    Successful Removal of Large Intraocular Foreign Body by 25-Gauge Microincision Vitrectomy Surgery

    We describe a new technique for removing a large intraocular foreign body by 25-gauge microincision vitrectomy surgery (25G-MIVS). Noncomparative interventional case series were performed at a single centre. Two patients with a long smooth intraocular vitreal foreign body underwent phacoemulsification and aspiration, intraocular lens implantation, 25G-MIVS, and extraction of the foreign body. The foreign body was removed through a posterior capsulorhexis, anterior continuous curvilinear capsulorhexis, and a corneal incision. In both cases, the foreign body was safely removed through the corneal incision, and IOL was implanted and well positioned. The surgical incision did not require suturing. No postoperative complications associated with this technique were found. The corneal endothelial cell density was maintained over 2000 cells/mm2 in both cases during recent follow-up examinations. Our findings indicate that 25G-MIVS with this technique can be used to extract a long slender smooth foreign body. It is safe, without complications, and can be performed without enlarging the 25-gauge sclerotomy

    Essential role of caspase-8 in p53/p73-dependent apoptosis induced by etoposide in head and neck carcinoma cells

    <p>Abstract</p> <p>Background</p> <p>Caspase-8 is a key upstream mediator in death receptor-mediated apoptosis and also participates in mitochondria-mediated apoptosis via cleavage of proapoptotic Bid. However, the role of caspase-8 in p53- and p73-dependent apoptosis induced by genotoxic drugs remains unclear. We recently reported that the reconstitution of procaspase-8 is sufficient for sensitizing cisplatin- but not etoposide-induced apoptosis, in chemoresistant and caspase-8 deficient HOC313 head and neck squamous cell carcinoma (HNSCC) cells.</p> <p>Results</p> <p>We show that p53/p73-dependent caspase-8 activation is required for sensitizing etoposide-induced apoptosis by utilizing HOC313 cells carrying a temperature-sensitive p53G285K mutant. Restoration of wild-type p53 function under the permissive conditions, together with etoposide treatment, led to substantial transcriptional activation of proapoptotic Noxa and PUMA, but failed to induce apoptosis. In addition to p53 restoration, caspase-8 reconstitution was needed for sensitization to etoposide-induced apoptosis, mitochondria depolarization, and cleavage of the procaspases-3, and -9. In etoposide-sensitive Ca9-22 cells carrying a temperature-insensitive mutant p53, siRNA-based p73 knockdown blocked etoposide-induced apoptosis and procaspase-8 cleavage. However, induction of p73 protein and up-regulation of Noxa and PUMA, although observed in Ca9-22 cells, were hardly detected in etoposide-treated HOC313 cells under non-permissive conditions, suggesting a contribution of p73 reduction to etoposide resistance in HOC313 cells. Finally, the caspase-9 inhibitor Ac-LEHD-CHO or caspase-9 siRNA blocked etoposide-induced caspase-8 activation, Bid cleavage, and apoptosis in both cell lines, indicating that p53/p73-dependent caspase-8 activation lies downstream of mitochondria.</p> <p>Conclusions</p> <p>we conclude that p53 and p73 can act as upstream regulators of caspase-8, and that caspase-8 is an essential mediator of the p53/p73-dependent apoptosis induced by etoposide in HNSCC cells. Our data suggest the importance of caspase-8-mediated positive feedback amplification in the p53/p73-dependent apoptosis induced by etoposide in HNSCC cells.</p

    Superconducting zinc heat switch for continuous nuclear demagnetization refrigerator and sub-mK experiments

