    Primary Follicular Lymphoma of the Duodenum with Erosions as Atypical Macroscopic Features

    A 52-year-old Japanese woman who was eventually diagnosed with primary follicular lymphoma of the duodenum showed atypical endoscopic features, namely, erosions with peripheral whitish edematous mucosa. Initial biopsy specimens taken from the erosions revealed insufficient numbers of lymphoma cells for histological diagnosis. Subsequent biopsy specimens from the peripheral mucosa containing the whitish enlarged villi showed infiltration of the lymphoma cells forming lymphoid follicles, which led us to the appropriate diagnosis. This case indicates that endoscopists should take biopsy samples from the peripheral mucosa with whitish enlarged villi rather than erosions in the rare instances that erosions appear as the main macroscopic feature of intestinal follicular lymphoma

    Partial Purification and Chraracterization of Dendritic Cell Differentiation Factor

    Previously, we reported that interleukin-2 (IL-2)-stimulated helper T cells produced an unknown soluble factor which induced dendritic cell-like differentiation in primary cultures of monocytic leukemia cells and we referred to this factor as dendritic cell differentiation factor (DCDF). In this study, we attempted to purify and characterize DCDF and investigated its biological effect on normal human monocytes. Gel filtration chromatography indicated that the molecular weight of DCDF is approximately 30-35 kDa. Chromatofocusing indicated that the isoelectric point of DCDF is approximately 5.0. DCDF, partially purified by subsequent gel filtration, chromatofocusing, and hydrophobic chromatography, significantly enhanced the HLA-DR expression of normal human monocytes and a human monocytic leukemia cell line, THP-1. This biological activity was not neutralized by any known antibodies to human cytokines. DCDF significantly amplified the T-cell stimulatory activity of monocytes in the allogeneic mixed leukocyte reaction (MLR). Moreover, DCDF significantly enhanced IL-1 beta and IL-6 production by monocytes in a dose-dependent manner. These results suggest that DCDF is a novel human cytokine which stimulates the accessory cell function of monocytes.</p

    質の高いデザイン活動による学生教育と周囲への波及効果に関する実践研究

     「DESIGN SOIL」のプロジェクトの教育的な効果を明らかにすることを目的とする。また、合わせてワークショップなども開催し、実習授業などに還元できるデザイン教育の有効な方法を発見することも目指す。 2016年4月のミラノでの展示会出展を通じて、複数の作品の商品化が決定するという成果が出た。 同年11月に神戸で展示会を開催したのに合わせて、DESIGN SOILのメンバーや卒業した元メンバーが話し手として自身の体験について語るトークイベントを開催した。参加した学生の声から、学生が直接現地の空気に触れ、自身の言葉で周りの学生に伝えることが最もリアリティのあることとして強く響くということが確認できた。 博物館見学をデザインと直結させるワークショップを通して、デザイン実習などにも応用できる手法を発見できた。 メンバーへのヒアリングから、DESIGN SOILの活動、海外での展示会出展が、参加学生や周囲の学生にも強い刺激を与えることができており、良い影響が循環してきていることが確認できた。It aims to clarify the educational effect of "DESIGN SOIL" project. In addition, we also hold workshops and aim to discover effective ways of design education that can be returned to practical lessons. Through the exhibition in Milan in April, several works were selected as candidates for commercialization. In conjunction with holding an exhibition in Kobe in November, members of DESIGN SOIL and former members who graduated held a talk event talking about their own experiences. It was confirmed that the words by students who exhibited in the Milan is effective as the most realistic thing for the surrounding students. Through a workshop linking the museum tour with design practice, I was able to find a method applicable to design practice etc. From the hearing to the members, the activities of DESIGN SOIL and the participation of the exhibition abroad were able to give strong stimulus to participating students and surrounding students, and it was confirmed that good influence is circulating

