    Beta herpes viruses in patients with cutaneous graft-versus-host disease

    Graft-versus-host disease is a major complication of allogenic bone marrow transplantation. Beta herpes viruses’ role in pathogenesis and prognosis of graft-versus-host disease is significant. Objective: To study beta herpes viruses evidence in skin biopsy specimens in graft-versus-host disease patients. Material and methods: Clinical features of 50 patients with cutaneous graft-versus-host disease were examined. Cytomegalovirus and human herpes virus 6 DNA detection in skin biopsy specimens was performed by PCR. Results: Correlation between human herpes virus 6 and poor prognosis in graft-versus-host disease patients has been found. Conclusion: Beta herpes virus detection after allogenic bone marrow transplantation could be important in prognosis of graft-versus-host disease

Cytomegalovirus and human herpes virus 6 DNA detection in skin biopsy specimens was performed by PCR. Results: Correlation between human herpes virus 6 and poor prognosis in graft-versus-host disease patients has been found. Conclusion: Beta herpes virus detection after allogenic bone marrow transplantation could be important in prognosis of graft-versus-host disease. Кожная реакция «трансплантат против хозяина» является опасным осложнением аллогенной трансплантации костного мозга, в патогенезе и течении которого особую роль уделяют бета-герпес-вирусным поражениям. Цель исследования – выявить наличие признаков поражения бета-герпес-вирусами в биоптатах кожи пациентов с кожной формой острой реакции «трансплантат против хозяина». Материалы и методы. Проведена оценка клинических данных больных с кожной формой острой реакции «трансплантат против хозяина» (50 пациентов). В биоптатах кожи методом ПЦР выделена вирусная ДНК цитомегаловируса и герпес вируса 6 типа. Результаты. При оценке клинических данных совместно с признаками бета-герпес-вирусных поражений была выявлена корреляция между наличием герпес-вируса 6 типа и неблагоприятным прогнозом заболевания. Заключение. Определение наличия бета-герпесвирусных поражений в коже пациентов, перенесших аллогенную трансплантацию костного мозга, может являться важным прогностических фактором течения посттрансплантационного периода.

    In 1996–2009, the incidence of syphilis in the North-West region of Russia has declined and changed its structure. The frequency of neurosyphilis and latent forms increased, especially of late and unspecified latent syphilis. Differences in the structure of syphilis between the Leningrad region and St. Petersburg have been identified, which consisted in the reduction of primary syphilis and an increase in early latent syphilis in comparison with the city.В 1996–2009 гг. снижалась заболеваемость сифилисом в Северо-Западном регионе России и изменилась его структура. Увеличилась частота нейросифилиса и скрытых форм, особенно позднего и неуточненного скрытого сифилиса. Выявлены различия в структуре сифилиса между Ленинградской областью и Санкт-Петербургом, которые состояли в снижении первичного и увеличении раннего скрытого сифилиса в области по сравнению с городом

    Micosis fungoides is a primary skin lymphoma characterized with indolent disease course and favorable prognosis. Опіу at some patients one can observe aggressive development of the disease to malignant stage with the exracutaneous outspread. the modern data about the prognostic factors are presented in the review. Disclosure of these factors allows to forecast the course of disease. there is given attention to integral estimation of survival rates on the ground of tNMB-staging sand estimation of the CUP-index. Definition of combination of different prognostic factors would allow to create prognostic models enabling to diagnose on the early stages of disease the patients with high risk of progression of mycosis fungoides

    Application of external drugs comprising silver compounds for the treatment of pyodermic patients

    An external anti-bacterial therapy is selected for pyodermic patients empirically. Silver sulfathiazole has an evident anti-inflammatory and anti-microbial effect against a broad range of microorganisms. Goal of the study. To assess the efficacy, safety and tolerability of a cream comprising 2% silver sulfathiazole vs. 3% tetracycline ointment for the treatment of pyodermic patients. Materials and methods. The study involved 50 pyodermic patients (at the mean age of 23.5 ± 4.1 years) including 31 (62%) male and 19 (38%) female patients. The patients were divided into two groups: Group I (n = 35) used the cream comprising 2% silver sulfathiazole while Group II (n = 15) was administered 3% tetracycline ointment. All of the patients were assessed for clinical and microbiological efficacy of the therapy. Results. The average patient recovery time in Group I amounted to 8.7 ± 0.9 days and 13.1 ± 2.7 days in Group II (р < 0.01). As many as 33 patients (94.3%) achieved clinical recovery and 2 patients (5.7%) achieved improvement by Day 10 in Group I; in Group II, clinical recovery was observed in 8 patients (53.3%), improvement - in 3 patients (20%), and no effect - in 4 patients (26.7%). Conclusion. The external anti-bacterial monotherapy of pyodermic patients with the cream comprising 2% silver sulfathiazole is more efficient than treatment with 3% tetracycline ointment. High clinical and microbiological efficacy of the cream comprising 2% silver sulfathiazole makes it possible to recommend it for empirical treatment of pyodermic patients

    Level of the proliferative activity of lymphocytes in case of mycosis fungoides and plaque parapsoriasis

    The level of the proliferative activity is an important predictor of a number of oncologic diseases; at the same time, its diagnostic value for mycosis fungoides was not adequately investigated. Goal. To study the proliferative activity of lymphocytes in the skin of patients suffering form mycosis fungoides and plaque parapsoriasis. Materials and methods. The level of CD3+ and Ki-67+ cells was determined in skin biopsy samples taken from patients suffering from mycosis fungoides (18 subjects) and plaque parapsoriasis (6 subjects) by means of immunohistochemistry. The epidermal to dermal skin cell proliferative activity ratio (Ki-67+) and cell proliferative activity index were calculated. The control group comprised 16 healthy people. Results. The epidermal to dermal skin cell proliferative activity ratio amounted to 10.9 in healthy people, 5.43 in patients suffering from plaque parapsoriasis, and 3.84 and 2.42 in patients suffering from mycosis fungoides, spotty and plaque and tumor stages, respectively. The percentage of proliferative CD3+Ki-67+ cells relative to the total number of T-lymphocytes amounted to 1% in the patients suffering from plaque parapsoriasis and 3% in the patients with mycosis fungoides, spotty stage. Conclusion. The epidermal to dermal skin cell proliferative activity ratio (Ki-67+) in patients suffering from mycosis fungoides at any stage is lower than that in healthy subjects and patients suffering from parapsoriasis. The cell proliferative activity index was higher in the patients with mycosis fungoides, plaque and tumor stage, than in the patients with parapsoriasis. The number of CD3+Ki-67+ cells was higher in the patients with mycosis fungoides, spotty and plaque and tumor stages, than in the healthy subjects and patients suffering from parapsoriasis
