Utilidade da determinação consecutiva da proteína C reactiva no follow-up da pneumonia adquirida na comunidade
Resumo: A pneumonia adquirida na comunidade (PAC) é a principal causa de morte devido a doença infecciosa no mundo ocidental, sendo responsável por um número superior a 20 hospitalizações/ano por mil habitantes.As guidelines internacionais preconizam a antibioterapia combinada, como seja a associação de um β-lactâmico e de um macrólido no tratamento inaugural da PAC grave. Tal facto implica a utilização de cuidados de saúde oneroso e o consumo elevado de antibióticos, contribuindo, assim, para o aparecimento de resistências bacterianas.Nos EUA, os custos do tratamento da PAC excedem os doze biliões de dólares e, em muitos paÃses desenvolvidos, observa-se um aumento da resistência aos macrólidos.Uma vez determinada a etiologia da PAC, a antibioterapia direccionada para o agente patogénico em causa pode ser iniciada. No entanto, até agora, não foi encontrada um biomarcador suficientemente sensÃvel e especÃfico para orientar a terapêutica inaugural, devendo ser criados protocolos com essa finalidade.Os autores do presente trabalho estudaram a relevância da determinação consecutiva da proteÃna C reactiva (PCR) no follow-up de doentes com PAC, avaliando o valor preditivo da normalização tardia dos nÃveis séricos da PCR relativamente ao risco de administração de antibioterapia inapropriada e a um prognóstico desfavorável.Trata-se de um estudo prospectivo multicêntrico controlado conduzido em cinco hospitais universitários holandeses, entre Junho de 2000 e Julho de 2003, sobre a eficácia clÃnica da instituição empÃrica de terapêutica antibiótica intravenosa ou oral na PAC.A PAC foi definida pela presença de, pelo menos, dois sintomas de infecção respiratória baixa aguda antes da admissão hospitalar e de um infiltrado pulmonar âde novoâ ou em progressão na radiografia do tórax. A classificação como grave está relacionada com um score no Ãndice de gravidade PSI â Pneumonia severity index >90 ou de acordo com a definição da American Thoracic Society.Foram excluÃdos indivÃduos com pneumonia intersticial, fibrose quÃstica, colonização a gram negativos devido a lesão estrutural do aparelho respiratório, esperança de vida 25c/min; Sat O2 <90% por oximetria de pulso; PaO2 <55mmHg), instabilidade hemodinâmica, alteração súbita do estado de consciência, admissão numa unidade de cuidados intensivos ou morte nos 3 primeiros dias após a admissão. A falência tardia foi decretada pela deterioração clÃnica ou complicações, incluindo a morte, necessidade de ventilação mecânica, reintrodução de antibioterapia IV após instituição de antibióticos orais, readmissão por infecção pulmonar após alta hospitalar ou pirexia após melhoria inicial no perÃodo de follow-up.O atraso na normalização da PCR foi definido por um declÃnio < 60% nos nÃveis da PCR no dia 3 e < 90% no dia 7.Foram englobados 289 doentes com uma idade média de 69,7±13,8 anos. Os scores dos Ãndices PSI e APACHE II foram respectivamente 112,4±25,7 e 13,8±4,6. Outras patologias estavam presentes em 180 indivÃduos: I cardÃaca congestiva, neoplasia, doença cerebrovascular, I renal crónica, doença hepática e doença pulmonar obstrutiva crónica (DPOC). O nÃvel sérico médio da PCR na admissão foi de 174mg/l, tendo sido ligeiramente inferior nos doentes submetidos a antibioterapia em ambulatório (135mg/l) e nos indivÃduos sob corticoterapia inalatória (146mg/l). Não se verificou associação entre os nÃveis da PCR e as caracterÃsticas demográficas ou a presença de comorbilidade. Cerca de 232 (80,3%) doentes receberam um β-lactâmico em monoterapia como terapêutica empÃrica inicial e 47 (16,3%) terapêutica combinada de um β-lactâmico com um macrólido.Em 122 (89,1%) dos 137 doentes com etiologia confirmada, a antibioterapia empÃrica foi considerada adequada. Um total de 20 indivÃduos (6,9%) morreram durante o perÃodo de follow-up e 9 foram transferidos para uma unidade de cuidados intensivos.O diagnóstico etiológico foi estabelecido em 137 (47,4%) doentes, sendo o S. pneumoniae o mais frequente (19% dos casos). Os valores da PCR mais elevados foram observados nos doentes infectados por S. pneumoniae (278mg/l) seguido pela L. pneumophila (247mg/l), H. influenzae (214mg/l), S. aureus (187mg/l), Enterobacteriaciae (129mg/l), C. pneumoniae (115mg/l), M. catarihalis (64mg/l) e M. pneumoniae (49mg/l). Doentes infectados com agentes patogénicos múltiplos tinham um valor médio da PCR, na admissão, de 213mg/l. Os doentes com etiologia desconhecida tinham uma concentração média, na admissão, inferior aos indivÃduos com confirmação bacteriológica (140mg/l versus 209mg/l).A determinação da PCR foi efectuada na admissão em todos os indivÃduos, em 264 (91,3%) no dia 3 e em 210 no dia 7 do internamento. O declÃnio médio da PCR foi de 38,4% até ao dia 3 e de 80,9% na primeira semana de follow-up. Os doentes com tratamento antibiótico inapropriado tinham uma normalização mais lenta dos nÃveis da PCR. O declÃnio <60% no dia 3 e <90% no dia 7 estava associado a um maior risco de terapêutica empÃrica inadequada. Os doentes com atraso na normalização dos nÃveis de PCR na primeira semana apresentavam uma taxa de mortalidade superior, mas tal facto não foi estatisticamente significativo.Doentes com atraso na normalização da PCR no dia 3 tinham um ligeiro aumento do risco de apresentar falência terapêutica precoce ou tardia, mas tal facto não foi estatisticamente significativo
Interferon-gamma release assays (IGRAs) in high-endemic settings: could they play a role in optimizing global TB diagnostics? Evaluating the possibilities of using IGRAs to diagnose active TB in a rural African setting
SummaryThe number of patients suffering from tuberculosis (TB) globally is increasing. Due to the HIV epidemic, most patients suffering from TB reside in sub-Saharan Africa. In order to improve TB diagnostics, new tests – interferon-gamma release assays (IGRAs) – have been developed over the last decade. In this paper we evaluate the possible use of these tests in diagnosing or excluding active TB in high HIV-burden, resource-limited settings. The inability to differentiate between active and latent TB, limited data on IGRA performance in HIV-infected patients, observed false-negative results, high costs, and logistic problems limit the potential benefit of IGRAs. We also present two theoretical study designs in order to further assess IGRAs. Setting up a study on this subject is complicated by the frequent unavailability of mycobacterial cultures, the difficulty in acquiring prospective data, and the impossibility of denying treatment to a patient suspected of having active TB. We feel that current evidence does not support the implementing of IGRAs in clinical practice in settings with high endemic latent TB infection (LTBI) and high HIV prevalence. As these settings are the ones that suffer the most from the TB epidemic, we believe that the role of IGRAs in global TB control is questionable
Long-Term Outcome of an HIV-Treatment Programme in Rural Africa: Viral Suppression despite Early Mortality
Objective. To define the long-term (2–4 years) clinical and virological outcome of an antiretroviral treatment (ART) programme in rural South Africa.
Methods. We performed a retrospective observational cohort study, including 735 patients who initiated ART. Biannual monitoring, including HIV-RNA testing, was performed. Primary endpoint was patient retention; virological suppression (HIV-RNA < 50 copies/mL) and failure (HIV-RNA > 1000 copies/mL) were secondary endpoints. Moreover, possible predictors of treatment failure were analyzed.
Results. 63% of patients (466/735) have a fully suppressed HIV-RNA, a median of three years after treatment initiation. Early mortality was high: 14% died within 3 months after treatment start. 16% of patients experienced virological failure, but only 4% was switched to second-line ART. Male gender and a low performance score were associated with treatment failure; immunological failure was a poor predictor of virological failure.
