    Kapal Jet Foil atau Pesawat Hydrofoil adalah merupakan produk teknologi tinggi yang di dalam rancang bangunnya menggunakan kombinasi dan modifikasi antara teknologi pesawat terbang dan teknologi perkapalan. Yang menjadikan Kapal Jet Foil lain dari kapal konvensional, 1adalah ter letak dengan dilengkapinya Kapal Jet Foil dengan strut dan foil yang dapat dirubah posisinya sehingga kapal dapat naik dan turun di permukaan air laut. Dengan adanya strut dan foil ini Kapal Jet Foil mampu berlayar dengan jelajah lambung a tau biasa disebut " Hullborne " maupun terbang di atas foilnya Foilborne ". Sebagai tenaga penggerak propulsi Kapal Jet Foi l digunakan mesin turbin gas dan water jet pump. Jet Foi l Propulsion Power diproduksi oleh dua buah turbin gas Allison 501 - K, masing - masing turbin ini dihubungkan dengan poros putaran tinggi ( High Speed Shaft ) dan reducing gear box ke water jet pump. Pada kondis i Hullborne, water jet pump memompa air masuk lewat Hullborne Water Intake II yang berada di lambung kapal dan dipompa keluar lewat nozzle di bagian belakang dengan kecepatan tinggi. Di sini terlihat bahwa salah satu dari sistem propulsi yaitu nozzle yang merupakan komponen terakhir dari sistem propulsor unit , ! yang dipakai untuk menaikkan energi kenetis dari aliran fluida. Karena pada prinsipnya gaya dorong pada Kapal Jet Foil dihasilkan oleh perbedaan momentum antara air masuk. dan momentum a.ir keluar pada propulsor system unit tersebut

Jet Foil Propulsion Power diproduksi oleh dua buah turbin gas Allison 501 -K, masing -masing turbin ini dihubungkan dengan poros putaran tinggi ( High Speed Shaft ) dan reducing gear box ke water jet pump. Pada kondisi Hullborne, water jet pump memompa air masuk lewat Hullborne Water Intake II yang berada di lambung kapal dan dipompa keluar lewat nozzle di bagian belakang dengan kecepatan tinggi. Pada kondisi foilborne water jet pump ini memompa air masuk lewat 11 Foilborne Water Intake 11 yang terletak pada sistem belakang dan sistem ini menutup dan menyatu dengan saluran water intake pada kondisi hullborne. Kemudian dialirkan/disemprotkan keluar dengan kecepatan tinggi melalui sebuah nozzle yang berfungsi menciptakan perbedaan momentum akhir dari Kapal Jet Foil. Di sini terlihat bahwa salah satu dari sistem propulsi yaitu nozzle yang merupakan komponen terakhir dari sistem propulsor unit , ! yang dipakai untuk menaikkan energi kenetis dari aliran fluida.     Lymphocyte-monocyte ratio as predictive factors for huvos scores in osteosarcoma extremities treated by neoadjuvan chemotherapy (cisplatin and doxorubicin)

    Background: Chemotherapy is a vital treatment in osteosarcoma but it’s responses are difficult to predict, and there are still no biomarkers that can estimate the prognosis of chemotherapy. Lymphocytes and monocytes are key immune cells which are examined on complete blood count test before chemotherapy and often associated with prognosis of various malignancies. Low lymphocyte-monocyte ratio (LMR) are associated with poor prognostics in some cancers.Methods: This study is cross-sectional retrospective analytics that was conducted at Sanglah Hospital from June to August 2018. The research subject was medical records of intramedullary conventional osteosarcoma patients which fulfil inclusion and exclusion criteria. In this study, LMR as independent variable and Huvos score as dependent variable.Results: The result in this study showed positive correlation between LMR before neoadjuvan Chemotherapy and Huvos score (r=0.500) with p<0.05.Conclusions: A positive correlation was found between LMR and Huvos scores. Low LMR before neoadjuvan chemotheraphy (<2.81) were correlated with low huvos scores (grade I and II)

    The role of aging, body mass index and estrogen on symptomatic lumbar osteoarthritis in post-menopausal women

