    Factors Affecting the Designation of Cerebrovascular Diseases as Work-Related in Administrative Litigation

    The purpose of this study was to identify factors that could be used as standardized criteria for evaluating occupational diseases in initial assessments or requests for examination. Using 100 administrative litigation cases on the work-relatedness of cerebrovascular diseases (CVDs) by the Seoul Branch of the Korea Labor Welfare Corporation (KLWC) from 1997 to 2002, we estimated the relationship between the investigated variables and designation of the work-relatedness of the CVD. As for the age, the odds ratio of the acceptance rate of a case as work-related in subjects over 60 yr of age was 0.08 (95% CI, 0.01-0.75), which was compared to subjects under 30 yr of age. Regarding working hours, the odds ratio of the acceptance rate of a case as work-related in CVDs in those over 56 hr was 9.50 (95% CI, 1.92-47.10) when compared to those less than 56 hr. As for the benefit type, the odds ratio of the acceptance rate of a case as work-related in medical benefits was 5.74 (95% CI, 1.29-25.54), compared to survivor benefits. As for the criteria for defining situations as work overload, the odds ratio of the acceptance rate of a case as work-related in injured workers was 12.06 (95% CI, 3.12-46.62), compared to that in non-injured workers. Our findings show that the criteria for defining situations of work overload played an important role in assessing the work-relatedness of CVDs in administrative litigation, and it is necessary to make the scientific evidence on judgement of work-relatedness on overwork

    Estrogen Receptor-1 Genetic Polymorphisms for the Risk of Premature Ovarian Failure and Early Menopause

    Background: The aim of this study was to investigate the role of the estrogen receptor 1 (ESR1) genetic polymorphisms for early menopause that was classified as premature ovarian failure (POF) and early menopause (EM) and to examine whether the associations of ESR1 genetic variants are different for POF and EM. Methods: We selected 100 POF cases and matched 100 EM cases and 200 normal menopause (NM) controls from the Korean Multi-Center Cohort. Among them, we restricted idiopathic POF and EM cases vs NM controls by excluding POF/EM cases with medical/surgical causes. The XbaI (rs9340799) and PvuII (rs2234693) in the ESR1 gene were genotyped. The single-nucleotide polymorphism (SNP) and haplotype effects were analyzed by multivariate logistic regression and haplotype analysis. Also nominal polytomous logistic regression was used to find whether ESR1 genetic variants are differently associated with POF and EM. Results: The global p values for idiopathic POF and EM were 0.08 and 0.39 (SNP-based), and <0.001 and 0.12 (haplotype-based), respectively. The XbaI genetic variant containing the X allele was marginally significantly associated with a reduced risk of idiopathic POF (OR=0.6, 95% CI 0.3-1.0). The P-x haplotype and diplotypes significantly decreased the risk of idiopathic POF (OR=0.5, 95% CI 0.2-0.9; OR 0.4, 95% CI 0.2-0.9, respectively). In contrast from POF, the P-x haplotypes and diplotypes insignificantly increased the risk for both idiopathic EM (p(polytomous)=0.009 for P-x haplotype; p(polytomous)=0.02 for P-x diplotypes).     Validity of Self-reported Healthcare Utilization Data in the Community Health Survey in Korea

    To evaluate the sensitivity and specificity of Community Health Survey (CHS), we analyzed data from 11,217 participants aged ā‰„ 19 yr, in 13 cities and counties in 2008. Three healthcare utilization indices (admission, outpatient visits, dental visits) as comparative variables and the insurance benefit claim data of the Health Insurance Review & Assessment Service as the gold-standard were used. The sensitivities of admission, outpatient visits, and dental visits in CHS were 54.8%, 52.1%, and 61.0%, respectively. The specificities were 96.4%, 85.6%, and 82.7%, respectively. This is the first study to evaluate the validity of nationwide health statistics resulting from questionnaire surveys and shows that CHS needs a lot of efforts to reflect the true health status, health behavior, and healthcare utilization of the population

    Characterization of the mitochondrial complete genome of Korean indigenous catfish, Liobagrus hyeongsanensis (Siluriformes: Amblycipitidae)

    Liobagrus hyeongsanensis, Korean indigenous catfish, was reported as a new species in 2015. The complete mitochondrial DNA sequence of L. hyeongsanensis was sequenced by next-generation sequencing (NGS) analysis. The mitochondrial genome was assembled with 16,529ā€‰bp in length and encoded 13 protein-coding genes (PCGs), 22 tRNAs, two rRNAs, and one control region (D-loop). Also, the gene structures such as gene order and content were totally identical with the congeneric species. Molecular phylogenetic analysis determined the taxonomical position of L. hyeongsanensis in species level among the genus Liobagrus

