    An investigation into the method of treatment for chronic diseases of the heart by physical exercises

    In a brief and all too imperfect fashion, is a summary of the conclusions to which I came after a period of more than three months spent largely in the investigation-of this matter. It is perhaps impossible altogether to' eliminate bias on one side or the other, but my endeavour has been to . state the facts as they offered themselves to me, in as fair and impartial a manner as possible.And yet but a small part of the subject has been touched. What' is the physiological explanation of these variations - in the bulk of the heart, if that interpretation of the physical signs be admitted - in the area of cardiac dullness if this is held to be all that we can vouch for. Is the heart, too, a much more readily moveable body than most of us have hitherto imagined, and are there other movements proper to it besides systole and diastole? What of this rhythmic periodic cycle of which we now begin to hear whispers? These and many others are questions of great and indeed possibly of vital interest. But they are hardly questions to be settled by the clinician & it is from that point of view that this paper has been written. -Once more, what is the bearing of this new Treatment, if we admit its efficacy, upon the older routine administration of Digitalis and other drugs? Here perhaps an opinion may he expressed, and it is that though the new Treatment may - and probably will prove of inestimable value, as an adjuvant to the rational exhibition of Digitalis, Strophantus & Strychnine, yet in the great proportion of cases it will neither be advisable or yet possible to subject the patient to the Treatment in a satisfactory and competent manner . Like so many other "cures" this seems destined to remain altogether beyond the reach of the great majority of our patients, and yet to be of real use and value to those, more favoured of fortune whose means will allow them to give it a full and fair trial

    Intensity-dependent reductions in resting blood pressure following short-term isometric exercise training

    To reduce resting blood pressure, a minimum isometric exercise training (IET) intensity has been suggested, but this is not known for short-term IET programmes. We therefore compared the effects of moderate- and low-intensity IET programmes on resting blood pressure. Forty normotensive participants (22.3 ± 3.4 years; 69.5 ± 15.5 kg; 170.2 ± 8.7 cm) were randomly assigned to groups of differing training intensities [20%EMGpeak (~23%MVC, maximum voluntary contraction, or 30%EMGpeak (~34%MVC)] or control group; 3 weeks of IET at 30%EMGpeak resulted in significant reductions in resting mean arterial pressure (e.g. −3.9 ± 1.0 mmHg, P 0.05). Moreover, after pooling all female versus male participants, IET induced a 6.9-mmHg reduction in systolic blood pressure in female participants, but only a 1.5-mmHg reduction in systolic blood pressure in male participants, although the difference was not significant. An IET intensity between 20%EMGpeak and 30%EMGpeak is sufficient to elicit significant resting blood pressure reductions in a short-term training period (3 weeks). In addition, sexual dimorphism may exist in the magnitude of reductions, but further work is required to confirm this possibility, which could be important in understanding the mechanisms responsible

    Exploring the role of hydrological pathways in modulating multi-annual climate teleconnection periodicities from UK rainfall to streamflow

    An understanding of multi-annual behaviour in streamflow allows for better estimation of the risks associated with hydrological extremes. This can enable improved preparedness for streamflow-dependant services, such as freshwater ecology, drinking water supply and agriculture. Recently, efforts have focused on detecting relationships between long-term hydrological behaviour and oscillatory climate systems (such as the North Atlantic Oscillation – NAO). For instance, the approximate 7 year periodicity of the NAO has been detected in groundwater-level records in the North Atlantic region, providing potential improvements to the preparedness for future water resource extremes due to their repetitive, periodic nature. However, the extent to which these 7-year, NAO-like signals are propagated to streamflow, and the catchment processes that modulate this propagation, are currently unknown. Here, we show statistically significant evidence that these 7-year periodicities are present in streamflow (and associated catchment rainfall), by applying multi-resolution analysis to a large data set of streamflow and associated catchment rainfall across the UK. Our results provide new evidence for spatial patterns of NAO periodicities in UK rainfall, with areas of greatest NAO signal found in southwest England, south Wales, Northern Ireland and central Scotland, and show that NAO-like periodicities account for a greater proportion of streamflow variability in these areas. Furthermore, we find that catchments with greater subsurface pathway contribution, as characterised by the baseflow index (BFI), generally show increased NAO-like signal strength and that subsurface response times (as characterised by groundwater response time – GRT), of between 4 and 8 years, show a greater signal presence. Our results provide a foundation of understanding for the screening and use of streamflow teleconnections for improving the practice and policy of long-term streamflow resource management

