نیست. سالم بودن یادگیری ضمنی و تحکیم آن را نیز میتوان به تأثیر نیمکره راست در یادگیری حرکتی ضمنی نسبت داد.

Implicit and explicit motor learning: Application to children with Autism Spectrum Disorder (ASD)
Aims and objectives: This study aims to determine whether children with Autism Spectrum Disorder (ASD) are capable of learning a motor skill both implicitly and explicitly. Methods: In the present study, 30 boys with ASD, aged 7-11 with IQ average of 81.2, were compared with 32 typical IQ- and age-matched boys on their performance on a serial reaction time task (SRTT). Children were grouped by ASD and typical children and by implicit and explicit learning groups for the SRTT. Results: Implicit motor learning occurred in both children with ASD (p = .02) and typical children (p = .01). There were no significant differences between groups (p = .39). However, explicit motor learning was only observed in typical children (p = .01) not children with ASD (p = .40). There was a significant difference between groups for explicit learning (p = .01). Discussion: The results of our study showed that implicit motor learning is not affected in children with ASD. Implications for implicit and explicit learning are applied to the CO-OP approach of motor learning with children with ASD
Cancer Patients’ Experiences with Telehealth before and during the COVID-19 Pandemic in British Columbia
Background: Patients have had their cancer care either postponed or changed to telehealth visits to reduce exposure to COVID-19. However, it is unclear how these changes may have affected their experiences. We aim to identify patient characteristics that affect telehealth experiences and evaluate their preferences for using telehealth in the future. Methods: Patients who completed the Outpatient Cancer Care (OCC) Patient Experience Survey were invited to participate. They comepleted the modified OCC Survey, which focused on telehealth during the pandemic. Linear and logistic regression analyses were used to identify patient characteristics that influenced telehealth experiences and preferences for future telehealth use. Results: Perceived ease of participation in telehealth is a significant predictor of the change in patients’ ratings of their telehealth experience. We found that cancer patients had lower preferences for using telehealth in the future if they were older, female, or non-white; resided in an urban area; had no previous telehealth experience; had lower education; and had poorer mental health.