    Turnover Intention and Its Related Factors of Employed Doctors in Korea

    Employment turnover among doctors at healthcare facilities negatively influences healthcare provision, facility management, and staffing. To support institutional and policy change, turnover intentions and its related factors of employed doctors were evaluated with 2016 Korean Physician Survey (n = 2719) in Korea. About 30.5% intended a turnover within two years. The significant related factors by multivariate analysis via binary logistic regression were gender, age, specialty, type of facility, length of current employment, usual number of hours worked per week, and income satisfaction. The odds of reporting turnover intention are 46.2% greater for males than females and 55.5% greater for aged 30&ndash;39 than aged 40&ndash;49. The odds are 28.9% smaller for support medicine than internal medicine. The odds are 224.2% greater for those employed at tertiary hospitals than those employed at clinics, but the odds are 34.0% smaller for convalescent hospital employment than general hospital employment. The number of years of current employment and income satisfaction each negatively, and the number of hours worked per week positively, related to turnover intentions. Fair compensation and performance evaluation systems and reasonable working hours should be guaranteed at healthcare facilities to reduce turnover, and institutional and policy measures should be implemented to improve workplace environmental quality

    The Heat Transfer Modeling for Minimization of Thermal Necrosis in Hip Resurfacing Arthroplasty

    In the past, the only option for replacing broken or otherwise malfunctioning hip bones was total hip replacement (THR), which involved the removal of the body's natural hip joint and replacing it with an entirely synthetic joint. However, the complications involved with such extensive surgery have led to the push for less invasive therapies. One such idea that has gained popularity for younger patients is hip resurfacing arthroplasty (HRA), which focuses on repairing the joint rather than entirely replacing it. However, there are some issues with HRA, namely thermal bone necrosis due to the heat of polymerization during cement hardening. This paper examines this necrosis as well as the thermal trends of hip resurfacing surgery through the use of COMSOL, a computer aided engineering tool. In COMSOL, a 2D-axisymetric geometry was developed to model the leg bone, cement, ball, cap, and the hipbone. Boundary condition and initial condition was set to portray a realistic surgical environment. With the objective of minimizing bone necrosis, the model's parameters were adjusted to simulate a variety of material properties (different prosthetic material, and different bone conditions), as well as plausible surgical conditions (pre-cooled cement, and convective cooling). Sensitivity analysis was also conducted to gain a better understanding of the thermal tendencies of HRA. Based on the data from these simulations we were able to provide some insight into what approaches may be best for minimizing the extent of the thermal necrosis. The following paragraphs describe the most significant among these conclusions. First of all, we found that precooling the implant from room temperature to 5 degrees C resulted in a drop in the maximum temperature by 7 degrees C. It appears that precooling the cement in addition to the implant had no additional significant temperature reduction. Also, our sensitivity analysis revealed that our system was most sensitive to changes in precooling temperature. All of these factors seem to suggest that precooling may be an effective means for reducing thermal bone necrosis. Secondly, simulations of cancellous bone showed a maximum temperature lower than that of normal bone. As cancellous bone is typical of elderly patients, these results suggest that there is a lower risk of complications due to thermal necrosis during HRA for elderly patients than for younger individuals. Third, when varying our parameters, we often incurred computational failure due to the presence of "hot spots," small regions of the cement that reached extremely high values. Our sensitivity analysis revealed that our system at its default parameter values was extremely close to values that resulted in these hot spots. While it's questionable if these hot spot events are consistent with realistic physics, the topic deserves further investigation considered the potentially damaging results. While our model does not conclusively provide guidelines for reducing thermal necrosis, it does provide some ideas for successful prevention. Computational analysis as a first step helps us understand the thermal effects of HRA and sets the stage for subsequent experimentation

    Association between Exposure to Ambient Air Pollution and Age-Related Cataract: A Nationwide Population-Based Retrospective Cohort Study