    Blood pressure lowering effects of a novel isometric exercise device following a 4-week isometric handgrip intervention

    Background: Hypertension is the leading risk factor for global mortality. Isometric resistance exercise training reduces blood pressure (BP). However, the protocols used are often limited by cost/immobility and the use of rigid exercise modalities. In response, a novel more versatile, isometric exercise (IE) device, the IsoBall (IB) was created. Purpose: The aim of this study was to test the BP-lowering effectiveness of this prototype. Methods: Twenty-three healthy participants (29.10±2.19 years old, 173.95±3.83 cm, 75.43±5.06 kg, SBP 127.10±10.37 mmHg, DBP 70.40±6.77 mmHg) were randomly allocated to either a control group (CON) or 2 isometric handgrip (IHG) training groups that used the Zona plus (ZON) and IB devices. The intervention groups completed 3 sessions each week of 4, 2 min IHG at 30% maximal voluntary contraction, with a 1-min rest, for 4 weeks. Resting BP, heart rate (HR) and IHG strength were measured in all groups at baseline and postintervention. Results: Postintervention systolic BP (SBP) was significantly lower in both ZON (114.5±8.2 mmHg, p = 0.000) and IB (119.9±7.0 mmHg, p = 0.000) compared to control (131.0±12.4 mmHg). Postintervention diastolic BP (DBP) was reduced in both intervention groups (ZON 66.6±7.4 mmHg, p = 0.004; IB 65.7±10.0 mmHg, p = 0.012) compared to CON (71.1±8.8 mmHg). Mean arterial pressure (MAP) was reduced in both groups (ZON 82.6±6.8 mmHg, p = 0.000; IB 84.3±9.1 mmHg, p = 0.000) compared to control (91.0±9.7 mmHg). No significant changes were seen in HR or strength (p > 0.05). Conclusion: The results of this study indicate that both the ZON and IB devices elicit significant SBP, DBP and MAP reductions. Despite the ZON group having larger reductions in BP, no significant differences were found between the two devices. Thus, this study indicates the IB device to be an effective alternative to the ZON that can also be used to perform other IE modalities

    Functioning of Coastal River-Dominated Ecosystems and Implications for Oil Spill Response: From Observations to Mechanisms and Models

    Coastal river-dominated oceans are physically complex, biologically productive, and intimately connected to human socioeconomic activity. The Deepwater Horizon blowout and subsequent advection of oil into coastal waters of the northern Gulf of Mexico (nGOM) highlighted the complex linkages among oceanographic processes within this river-dominated system and knowledge gaps about it that resulted in imprecise information on both oil transport and ecosystem consequences. The interdisciplinary research program implemented through the CONsortium for oil exposure pathways in COastal River-Dominated Ecosystems (CONCORDE) is designed to identify and quantitatively assess key physical, biological, and geochemical processes acting in the nGOM, in order to provide the foundation for implementation of a synthesis model (coupled circulation and biogeochemistry) of the nGOM shelf system that can ultimately aid in prediction of oil spill transport and impacts. CONCORDE field and modeling efforts in 2015–2016 focused on defining the influence of freshwater input from river plumes in the nGOM. In situ observations, combined with field-deployed and simulated drifters, show considerable variability in the spatial extent of freshwater influence that is related to wind direction and strength. Increased primary production and particle abundance (a proxy for secondary production) was observed during the spring when nGOM shelf waters were becoming stratified. Zooplankton and marine snow displayed intense vertical and horizontal patchiness during all seasons, often aggregating near the halocline. Simulations of a neutrally buoyant tracer released offshore of the Mississippi Bight showed surface advection of low tracer concentrations onto the inner shelf under high river discharge, high stratification, and variable wind conditions compared to almost no advection onto the inner shelf under low discharge, negligible stratification, and generally northeasterly winds. The interconnectedness of environmental variables and biological activity indicate that multiple factors can affect the transport of oil and the resulting ecological impacts. The process-oriented understanding provided by CONCORDE is necessary to predict ecosystem-level impacts of oil spills, and these results are applicable to other river-dominated coastal systems worldwide that often support oil extraction activities
