Après avoir rappellé les effets désastreux pour les patients de l'adoption d'une idéologie de la libération mal comprise et mis en garde contre l'accent mis sur le social et la réadaptation aux dépens des soins aux malades, l'auteur affirme qu'il n'y a pas de traitement possible d'un schizophrène en-dehors d'une institution. S'appuyant sur ses vastes connaissances cliniques, il discute cette affirmation et énonce les conditions qui rendent le traitement institutionnel possible.This article suggests that the solution to treating schizophrenia can only be found in an institution. The author reaches this conclusion after pointing out the disastrous effects on patients of a poorly-understood liberating theory and the misguided emphasis on social skills and readaptation at the expense of patient care. The author uses his vast knowledge acquired in clinical work to discuss his argument and lay out the conditions needed to make institutional treatments possible

La peur du fou

La peur du fou est, sans doute, aussi vieille que le monde. Elle a donné naissance à nos institutions et à nos idéologies. Longtemps on a cru que le fou avait quelque chose de plus que les autres, qu’il était un voyant, un prophète, un possédé. Aujourd’hui, nous avons plutôt tendance à le considérer comme quelqu’un à qui il manque quelque chose, comme la victime d’une déficience de la « fonction symbolique », du « narcissisme », de « l’espace transitionnel », autant de termes savants qui rejoignent une intuition populaire : il lui manque « une vis », « une case ». Mais qu’il soit déficient ou hypertrophié, le fou fait peur. D’où les pratiques d’enfermement et de ségrégation à son égard. J’aimerais m’interroger ici sur les manifestations de cette peur au sein des équipes de psychiatrie communautaire, c’est-à-dire parmi ceux qui essaient de vivre leur rencontre avec le psychotique sans le filet protecteur de la mise à l’écart que constitue toute institution peu ou pro asilaire.The author shows us that, apart from a physical fear that we may experience at times, there exists a more subtle and destructive fear of the insane which determined - and still determines in many circumstances - the attitudes and even the policies which we advocate in regard to the mentally ill. The fear of being overcome, of being consumed by the avidity, the hate, and the projective identification which exist in the psychotic. In the face of this fear we have developed a series of rather inadequate and inefficient defense mechanisms which range from the denial of insanity to institutiona-lization and include all forms of activism and intolerance. In the end, however, this fear must not create in us a state of helplessness nor an oppressive reaction of omnipotence. Perhaps all that is necessary is simply to continue to listen to oneself and others

Categories, words and rules in language acquisition

Acquiring language requires learning a set of words (i.e. the lexicon) and abstract rules that combine them to form sentences (i.e. syntax). In this thesis, we show that infants acquiring their mother tongue rely on different speech categories to extract: words and to abstract regularities. We address this issue with a study that investigates how young infants use consonants and vowels, showing that certain computations are tuned to one or the other of these speech categories..

This article describes the results of an experimental comparative evaluation of four modern pressure mapping Systems (Tekscan, Xsensor, Novel and FSA) using a special loading device.The tests generated dato on numerical accuracy, linearity and hysteresis. The results show that the accuracy of pressure mapping Systems is still limited and results obtained from different Systems can not be directly compared

Zusätzlich sollen sie helfen, Druckgeschwüre durch mechanische Entlastung und einen Abtransport von Feuchtigkeit und Wärme zu vermeiden. Die Auswahl eines Sitzkissen erfolgt dabei häufig anhand einer subjektiven Einschätzung der Sitzkisseneigenschaften, was die durchschnittliche Qualität der Versorgung negativ beeinflussen kann und unter Kostengesichtspunkten die Auswahl hochwirksamer Kissen nur schwer rechtfertigt. Dieser Artikel beschreibt ein Verfahren, Sitzkissen anhand von messtechnisch erfassten Kennwerten und einer standardisierten Patientenprofilerhebung auszuwählen. Dazu wird eine Übersicht über die am Markt erhältlichen Sitzkissen gegeben und Möglichkeiten erörtert, die Sitzkissen anhand von strukturierten Entscheidungshilfen nachvollziehbar auszuwählen und die Anforderungen des Patienten unterstützend einzuteilen und zuzuordnen.A wheelchair seat cushion represents the interface between the wheelchair and the wheelchair driver and has to manage the force transmission and the stabilization between both of them. An additional very important requirement is the prevention of pressure ulcers by pressure relief and the transfer of humidity and heat away from the patient. The allocation of a cushion to the patient happens normally on the base of a subjective description of the cushion without any transparent decision rules. Consequently the quality of this procedure could be improved by a better description of the cushion, based on standardized laboratory tests and by the development of decision tools helping to identify the main efforts of the user. This article describes a procedure that allows the allocation of wheelchair seat cushions based on the characteristic of cushions from standardized laboratory tests and on the brief description of the user. Additionally the existing principles for wheelchair cushions are listed, described and compared. The possibilities to develop structured decision tools for wheelchair cushions are discussed