    Background: Low back pain is a common complaint which is always found in old age due to degeneration process. Degeneration process of spine, especially in the lumbar region is called lumbar osteoarthritis (OA). Various factors are thought to be the cause of lumbar OA, including primarily due to hormonal changes of estrogen and increase of age and body mass index in postmenopausal women.Methods: The purpose of this study is to determine the role of hormonal changes pathogenesis in lumbar osteoarthritis in postmenopausal women and its correlation with age and body mass index. Cross sectional analytic method had been conducted to determine the role of age, body mass index and estrogen for lumbar OA in postmenopausal women. The study was conducted in Sanglah General Hospital from October 2015 until March 2016 by anthropometric measures and blood samples.Results: From 196 samples showed that the estrogen deficiency in postmenopausal women have a correlation with the symptomatic lumbar OA (r=0.252, p = 0.000). While age and the body mass index in postmenopausal women has a correlation with symptomatic lumbar OA (r = 0.150 and 0.198, p = 0.000 and 0.013).Conclusions: The aging process, body mass index and estrogen deficiency correlated with the symptomatic OA lumbar in post-menopausal women

    The relationship between tourniquet time and Il-6, D-Dimer and visual analogue scale in total knee arthroplasty

    Background: The use of tourniquet during TKA aside from having several advantages, is also accompanied by several risks. This study was performed to prove the relation between the duration of tourniquet application to inflammatory response, pro-thrombotic condition, and pain response of the patients undergoing TKA procedures.Methods: This study is a cross-sectional analytical experimental research. A femoral tourniquet was applied to 34 primary osteoarthritis (OA) patients undergoing the standard procedure of TKA and the duration of tourniquet application data was noted and collected. Pain assessment was performed by using Visual Analogue Scale (VAS), whereas blood samples are taken from the patients and IL-6 cytokine and D-dimer levels were identified at 12 hours and 24 hours post-operatively. Statistical analysis (Pearson correlation) was performed to see the correlation of tourniquet duration to the VAS, IL-6, and D-dimer serum level at 12 hours and 24 hours post-operatively.Results: The result of this study showed a positive correlation between the duration of tourniquet use with IL-6 at 12 hours post operation (r=0.359) and 24 hours post operation (r=0.658); with D-dimer at 12 hours post operation (r=0.491) and 24 hours post operation (r=0.483); and with VAS at 12 hours post operation (r=0.647) and 24 hours post operation (r=0.507) with p&lt;0,05. A positive correlation was found between tourniquet time and IL-6, D-dimer and VAS.Conclusions: The duration of tourniquet cut-off time of more than 122.5 minutes elevates IL-6 and D-Dimer which increases the risk for SIRS and DVT

    Predictors of Anxiety toward COVID-19 Delta Variant: A Cross- Sectional Study among Healthcare Providers in Java and Bali, Indonesia

    Health facilities are experiencing overcapacity, oxygen scarcity, and a limited number of healthcare providers due to the coronavirus disease 2019 (COVID-19), thus impacted on anxiety. This study aimed to determine predictors of anxiety among healthcare providers toward the Delta variant of COVID-19 in Indonesia. A cross-sectional study was conducted with 371 healthcare providers in Java and Bali Islands, and the snowball sampling technique was used. Data were collected using a questionnaire and distributed through social media (WhatsApp), then analyzed using univariate analysis, bivariate analysis (Chi-square test), and multivariate analysis (multiple logistic regression). The results showed that 81 (21.8%) respondents experienced anxiety. The workplace (AOR: = 0.617; p-value = 0.011), a history of confirmed positive for COVID-19 (AOR = 2.965; p-value<0.001), and the respondent's comorbidities (AOR = 8.753; p-value<0.001) were significantly associated with anxiety toward the Delta variant. Healthcare managers must regularly evaluate the psychological condition of their subordinates during the COVID-19 pandemic so that anxiety can be detected and overcome early through constructive self-adaptation and positive coping mechanisms.

