    Paleoalvei della Laguna: Quattro scenari per Venezia

    Il contributo che proponiamo ha come oggetto la Laguna di Venezia che, ancora una volta nella sua lunga storia, necessita di un profondo ripensamento alla luce della crisi ambientale, del declino demografico e della pressione turistica che la attraversano. Nel 2018 il consorzio CoRiLa ha avviato un nuovo programma di ricerca che intende investigare l’evoluzione del territorio veneziano in prospettiva dell’entrata in funzione del Mose, «quando il sistema lagunare di Venezia diventerà regolato». L’ampio programma di ricerca entro cui si sviluppano le riflessioni di questo testo contempla la realizzazione di scenari per il futuro di Venezia e della sua laguna e coinvolge un vasto numero di ricercatori, architetti, urbanisti, ingegneri idraulici, scienziati ambientali, biologi, delle Università Iuav, Ca’ Foscari, Padova e i degli enti nazionali di ricerca CNR e OGS. Di fronte all’incertezza e all’incapacità collettiva di immaginare un futuro condiviso per Venezia, il paper che avanziamo prova a ripercorrere la storia di questo territorio d’acqua attraverso episodi urbanistici pensati e documentati. Dalla nascita della Serenissima, la Laguna Veneta è stata oggetto di numerosi interventi per consentire la sopravvivenza dell’uomo ma la sua storia progettuale è fatta anche da piani – importanti e poco conosciuti – che oggi si materializzano in pensieri documentati. Esiste infatti un archivio di disegni, progetti, testi, descrizioni con cui è possibile capire che i processi sociali di abbandono e degrado e i rischi ambientali a cui la laguna oggi è sottoposta non solo erano già noti in passato, ma a questi sono state anche avanzate delle soluzioni che mettono al centro il territorio e la sua trasformazione. A partire da testi elaborati da diversi autori negli ultimi sei secoli e confrontandosi con le sfide ambientali, sociali ed economiche che ci attendono, il testo che segue ragiona sulla potenziale attualizzazione di una serie di lagune mai realizzate

    Sex-Related Disparities in Cardiac Masses: Clinical Features and Outcomes

    Background. Cardiac masses (CM) represent a heterogeneous clinical scenario, and sex-related differences of these patients remain to be established. Purpose: To evaluate sex-related disparities in CMs regarding clinical presentation and outcomes. Material and Methods. The study cohort included 321 consecutive patients with CM enrolled in our Centre between 2004 and 2022. A definitive diagnosis was achieved by histological examination or, in the case of cardiac thrombi, with radiological evidence of thrombus resolution after anticoagulant treatment. All-cause mortality at follow-up was evaluated. Multivariable regression analysis assessed the potential prognostic disparities between men and women. Results. Out of 321 patients with CM, 172 (54%) were female. Women were more frequently younger (p = 0.02) than men. Regarding CM histotypes, females were affected by benign masses more frequently (with cardiac myxoma above all), while metastatic tumours were more common in men (p < 0.001). At presentation, peripheral embolism occurred predominantly in women (p = 0.03). Echocardiographic features such as greater dimension, irregular margin, infiltration, sessile mass and immobility were far more common in men. Despite a better overall survival in women, no sex-related differences were observed in the prognosis of benign or malignant masses. In fact, in multivariate analyses, sex was not independently associated with all-cause death. Conversely, age, smoking habit, malignant tumours and peripheral embolism were independent predictors of mortality. Conclusions. In a large cohort of cardiac masses, a significant sex-related difference in histotype prevalence was found: Benign CMs affected female patients more frequently, while malignant tumours affected predominantly men. Despite better overall survival in women, sex did not influence prognosis in benign and malignant masses

    Performance of Prognostic Scoring Systems in MINOCA: A Comparison among GRACE, TIMI, HEART, and ACEF Scores

    Background: the prognosis of patients with myocardial infarction with non-obstructive coronary arteries (MINOCA) is not benign; thus, prompting the need to validate prognostic scoring systems for this population. Aim: to evaluate and compare the prognostic performance of GRACE, TIMI, HEART, and ACEF scores in MINOCA patients. Methods: A total of 250 MINOCA patients from January 2017 to September 2021 were included. For each patient, the four scores at admission were retrospectively calculated. The primary outcome was a composite of all-cause death and acute myocardial infarction (AMI) at 1-year follow-up. The ability to predict 1-year all-cause death was also tested. Results: Overall, the tested scores presented a sub-optimal performance in predicting the composite major adverse event in MINOCA patients, showing an AUC ranging between 0.7 and 0.8. Among them, the GRACE score appeared to be the best in predicting all-cause death, reaching high specificity with low sensitivity. The best cut-off identified for the GRACE score was 171, higher compared to the cut-off of 140 generally applied to identify high-risk patients with obstructive AMI. When the scores were tested for prediction of 1-year all-cause death, the GRACE and the ACEF score showed very good accuracy (AUC = 0.932 and 0.828, respectively). Conclusion: the prognostic scoring tools, validated in AMI cohorts, could be useful even in MINOCA patients, although their performance appeared sub-optimal, prompting the need for risk assessment tools specific to MINOCA patients

    Questo saggio, a partre dall’enorme archivio di proget non realizzat per la laguna di Venezia, contestualizza tre piani (mai realizzat o compiut solo in parte) con l’obietvo generale di defnire nuovi strument e argomentazioni per problematzzare più a fondo gli intervent infrastruturali atualmente in corso. Quest proget, infat, sono casi paradigmatci con cui raccontare come specifci modelli di razionalizzazione del territorio si siano susseguit e come quest abbiano interpretato la laguna nell’ultmo secolo. Nello specifco, verrà dimostrato come a proget puntuali corrispondano specifche visioni della laguna all’interno di un quadro geografco di relazioni più estese.Starting from the extensive archive of unrealized projects for the Venice lagoon, the essay contextualizes three plans (never or partially completed) in the aim of defining new tools and argumentations to further problematize the ongoing infrastructural interventions. These projects, indeed, are paradigmatic cases in order to report how specific models of rationalization of the territory came in succession and how they interpreted the lagoon during last century.     Several major infrastructure projects and energy scenarios, developed in the Mediterranean basin between the 20th and 21st centuries, are described in this essay in the belief that it is difficult to discuss about circulation and adaptation of urban models regardless of the technological models which slowly and gradually materialized establishing, on the Earth surface, a comprehensive energy supply infrastructure. In the first part, it is argued how, today, the Mediterranean, starting from the energy issue, has acquired a new pivotal role in the world geopolitical framework. The second part takes a step back, it examines the capacity of Atlantropa \u2013 a 20th century colossal energy project \u2013 to build a single and integral imaginary around the Mediterranean. After World War II, the global and unitary perspective of the Mediterranean physically developed thanks to the infrastructural investments sponsored by disparate energy companies, such as Eni or Total. During the last century, these European agencies built, at the Mediterranean scale, a massive fossil-fueled energy production machine which, today, appears to be at the end of its cycle. Describing and reviewing two recent renewables energy scenarios (Desertec and Roadmap 2050), latest paragraphs of the essay question our future reflecting on the need of envisioning it through the project and its tools, and on the sense of a new and possible cooperation between regions in the Mediterranean