    This study aimed to investigate the association between ambient air pollutants and cataracts in the general population aged 50 years or older using data from the Korean National Insurance Service&mdash;National Sample Cohort. Cataract patients were defined as those diagnosed by a physician and having undergone cataract surgery. After matching the average concentrations of PM2.5, PM10, NO2, CO, SO2, and O3 in residential areas, the association between quartile level of air pollutants and incidence of cataract was analyzed using a multivariate Cox-proportional hazard risk model. Among the 115,728 participants, 16,814 (14.5%) were newly diagnosed with cataract and underwent related surgery between 1 January 2004, and 31 December 2015. Exposure to PM10, NO2, and SO2 was positively associated with cataract incidence, while O3 was negatively associated. The adjusted hazard ratio (HR) with 95% confidence interval was 1.069 (1.025&ndash;1.115) in PM10 and 1.080 (1.030&ndash;1.133) in NO2. However, the association between cataract and the quartile of PM2.5 measured during one year in 2015 was not clear. The HR of female participants aged 65 or older was significantly increased according to quartile of air pollutants. We identified exposure to PM10, NO2, SO2, and O3 associated with cataract development in Korean adults aged &ge; 50 years. This information may be helpful for policymaking to control air pollution as a risk factor for eye health

    Factors Affecting Korean Physician Job Satisfaction

    This study examines job satisfaction of physicians in Korea and investigates factors affecting their satisfaction. The majority of the past studies tend to cover few minor factors&mdash; including stress and occupation professionality or insufficient scale of respondents in particular regions&mdash;thus leading to restricted explanations on job satisfaction of the overall physician pool in Korea. This study examines the level of job satisfaction of physicians in Korea and factors affecting their satisfaction by using the &lsquo;2016 Korean Physician Survey (KPS)&rsquo; data which included all physicians in Korea. Ordinal logistic regression analysis was conducted in this study in order to identify the factors affecting job satisfaction of physicians in terms of care environment attributes. These attributes included autonomy for care delivery, colleagues/staff/patient relations, income, healthcare resources, social reputation, personal leisure time, administration, restrictions and regulations, and work hours and loads. For the ordinal logistic regression analysis, general socio-demographic attributes, such as gender, age, specialty, job position, type of affiliated healthcare organization, working region, and length of service were controlled beforehand. The result of our measures, the affecting factors of job satisfaction for physicians, include being able to; maintain positive relations with patients through adequate time for consultation and necessary healthcare, have the autonomy to make clinical decisions, have healthy relations with peers and staff, obtain respect from family and society, work in an environment with desirable income and have adequate health resources, and have appropriate work hours and loads for those who facilitate high-quality care. Creating an environment in which physicians can focus on patient-oriented healthcare will contribute to promoting national healthcare

    Comparison of Physical, Occupational, and Sociocognitive Characteristics of Corporate and Private Taxi Drivers in Korea

    Taxis are a form of public transport which is very closely related to the safety of the public. Although private and corporate taxis have quite different characteristics, there have only been a few studies comparing the characteristics of corporate and private taxis. Moreover, among various characteristics, research was conducted mainly focusing on occupational characteristics. This study was undertaken to compare various physical, occupational, and sociocognitive characteristics of corporate and private taxi drivers. A descriptive cross-sectional study was conducted from 22 August to 11 September 2018. The subjects of this study were 960 corporate and private taxi drivers over 30 years old in Seoul to compare the means and association between private and corporate taxi driversā€™ characteristics. In terms of the physical characteristics, corporate taxi driversā€™ general physical health status was worse. In terms of the occupational characteristics, corporate taxi drivers had a high working intensity, and the incidence rate of traffic accidents and near misses was also high. This comparison of the characteristics of corporate and private taxis is expected to serve as evidence for developing tailored policies and programs to improve the health of corporate and private taxi drivers

    Fish Diversity and Ichthyofauna of Areas Adjacent to the Demilitarized Zone in South Korea

    The Korean Demilitarized Zone (DMZ) was established in 1953 during the cessation of the Korean War, which divided the Korean Peninsula into North and South Korea. The DMZ is a representative biodiversity resource because it limits human activities. The current status of faunal diversity in adjacent areas of the DMZ was investigated in five regions at 91 sites from 2015 to 2019. A total of 19,562 individuals were collected and identified, including 81 species, 19 families, and 11 orders. Zacco koreanus was the most abundant fish species (relative abundance of 21.9%), while other key species were Zacco platypus (18.2%), Rhynchocypris oxycephalus (5.2%), Rhynchocypris steindachneri (4.9%), and Pungtungia herzi (3.7%). Notably, twenty-nige endemic and seven Red Data Book species were recorded, and three exotic species, Carassius cuvieri, Micropterus salmoides, and Lepomis macrochirus were found at five sites. Human activities and disturbances were found at 34.0% of all sampling sites, and the highest disturbance factors were river dredging and agricultural use (seven sites), sewage (five sites), and fisheries and development (two sites). We recommend that the governments of South and North Korea collaboratively manage the DMZ to protect ecological diversity and maintain its status as a symbol of peace
