    Hetero-Masculinity, Homosexual Desire: A Perfect Gay World

    In order to create a world where homosexual desire, masculinities, and sex is reconciled, gay pom challenges and presents new ways of framing our understanding of sexuality, gender and sex. It does so by offering options that are more fluid and interchangeable, and by rearranging normative lines of logic in ways that better fit the gay male experience. Several popular categories of gay pom blur the lines between normative understandings of sexual identity, masculinity and sexual behavior, allowing space for fluidity and a more fulfilling experience of desire. To demonstrate this, I explored three popular experiences in online gay pom; Gay-for-pay websites; websites that depict married men and fraternity guys engaging in same-sex sexual behavior; and websites that depict same-sex behaviors between men in the military, teacher/students in schools, locker rooms, and the work place. What I found is that the experiences in these three categories attempt to construct a world of fluid sexuality and sex by homoeroticizing the dominant frameworks of masculinity and heterosexuality.B.S.     This thesis presents a study of two different applications of Networked Control Systems. The first is: Ethernet Networked Control System On-board of Train-wagons. An Ethernet backbone is shared between control and entertainment. The wagon contains a dedicated control server and a dedicated entertainment server, which act as fault-tolerant machines for one another. In the event of a server failure, the remaining machine can serve both entertainment and/or control. The study aims at enhancing system design in order to maximize the tolerable entertainment load in the event of a control/entertainment server failure, while not causing any control violations. This fault-tolerant system is mathematically analyzed using a performability model to relate failure rates, enhancements and rewards. The model is taken further to test two identical wagons, with a total of four fault-tolerant servers. All possible failure sequences are simulated and a different communication philosophy is tested to further minimize the degradation of the entertainment load supported during the failure of up to three of the four servers. The system is shown to be capable of operating with minimal degradation with one out of four servers. The second is: Wireless Networked Control Systems (WNCS) for Industrial Automation. A WNCS using standard 802.11 and 802.3 protocols for communication is presented. Wireless Interface for Sensors and Actuators (WISA) by ABB is used as a benchmark for comparison. The basic unit is a single workcell, however, there is a need to cascade several cells along a production line. Simulations are conducted and a nontraditional allocation scheme is used to ensure correct operation under the effect of co-channel interference and network congestion. Next, fault-tolerance at the controller level is investigated due to the importance of minimizing downtime resulting from controller failure. Two different techniques of interconnecting neighboring cells are investigated. The study models both a two and three-cell scenario, and all systems show that fault-tolerance is achievable. This is mathematically studied using a performability analysis to relate failure rates with rewards at each failure state.     The Impact of Risk Factors Reduction Scenarios on Hospital Admissions, Disability-Adjusted Life Years and the Hospitalisation Cost of Cardiovascular Disease in Thailand

