Hernie de Spiegel: a propos d’un cas
Introduction: La hernie de Spiegel ou hernie ventrale latérale est une déhiscence inhabituelle apparaissant sur la ligne ou fascia semi-lunaire de Spiegel. C’est une entité clinique rare, représente 0.10 à 1% des hernies. Aussi, nous a-t-il paru opportun de rapporter ce cas colligé dans le service de chirurgie B du CHU Hassan II de Fès. augmentant progressivement de volume, Une hernie de Spiegel a été suspectée à l’examen clinique, et le diagnostic d’éventration antérolatérale gauche a été retenu à la tomodensitométrie abdominale. Une cure de la hernie par plaque de prolène a été réalisée et les suites opératoires étaient simples. Conclusion: La hernie de Spiegel est une affection rare, son diagnostic clinique peut être difficile. Elle est asymptomatique dans 90% des cas et Son diagnostic positif est radiologique. Le
risque d’étranglement non négligeable impose un traitement chirurgical une fois le diagnostic est confirmé
The Effectiveness of Online Course Intervention to Improve Knowledge of Antimicrobial Resistance among Dental Students, in Comparison to Reference Group Using a Randomized Controlled Trial
AIM: This study aimed to assess the effectiveness of a recognised antimicrobial resistance (AMR) online module on knowledge and perception among dental students, using a randomised controlled trial study design.
METHODS: Dental students (n = 64, aged 21-25 years) in clinical years agreed to participate in this triple-blinded, parallel, randomised controlled trial. There were 34 students in the study group and 30 students in the control group. The study group participated in an online course covering information about AMR, while students in the control group received another online course about microorganisms in dentistry. Both groups were assessed three times using online questionnaires: before the intervention (T1), after the intervention (T2), and two months later (T3). Each one of T1, T2 and T3 had 22 questions. The questions were repeated each time in T1, T2, and T3 asking about AMR but with different question format, to avoid the possibility of students to memorise the answers.
RESULTS: The mean (m) of correct answers for all students on T1 was 12.56, with standard deviation (SD) of 3.2. On T2, m = 14.03 and SD = 3.85, and on T3, m = 14.36 and SD = 3.71. Scores ranged from 0 to 22. The participants in the study and control groups showed significant score improvements from T1 to T2, immediately after the intervention, but there was no significant difference between T2 and T3. The study group students’ scores did not improve significantly from T1 to T3, in contrast to the control group students’ scores. More importantly, there was no significant difference in improvement from T1 to T2 when comparing the study and control groups.
CONCLUSION: Online courses might not be reliable learning methods for ensuring the optimal levels of AMR knowledge that are needed by dental practitioners
Occurrence and sources of aliphatic hydrocarbons in surface soils from Riyadh city, Saudi Arabia
AbstractSoil particles contain a variety of anthropogenic and natural organic components derived from many sources such as industrial and traffic fossil fuel emissions and terrestrial biota. The organic contents of soil and sand from the Arabian region have not fully characterized. Thus, samples of fine soil particles (sieved to <125μM) were collected from the Riyadh area in November 2006 (late summer) and February 2007 (late winter). The samples were extracted with a mixture of dichloromethane/hexane and analyzed by gas chromatography–mass spectroscopy (GCMS) in order to characterize the chemical composition and sources of aliphatic hydrocarbons. The results showed that both anthropogenic and natural biogenic inputs were the major sources of the aliphatic hydrocarbons in these extracts. Vehicular emission products and discarded plastics were the major anthropogenic sources in the fine particles of the soils and ranged from 64% to 96% in November 2006 and from 70% to 92% in February 2007. Their tracers were n-alkanes, hopanes, sterane, plasticizers and UCM. Vegetation was also a major natural source of hydrocarbon compounds in samples ranging from ∼0% to18% in November 2006 and from 1% to 13% in February 2007 and included n-alkanes and triterpenoids
GIS as an Efficient Tool to Manage Educational Services and Infrastructure in Kuwait
ABSTRACT The State of Kuwait Ministry of Education (MoE) has clearly defined land use standards for the location of public schools, and an inventory of reserved lands for future facilities. Unless, there is a geographical efficient tool to manage and plan the education system in a rapidly developing country such as Kuwait there will be huge deficit in such services. Geographic Information Systems (GIS) was used to fill in this gap and effectively evaluate and analyze their facilities and unoccupied lands to ensure they continue to meet the population and future needs of Kuwaiti students. This paper utilized the GIS to inventory, map, and analyze MoE facilities and unoccupied land reservations with a goal of improved planning and decision making. Unfortunately, the initial spatial analysis of the data showed huge percent of districts that have no schools failing to meet the minimum standard of the MoE including kindergartens, primary schools, intermediate schools for girls, intermediate schools for boys, secondary schools for girls and secondary schools for boys at 72%, 71%, 48%, 43%, 54%, and 55% respectively. Such critical results will enable the decision makers to prioritize the immediate action of relocation the schools or widen the services and accessibility. Moreover, the analysis of the data showed a critical and immediate need to reserve land for five districts where they are heavily populated and lacked reserved land. However, based on long term land use plans, there is an urgent need to relocate some land and reserve others to meet the future urbanization plans and population growth
Understanding Preclerkship Medical Students’ Poor Performance in Prescription Writing