    High expression levels of bone morphogenetic protein-2 and p-glycoprotein related with progressivity in patient with osteosarcoma

    Background: Osteosarcoma progression characterized by the presence of metastasis dan local recurrence. Several studies focused on bone morphogenetic protein-2 (BMP-2) and P-glycoprotein as protein that influence the progression of osteosarcoma, which are characterized by metastasis and local recurrence.Methods: This study was a cross sectional analytic study. This study looks at the relationship level of BMP-2 and P-glycoprotein with metastasis and local recurrence in osteosarcoma. Twenty three research subjects are paraffin blocks of osteosarcoma patients and secondary data was conducted through medical records to determine metastasis and local recurrence that occurs in these patients.Results: The result showed significant correlation between BMP-2 and P-glycoprotein with metastasis and local reccurence.Conclusions: BMP-2 and P-glycoprotein positivity as determined by immunohistochemistry, was a strong correlate of more rapid disease progression. It should be taken into consideration to identify a subgroup of osteosarcoma patients with poor outcome at the time of diagnosis. So it might be an important marker in planning innovative chemotherapeutic regimens

    Higher RANK ligand/osteoprotegerin ratio results in lower callus diameter and osteocalcin expression on the healing of femur fracture between diabetic and nondiabetic rats using intramedullary wire

    Background: Diabetes with uncontrolled glucose level offer a significant risk of acute or chronic complications. One of the chronic complications is a decrease in bone mineral density which is regulated by RANKL and OPG balance. Diabetes is estimated to slow the healing process of fracture. Both are related to fracture risk and fracture healing process. Fracture healing is characterized by measurement of callus diameter and osteocalcin expression. Aim: This research aimed to find out the difference in RANKL/OPG ratio, callus diameter and osteocalcin expression on the fracture healing in diabetic and nondiabetic rats. Methods: This is an experimental randomized post-test only with 32 Wistar rats. The population was divided into two groups, the control group did not get treatment and the second group was the diabetes modeling. On the 7th day, both groups were examined for their ratio of RANKL/OPG using ELISA and fracture was performed. On the 28th day, the rats were sacrificed, and the effects were assessed using histopathology examination of callus diameter and immunohistochemistry examination of osteocalcin expression at the fracture area. Result: Higher median ratio of RANKL/OPG in diabetic than nondiabetic rats with p = 0,000 (p &lt;0,05). The diameter of callus on the healing of femur fracture in the nondiabetic group was higher than the diabetic group with p = 0,000 (p &lt;0.005). Osteocalcin levels in nondiabetic group were higher than diabetic group with p = 0,000 (p &lt;0.005). There was a negative correlation between RANKL/OPG ratio with callus diameter and osteocalcin level with a correlation coefficient of -0.701 and -0.652 respectively with p = 0,000 (p &lt;0,05). Conclusions: Level of RANKL/OPG ratio has a negative correlation with callus size and osteocalcin expression on healed femur fractures in diabetic rats model.</p

    Vascular Endothelial Growth Factor (VEGF) on rabbit meniscus injury in the white-white zone expressed higher blood vessels distribution and type 1 collagen bridging post meniscal suturing

    Background: Meniscus injury can occur in red - red zone with good prognostic as well as in white - white zone with bad one. General treatment of meniscus injury in white – white is menisectomy. It will destroy the articular surface of the knee joint cartilage. Administration the Vascular Endothelial Growth Factor (VEGF) will increase the proliferation of endothelial cells, fibroblast cells, collagen fibers type 1, and good prognostic for the healing of meniscus injury in white – white zone. Aim: The study aims to explore the effect of VEGF on rabbit meniscus injury. Methods: This research implemented an experimental study with randomized post-test control group design which used 38 male New Zealand Rabbits. There were two groups (18 rabbits as a control group and 18 rabbits as treatment one). They were adapted for one week, then control group had a sharp incision and sutured in white – white zone meniscus only, and treatment group had administration VEGF after done a sharp incision and sutured in white – white zone meniscus. In the next three weeks, all rabbits have been euthanized and then examined the meniscus for the expression of blood vessels distribution and the bridging of collagen type 1 by histopathology and immunohistochemistry examination. Results: The average median number for blood vessels distribution after administration VEGF in the sutured white-white zone was about 11.00 (interquartile range 2.00) and without VEGF was about 5.00 (interquartile range 1.00). In the treatment group with VEGF, 18 rabbits (100%) showed bridging collagen type 1, and the control one showed in 6 rabbits (33.33%). An inferential test for blood vessels distribution in white – white zone meniscus between treatment group with VEGF and control group without VEGF was significantly different (p=0.000). Analysis for bridging collagen type 1 between treatment group with VEGF and control group without VEGF showed significant differences (p=0.000). Conclusion: Administration VEGF on rabbit meniscus injury in the post suturing white-white zone expressed higher blood vessels distribution and bridging collagen type 1 compared with suturing only without VEGF. </p