    日本のデザイン教育は、学生や学生の作品を市場に結びつける段階まではカバーできておらず、デザインインキュベーションという視点においてヨーロッパのデザイン先進国から大きく遅れを取っている状況にある。本研究では、デザインインキュベーションセンター立ち上げを目指して、先例を実践している大学の調査を行う。また、作品を商品として完成させるプロセスを実践し検証することを通して、教育プログラムとしての可能性も検討する。4 月のミラノでの展示会出展、6 月の神戸での展示会開催などを経て、継続的に展示会出展を含む活動を行なっていることの成果を確認できた。デザイン先進国では、国柄や文化に合った形で大学や学生が市場と繋がる取り組みを積極的に行っている。調査を通じて、日本でも独自のデザイン・インキュベーションを推進していく上で参考となる事例を確認できた。In Japanese design education, educational institutions cannot cover the stage of linking student and student work tothe market. From the viewpoint of design incubation,Japan is significantly behind the design advanced countriesof Europe.In this study, in order to set up the Design IncubationCenter, we will conduct a survey of universities thatpractice precedents.And we also consider possibilities as aneducational program through practicing and verifying theprocess of completing a work as a product.After having exhibited in Milan in April 2017 and holdingan exhibition in Kobe in June, We confirmed the result ofcontinuing activities including participation in theexhibition.In advanced countries in the design, universities andstudents are actively engaged in activities linked to themarket in a way that suits nationalities and cultures.Through the survey, we have confirmed cases that serve asreference for promoting unique design incubation in Japan

    One of the causative diseases of intestinal obstruction in young women is bowel endometriosis. During the course of ectopic endometriosis, it is estimated that about 10% of patients develop bowel endometriosis. The first step in treatment is drug therapy. In cases of bowel endometriosis of the colon or rectum leading to intestinal obstruction, laparotomy is often required. A 47-year-old woman with a history of endometriosis was undergoing drug therapy. She developed abdominal pain and nausea, and was diagnosed with septic shock and fecal ileus. A transanal drainage tube was inserted for decompression. The patient’s general condition improved, and a laparoscopic low anterior resection was performed on the 23rd day. The patient was discharged on the 10th postoperative day without any postoperative problems. This case suggests that even in the case of septic shock caused by rectal stricture due to intestinal endometriosis, initial treatment with transanal decompression may stabilize the general condition, and may be superior in cosmetic change

    Gastric variceal bleeding caused by an intrahepatic arterioportal fistula that formed after liver biopsy: a case report and review of the literature

    An intrahepatic arterioportal fistula is a rare cause of portal hypertension and variceal bleeding. We report on a patient with an intrahepatic arterioportal fistula following liver biopsy who was successfully treated by hepatectomy after unsuccessful arterial embolization. We also review the literature on symptomatic intrahepatic arterioportal fistulas after liver biopsy. A 48-year-old male with bleeding gastric varices and hepatitis B virus-associated liver cirrhosis was transferred to our hospital; this patient previously underwent percutaneous liver biopsies 3 and 6 years ago. Abdominal examination revealed a bruit over the liver, tenderness in the right upper quadrant, and splenomegaly. Ultrasonographic examination, computed tomography, and angiography confirmed an arterioportal fistula between the right hepatic artery and the right portal vein with portal hypertension. After admission, the patient suffered a large hematemesis and developed shock. He was treated with emergency transarterial embolization using microcoils. Since some collateral vessels bypassed the obstructive coils and still fed the fistulous area, embolization was performed again. Despite the second embolization, the collateral vessels could not be completely controlled. Radical treatment involving resection of his right hepatic lobe was performed. For nearly 6 years postoperatively, this patient has had no further episodes of variceal bleeding

    Endoscopic Needle Knife Precut Papillotomy for Inaccessible Bile Duct following Failed Pancreatic Duct Access

    Aims To evaluate the safety, effectiveness, success rate and complications of needle knife precut papillotomy for inaccessible bile duct after failed pancreatic ducts access. Methods Selective common bile duct cannulation was required for 582 patients from November 2004 to May 2008. Precut sphincterotomy was performed in 28 patients (16 male, 12 female; mean age 71). When standard bile duct cannulation was unsuccessful after more than 20 minutes, the bile duct was considered inaccessible. Group A consisted of patients where pancreatic duct access was possible and transpancreatic papillary septotomy was performed (20 patients). If pancreatic duct cannulation also failed, needle knife precut method was performed and these patients belonged to group B (8 patients). The success and complication rates of the two groups were compared using the Chi-square test. Results The success rates were 85% and 87.5% respectively. Of the 28 patients in group A, 6 had mild to moderate pancreatitis and one patient had mild bleeding. The complication rates were 35% and 0% respectively and the differences were not significant. Conclusion Needle knife precut papillotomy is useful and acceptable in patients after failed pancreatic duct access