    Cardiovascular disease (CVD) is considered to be one of the leading health issues in Thailand. CVD not only contributes to an increase in the number of hospital admissions year by year but also impacts on the rising health care expenditure for the treatment and long-term care of CVD patients. Therefore, this study is aimed at examining the impacts of risk reduction strategies on the number of CVD hospital admissions, Disability-Adjusted Life Years (DALYs) and the costs of hospitalisation. To estimate such impacts a CVD cost-offset model was applied using a Microsoft Excel spreadsheet. The number of the mid-year population was classified by age, gender and the CVD risk factor profiles from the recent Thai National Health Examination Survey (NHES) IV. This survey was chosen as the baseline population. The CVD risk factor profiles included age, gender, systolic blood pressure, total cholesterol, and smoking status. The Asia-Pacific Collaborative Cohort Study (APCCS) equation was applied to predict the probability of developing CVD over the next eight-year period. Estimates on the following were obtained from the model: 1) the CVD events both fatal and non-fatal; 2) the difference between the projected number of deaths and the actual number of deaths in that population; 3) the number of patients who are expected to live with CVD; 4) the DALYs from the estimated number of fatal and non-fatal events; 5) the cost of hospital admissions. Four CVD risk strategy scenarios were investigated as follows: 1) the do-nothing scenario; 2) the optimistic scenario; 3) achieve the UN millennium development goal; and 4) the worst-case scenario. The findings showed that over the next eight years, there are likely to be 3,297,428 recorded cases of CVD; 5,870,049 cases of DALYs; and, approximately ฿57,000 million, (1.9billion),isprojectedasthetotalcostofhospitaladmissions.However,ifthecurrenthealthpolicycanreducethelevelsofriskfactorsasdefinedintheoptimisticscenarioorsuchpolicymeetsthespecificationsoftheUNmillenniumdevelopmentgoal,therewouldbeasignificantreductioninthenumberofhospitaladmissions.Theseareestimatedtobeareductionof522,179maleand515,416femalecases.Withtheseresults,itisexpectedthathealthcarecostswouldsaveapproximately฿9000million,(1.9 billion), is projected as the total cost of hospital admissions. However, if the current health policy can reduce the levels of risk factors as defined in the optimistic scenario or such policy meets the specifications of the UN millennium development goal, there would be a significant reduction in the number of hospital admissions. These are estimated to be a reduction of 522,179 male and 515,416 female cases. With these results, it is expected that health care costs would save approximately ฿9000 million, (298.3 million), for CVD and 900,000 million DALYs over the next eight years. However, if there is an upward trend in the risk factors as predicted in the worst-case scenario, then there will be an increase of 428,220 CVD cases; consequently, DALYs cases may rise by 766,029 while the hospitalisation costs may increase by approximately ฿7000 million, ($232.1 million). Based on our findings, reducing the levels of CVD risk factors in the population will drastically reduce: 1) the number of CVD cases; 2) DALYs cases; and 3) health care costs. Therefore it is recommended that the health policy should enhance the primary prevention programs which would be targeted at reducing the CVD risk factors in the population

    Background: Cardiovascular disease (CVD) has become a leading cause of death and disability in Thailand due to the unhealthy lifestyle of the populace; triggering high risk of exposure to CVD, increase in the number of hospital admissions year on year. Objectives: The concerns generated by the inflation in the health care expenditure among service providers motivated this study to examine the costs of hospitalisation of inpatients with (CVD) conditions in Thailand, 2009. Methods: Anonymised secondary data of 327,435 CVD inpatients under “Universal Coverage” (UC) health care scheme were obtained from the National Health Security Office (NHSO), Thailand. The data(51.69%- women and 48.31% - men) were classified using International Classification of Diseases, Tenth Revision (ICD-10) code, of which I20-I25 are Ischemic heart disease (IHD), I60-I69 are stroke and I00- I99areallCVD conditions. Results: Average costs of treatments for all CVD conditions, IHD and stroke were ฿21,921 (£1 = ฿50), ฿32,884 (highest) and ฿25,617.67per patient respectively. Absolute total cost increased with age and the cost of admission of male patients is higher than female. The average (three months) length of stay for stroke patients was found to be the highest. Conclusion: Providers generally spent a total of ฿7,177 million on the treatment of CVD with IHD and stroke taking ฿2,544 million and ฿1,920 million respectivel

    Modeling the Telemarketing Process using Genetic Algorithms and Extreme Boosting: Feature Selection and Cost-Sensitive Analytical Approach

    Currently, almost all direct marketing activities take place virtually rather than in person, weakening interpersonal skills at an alarming pace. Furthermore, businesses have been striving to sense and foster the tendency of their clients to accept a marketing offer. The digital transformation and the increased virtual presence forced firms to seek novel marketing research approaches. This research aims at leveraging the power of telemarketing data in modeling the willingness of clients to make a term deposit and finding the most significant characteristics of the clients. Real-world data from a Portuguese bank and national socio-economic metrics are used to model the telemarketing decision-making process. This research makes two key contributions. First, propose a novel genetic algorithm-based classifier to select the best discriminating features and tune classifier parameters simultaneously. Second, build an explainable prediction model. The best-generated classification models were intensively validated using 50 times repeated 10-fold stratified cross-validation and the selected features have been analyzed. The models significantly outperform the related works in terms of class of interest accuracy, they attained an average of 89.07\% and 0.059 in terms of geometric mean and type I error respectively. The model is expected to maximize the potential profit margin at the least possible cost and provide more insights to support marketing decision-making

    Modified Genetic Algorithm for Feature Selection and Hyper Parameter Optimization: Case of XGBoost in Spam Prediction