Objectives: This study aimed to explore reasons for poor performance in prescription writing stations of the objective structured practical examination (OSPE) and absenteeism in prescription writing sessions among preclerkship medical students at the Arabian Gulf University (AGU) in Manama, Bahrain. Methods: This descriptive study was carried out between September 2014 and June 2015 among 157 preclerkship medical students at AGU. Data were collected using focus group discussions and a questionnaire with closed- and open-ended items. Results: All 157 students participated in the study (response rate: 100.0%). The most frequently cited reasons for poor performance in OSPE stations were an inability to select the correct drugs (79.6%), treatment duration (69.4%), drug quantity (69.4%) and drug formulation (68.2%). Additionally, students reported inadequate time for completing the stations (68.8%). During focus group discussions, students reported other reasons for poor performance, including examination stress and the difficulty of the stations. Absenteeism was attributed to the length of each session (55.4%), lack of interest (50.3%), reliance on peers for information (48.4%) and optional attendance policies (47.1%). Repetitive material, large group sessions, unmet student expectations and the proximity of the sessions to summative examinations were also indicated to contribute to absenteeism according to open-ended responses or focus group discussions. Conclusion:This study suggests that AGU medical students perform poorly in prescription writing OSPE stations because of inadequate clinical pharmacology knowledge. Participation in prescription writing sessions needs to be enhanced by addressing the concerns identified in this study. Strategies to improve attendance and performance should take into account the learner-teacher relationship

Mesoscale and Nutrient Conditions Associated with the Massive 2008 Cochlodinium polykrikoides Bloom in the Sea of Oman/Arabian Gulf
Cochlodinium polykrikoides formed large blooms in the coastal waters of Oman from October 2008 through mid-January 2009, and satellite images from Aqua-MODIS and region-wide reports suggest that this bloom was found throughout the Arabian Gulf and Sea of Oman for more than 10 months. The unusual occurrence of this species appears to have supplanted the more regularly occurring bloom species, Noctiluca scintillans, in 2008–2009. For the first 2 weeks of the coastal Omani bloom, C. polykrikoides abundance was near monospecific proportions, with cell densities ranging from 4.6 × 103 to 9 × 106 cells L−1 and very high levels of chlorophyll a (78.0 μg L−1) were also recorded. The regional progression of the bloom likely began with stronger than normal upwelling along the Iranian and northern Omani coasts during the southwest monsoon in late summer, followed by discharge of unusually warm coastal plume water along the coast of Oman with the reversal of monsoonal winds in late October. The occurrence and persistence of high densities of C. polykrikoides in Oman coastal water were also significantly influenced by an elevated nutrient load and warmer than normal temperatures. Concentrations of nutrients, especially NH4 +, urea, PO4 3−, and organic nitrogen and phosphorus, were manyfold higher than observed in the year prior or since. These findings suggest that mesoscale features were important in bloom dynamics more regionally, but locally the bloom was sustained by nutrient enrichment supplemented by its mixotrophic capabilities
Carbonic anhydrase II based biosensing of carbon dioxide at high temperature: an analytical and MD simulation study
Concentration of carbon dioxide (CO2) in the atmosphere has increased significantly due to anthropogenic
activities and attributed as a major factor to global warming. Its detection by biosensing methods will provide an
alternative for the assessment of CO2 concentration. Biomineralization of CO2 is one of the available methods for
the biological conversion of CO2 to carbonate using a highly active enzyme, carbonic anhydrase II (CAII). CAII was
used for the carbonation reaction to convert CO2 to CaCO3. The precipitation of calcium carbonate (CaCO3) was
promoted in the presence of the CAII at 325 K. CAII showed an enhanced formation of solid CaCO3 through the
acceleration of CO2 hydration rate at 325 K. Furthermore, the electrocatalytic properties of glassy carbon electrode
enable us to determine the reduction peak potential values of CO2 through cyclic voltammetry at –1.75 and 0.3 V at
325 K. Molecular dynamic (MD) simulations were performed each at 50 ns time scale provided a deeper insight into
the molecular basis of the CAII interaction with CO2 at different temperatures, highlighted that the CAII can detect
CO2 up to 325 K. We assume that CAII could be an effective and economical biosensor for biomineralization of CO2
at high temperature 325 K
Doubly Orthogonal Wavelet Packets for Multi-Users Indoor Visible Light Communication Systems
Visible Light Communication (VLC) is a data communication technology that modulates the intensity of the light to transmit the information mostly by means of Light Emitting Diodes (LEDs). The data rate is mainly throttled by the limited bandwidth of the LEDs. To combat, Multi-carrier Code Division Multiple Access (MC-CDMA) is a favorable technique for achieving higher data rates along with reduced Inter-Symbol Interference (ISI) and easy access to multi-users at the cost of slightly reduced compromised spectral efficiency and Multiple Access Interference (MAI). In this article, a multi-user VLC system is designed using a Discrete Wavelet Transform (DWT) that eradicates the use of cyclic prefix due to the good orthogonality and time-frequency localization properties of wavelets. Moreover, the design also comprises suitable signature codes, which are generated by employing double orthogonality depending upon Walsh codes and Wavelet Packets. The proposed multi-user system is simulated in MATLAB software and its overall performance is assessed using line-of-sight (LoS) and non-line-of-sight (NLoS) configurations. Furthermore, two sub-optimum multi-users detection schemes such as zero forcing (ZF) and minimum-mean-square-error (MMSE) are also used at the receiver. The simulated results illustrate that the doubly orthogonal signature waveform-based DWT-MC-CDMA with MMSE detection scheme outperforms the Walsh code-based multi-user system