Conclusions. An “all or nothing” phenomenon was observed in this rural South African ART programme: high early attrition, but good virological control in those remaining in care. Continued efforts are needed to enrol patients earlier. Furthermore, the observed viro-immunological dissociation emphasises the need to make HIV-RNA testing more widely available

We show that observed properties of the low luminosity GRB171205A and its
afterglow, like those of most other low-luminosity (LL) gamma ray bursts (GRBs)
associate with a supernova (SN), indicate that it is an ordinary SN-GRB, which
was produced by inverse Compton scattering of glory light by a highly
relativistic narrowly collimated jet ejected in a supernova explosion and
viewed from a far off-axis angle. As such, VLA/VLBI follow-up radio
observations of a superluminal displacement of its bright radio afterglow from
its parent supernova, will be able to test clearly whether it is an ordinary
SN-GRB viewed from far off-axis or it belongs to a distinct class of GRBs,
which are different from ordinary GRBs, and cannot be explained by standard

Mannose binding lectin plays a crucial role in innate immunity against yeast by enhanced complement activation and enhanced uptake of polymorphonuclear cells
<p>Abstract</p> <p>Background</p> <p>Mannose binding lectin (MBL) is an important host defence protein against opportunistic fungal pathogens. This carbohydrate-binding protein, an opsonin and lectin pathway activator, binds through multiple lectin domains to the repeating sugar arrays displayed on the surface of a wide range of clinically relevant microbial species. We investigated the contribution of MBL to antifungal innate immunity towards <it>C. parapsilosis in vitro</it>.</p> <p>Results</p> <p>High avidity binding was observed between MBL and <it>C. albicans </it>and <it>C. parapsilosis</it>. Addition of MBL to MBL deficient serum increased the deposition of C4 and C3b and enhanced the uptake of <it>C. albicans</it>, <it>C. parapsilosis </it>and acapsular <it>C. neoformans </it>by polymorphonuclear cells (PMNs). Compared to other microorganisms, such as <it>Escherichia coli</it>, <it>Staphylococcus aureus </it>and <it>Cryptococcus neoformans</it>, <it>C. parapsilosis </it>and <it>Candida albicans </it>were potent activators of the lectin pathway.</p> <p>Conclusion</p> <p>Our results suggest that MBL plays a crucial role in the innate immunity against infections caused by yeast by increasing uptake by PMN.</p
Implementation and effect of intensified case finding on diagnosis of tuberculosis in a large urban HIV clinic in Uganda: a retrospective cohort study.
BACKGROUND: Increased detection of tuberculosis (TB) using intensified or active case finding (ICF) is one of the cornerstones of the Stop TB Strategy, and contrasts with passive case finding (PCF) which relies on self-reported symptoms. There is no clear guidance on implementation strategies. We implemented ICF in addition to ongoing PCF in our large urban HIV clinic in July 2010 using a twice-daily announcement screen method by a trained peer educator, asking waiting patients to self-refer to a trained peer supporter for screening of TB symptoms. We sought to determine the associated effect on TB case detection. METHODS: Suspects were investigated by sputum smear, chest X-ray and ultrasound, if indicated. Routinely collected clinical and laboratory data were merged with the ICF register and TB clinic data for patients attending the clinic in 2010. We compared the yield of TB cases (defined as the prevalence of newly diagnosed TB cases in the screened population), the type of TB diagnosed and the total cost per TB case identified (in United States Dollars [USD]) for the period before and after ICF implementation. RESULTS: Of the 20,456 patients who visited the clinic in 2010, 614 were identified as TB suspects, 220 pre-ICF and 394 post-ICF (229 via PCF and 165 via ICF). The proportion diagnosed with TB dropped from 66% to 48% (60% in suspects identified through PCF and 31% through ICF). During the post-ICF period, TB suspects identified through ICF compared to PCF identification were more likely to be female, older, on ART and to have been enrolled in HIV care for a longer duration. The yield of combined PCF and ICF screening was 1.4% pre-ICF and 1.7% post-ICF with a cost per TB case identified of 12.29 USD and 21.80 USD, respectively. CONCLUSIONS: Implementation of ICF in a large HIV clinic yielded more TB suspects and cases, but substantially increased costs and was unable to capture the majority of TB suspects who were referred for diagnosis by clinicians through PCF. The overall yield of TB cases in a mature HIV clinic was low, although targeted screening of those recently enrolled in care may increase the yield