    Recently, spam on online social networks has attracted attention in the research and business world. Twitter has become the preferred medium to spread spam content. Many research efforts attempted to encounter social networks spam. Twitter brought extra challenges represented by the feature space size, and imbalanced data distributions. Usually, the related research works focus on part of these main challenges or produce black-box models. In this paper, we propose a modified genetic algorithm for simultaneous dimensionality reduction and hyper parameter optimization over imbalanced datasets. The algorithm initialized an eXtreme Gradient Boosting classifier and reduced the features space of tweets dataset; to generate a spam prediction model. The model is validated using a 50 times repeated 10-fold stratified cross-validation, and analyzed using nonparametric statistical tests. The resulted prediction model attains on average 82.32\% and 92.67\% in terms of geometric mean and accuracy respectively, utilizing less than 10\% of the total feature space. The empirical results show that the modified genetic algorithm outperforms Chi2Chi^2 and PCAPCA feature selection methods. In addition, eXtreme Gradient Boosting outperforms many machine learning algorithms, including BERT-based deep learning model, in spam prediction. Furthermore, the proposed approach is applied to SMS spam modeling and compared to related works

    Absorbable suture materials for primary repair of episiotomy and second degree tears

    Background Approximately 70% of women will experience perineal trauma following vaginal delivery and will require stitches. This may result in pain, suture removal and superficial dyspareunia. Objectives To assess the effects of different suture materials on short- and long-term morbidity following perineal repair. Search methods We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (February 2010). Selection criteria Randomised trials comparing different suture materials for perineal repair after vaginal delivery. Data collection and analysis Two review authors independently assessed trial quality and extracted data. Main results We included 18 trials with 10,171 women; comparisons included: catgut with standard synthetic (nine trials), rapidly absorbing synthetic (two trials), and glycerol impregnated catgut sutures (two trials); and standard synthetic sutures with rapidly absorbing synthetic (five trials) and monofilament sutures (one trial). Compared with catgut, standard synthetic sutures were associated with less pain up to three days after delivery (risk ratio (RR) 0.83, 95% confidence interval (CI) 0.76 to 0.90); and less analgesia up to ten days postpartum (RR 0.71, 95% CI 0.59 to 0.87). More women with catgut sutures required resuturing (15/1201) compared with synthetic sutures (3/1201) (RR 0.25, 95% CI 0.08 to 0.74); while more women with standard synthetic sutures required the removal of unabsorbed suture material (RR 1.81, 95% CI 1.46 to 2.24). Comparing standard synthetic with rapidly absorbing sutures, short- and long-term pain were similar; in one trial fewer women with rapidly absorbing sutures reported using analgesics at 10 days (RR 0.57, 95% CI 0.43 to 0.77). More women in the standard synthetic suture group required suture removal compared with those in the rapidly absorbed group (RR 0.24, 95% CI 0.15 to 0.36). There was no evidence of significant differences between groups for long-term pain (three months after delivery) or for dyspareunia at three, or at six to 12 months. When catgut and glycerol impregnated catgut were compared, results were similar for most outcomes, although the latter was associated with more short-term pain. One trial examining monofilament versus standard polyglycolic sutures found no differences for most outcomes. Authors' conclusions Catgut may increase short-term pain compared with synthetic sutures. There were few differences between standard and rapidly absorbing synthetic sutures but more women needed standard sutures removing. For other materials, there was insufficient evidence to draw conclusions. Findings should be interpreted in the context of the related Cochrane review on suturing techniques

    Integrating Balanced Scorecard and Simulation Modelling to Improve Emergency Department Performance in Irish Hospitals

    In the healthcare sector, there is a requirement for innovative solutions in managing the high levels of complexity and uncertainty within Emergency Departments (EDs). Simulation modeling is currently seen as a competent means of analyzing EDs, which allows changes effects to be understood and predicted more easily. The Balanced Scorecard (BSC), well-known performance management concept, has become a steering method in approaching new improvement cycles. This paper presents a methodology that integrates BSC and simulation modeling to improve the performance of ED in a University Hospital in North Dublin. BSC design began with understanding patient\u27s needs, ED activities, as well as training and development programs. Then a detailed simulation model was developed and integrated with the BSC to provide a comprehensive decision support system. This integrated model can be used for evaluation of various decisions in emergency area. The developed integrated model is also a tool for improvement
