    The impact of genetic counselling on risk perception in women with a family history of breast cancer.

    Women with a family history of breast cancer generally self-refer because they have a feeling that their risk is high. However, they have, in general, only a hazy notion of the population risk of breast cancer and their own risk in relation to this. It is probable that they are helped by genetic counselling and, if at substantial risk, by annual mammography. However, the psychological impact of assigning true risk and the value of mammography need to be evaluated. We have assessed risk perception by questionnaire in 517 new referrals to a family history clinic and 200 women returning to the clinic at least 1 year after counselling. Correct assignment of population lifetime risk of breast cancer was 16% in the uninformed precounsel group and 33% in the post-counsel group, likewise personal risk was correct in 11% and 41% respectively. Post-counsel women were significantly more likely to retain information if they were sent a post-clinic letter or if they assessed their personal risk as too high initially

    Evidence for field change in oral cancer based on cytokeratin expression.

    It was hypothesised that one may be able to visualise field changes, which are proposed to exist around tumours, as alterations in keratin intermediate filament protein expression. Standard immunohistochemical analysis using a panel of monoclonal anti-keratin antibodies was applied to fresh tissue sections to look for subtle changes in epithelial differentiation not visible in H&E sections. Such changes were observed in clinically normal epithelium from oral cancer patients, involving primarily substantial expression of keratins K8/K7 (using CAM 5.2) in the basal cells of 12 out of 34 biopsies, and also a trend towards a reduction in the complexity of keratin differentiation. Monitoring such changes may prove to be a valuable adjunct to conventional H&E staining if found to have prognostic and diagnostic significance

    Herschel-SPIRE-Fourier Transform Spectroscopy of the nearby spiral galaxy IC342

    We present observations of the nearby spiral galaxy IC342 with the Herschel Spectral and Photometric Imaging Receiver (SPIRE) Fourier Transform Spectrometer. The spectral range afforded by SPIRE, 196-671 microns, allows us to access a number of 12CO lines from J=4--3 to J=13--12 with the highest J transitions observed for the first time. In addition we present measurements of 13CO, [CI] and [NII]. We use a radiative transfer code coupled with Bayesian likelihood analysis to model and constrain the temperature, density and column density of the gas. We find two 12CO components, one at 35 K and one at 400 K with CO column densities of 6.3x10^{17} cm^{-2} and 0.4x10^{17} cm^{-2} and CO gas masses of 1.26x10^{7} Msolar and 0.15x10^{7} Msolar, for the cold and warm components, respectively. The inclusion of the high-J 12CO line observations, indicate the existence of a much warmer gas component (~400 K) confirming earlier findings from H_{2} rotational line analysis from ISO and Spitzer. The mass of the warm gas is 10% of the cold gas, but it likely dominates the CO luminosity. In addition, we detect strong emission from [NII] 205microns and the {3}P_{1}->{3}P_{0} and {3}P_{2} ->{3}P_{1} [CI] lines at 370 and 608 microns, respectively. The measured 12CO line ratios can be explained by Photon-dominated region (PDR) models although additional heating by e.g. cosmic rays cannot be excluded. The measured [CI] line ratio together with the derived [C] column density of 2.1x10^{17} cm^{-2} and the fact that [CI] is weaker than CO emission in IC342 suggests that [CI] likely arises in a thin layer on the outside of the CO emitting molecular clouds consistent with PDRs playing an important role.Comment: 9 pages, 8 figures, accepted for publication in Monthly Notices of the Royal Astronomical Society (MNRAS

    Effect of Electron Energy Distribution Function on Power Deposition and Plasma Density in an Inductively Coupled Discharge at Very Low Pressures

    A self-consistent 1-D model was developed to study the effect of the electron energy distribution function (EEDF) on power deposition and plasma density profiles in a planar inductively coupled plasma (ICP) in the non-local regime (pressure < 10 mTorr). The model consisted of three modules: (1) an electron energy distribution function (EEDF) module to compute the non-Maxwellian EEDF, (2) a non-local electron kinetics module to predict the non-local electron conductivity, RF current, electric field and power deposition profiles in the non-uniform plasma, and (3) a heavy species transport module to solve for the ion density and velocity profiles as well as the metastable density. Results using the non-Maxwellian EEDF model were compared with predictions using a Maxwellian EEDF, under otherwise identical conditions. The RF electric field, current, and power deposition profiles were different, especially at 1mTorr, for which the electron effective mean free path was larger than the skin depth. The plasma density predicted by the Maxwellian EEDF was up to 93% larger for the conditions examined.     In a randomized controlled trial, the additional provision of information on videotape was no more effective than written information alone in reducing pre-treatment worry about radiotherapy.     A novel compartment, the 'subqpical stem' of the aerial hyphae, is the location of a sigN-dependent, developmentally distinct transcription in Streptomyces coelicolor.

    Streptomyces coelicolor has nine SigB-like RNA polymerase sigma factors, several of them implicated in morphological differentiation and/or responses to different stresses. One of the nine, SigN, is the focus of this article. A constructed sigN null mutant was delayed in development and exhibited a bald phenotype when grown on minimal medium containing glucose as carbon source. One of two distinct sigN promoters, sigNP1, was active only during growth on solid medium, when its activation coincided with aerial hyphae formation. Transcription from sigNP1 was readily detected in several whi mutants (interrupted in morphogenesis of aerial mycelium into spores), but was absent from all bld mutants tested, suggesting that sigNP1 activity was restricted to the aerial hyphae. It also depended on sigN, thus sigN was autoregulated. Mutational and transcription studies revealed no functional significance to the location of sigN next to sigF, encoding another SigB-like sigma factor. We identified another potential SigN target, nepA, encoding a putative small secreted protein. Transcription of nepA originated from a single, aerial hyphae-specific and sigN-dependent promoter. While in vitro run-off transcription using purified SigN on the Bacillus subtilis ctc promoter confirmed that SigN is an RNA polymerase sigma factor, SigN failed to initiate transcription from sigNP1 and from the nepA promoter in vitro. Additional in vivo data indicated that further nepA upstream sequences, which are likely to bind a potential activator, are required for successful transcription. Using a nepA–egfp transcriptional fusion we located nepA transcription to a novel compartment, the ‘subapical stem’ of the aerial hyphae. We suggest that this newly recognized compartment defines an interface between the aerial and vegetative parts of the Streptomyces colony and might also be involved in communication between these two compartments