    We have developed and tested a zinc superconducting heat switch suitable for magnetic refrigeration and calorimetric experiments at sub-millikevin (sub-mK) temperatures. The specific application here is an adiabatic demagnetization refrigerator with two PrNi5_{5} nuclear stages, which can keep a temperature of 0.8 mK continuously, (CNDR) proposed by Toda et al. (J. Phys.: Conf. Ser. 969, 012093 (2018). The switch consists of six high-purity zinc foils of 0.25 mm thick which contact seven silver foils by diffusive bonding. The silver foils are electron beam welded to silver rods that are thermal links to other components. The choice of the thin zinc foils is due to reduce the magnetic latent heat on switching and the contact thermal resistance under a constraint on the aspect ratio of the switch element. The measured thermal conductance of the whole switch assembly in the normal (closed) state, KclosedK_\mathrm{closed}, agrees very well down to 70 mK with the value estimated from the residual electrical resistance 114 nΩ\mathrm{\Omega} at 4.2 K, indicating the validity of the Wiedemann-Franz law for zinc. The measured thermal conductance in the superconducting (open) state, KopenK_\mathrm{open}, follows nicely the prediction from the BCS theory and approaches the value expected from the Debye model for thermal phonons near 70 mK. The heat leak through the HSW from the higher temperature side of 30 mK at most is estimated to be less than 0.5 nW, which is much lower than the expected cooling power (=10= 10 nW) of the CNDR at 0.8 mK . The switching ratio Kclosed/KopenK_\mathrm{closed}/K_\mathrm{open} extrapolated to 30 mK, is as high as 5×104\times10^4. All the test results meet the requirements for the CNDR and, for example, heat capacity measurements at sub-mK.Comment: 15 pages, 5 figure

    Spectrocolorimetric assessment of cartilage plugs after autologous osteochondral grafting: correlations between color indices and histological findings in a rabbit model

    We investigated the use of a commercial spectrocolorimeter and the application of two color models (L* a* b* colorimetric system and spectral reflectance distribution) to describe and quantify cartilage plugs in a rabbit model of osteochondral autografting. Osteochondral plugs were removed and then replaced in their original positions in Japanese white rabbits. The rabbits were sacrificed at 4 or 12 weeks after the operation and cartilage samples were assessed using a spectrocolorimeter. The samples were retrospectively divided into two groups on the basis of the histological findings (group H: hyaline cartilage, successful; group F: fibrous tissue or fibrocartilage, failure) and investigated for possible significant differences in the spectrocolorimetric analyses between the two groups. Moreover, the relationships between the spectrocolorimetric indices and the Mankin histological score were examined. In the L* a* b* colorimetric system, the L* values were significantly lower in group H than in group F (P = 0.02), whereas the a* values were significantly higher in group H than in group F (P = 0.006). Regarding the spectral reflectance distribution, the spectral reflectance percentage 470 (SRP470) values, as a coincidence index for the spectral reflectance distribution (400 to 470 nm in wavelength) of the cartilage plugs with respect to intact cartilage, were 99.8 ± 6.7% in group H and 119.8 ± 10.6% in group F, and the difference between these values was significant (P = 0.005). Furthermore, the a* values were significantly correlated with the histological score (P = 0.004, r = -0.76). The SRP470 values were also significantly correlated with the histological score (P = 0.01, r = 0.67). Our findings demonstrate the ability of spectrocolorimetric measurements to predict the histological findings of cartilage plugs after autologous osteochondral grafting. In particular, the a* values and SRP470 values can be used to judge the surface condition of an osteochondral plug on the basis of objective data. Therefore, spectrocolorimetry may contribute to orthopedics, rheumatology and related research in arthritis, and arthroscopic use of this method may potentially be preferable for in vivo assessment

    Therapeutic Effects of a Sodium Glucose Cotransporter 2 Inhibitor in Diabetic Patients with Chronic Kidney Disease

    Multiple large-scale clinical trials have indicated that sodium glucose cotransporter 2(SGLT2)inhibitors reduce the incidence of cardiovascular events, deterioration of renal function and mortality. However, the therapeutic effects of SGLT2 inhibitors are supposed to be limited in patients with reduced renal function considering the mechanism of their action. In this study, a SGLT2 inhibitor, ipragliflozin was given to 30 type 2 diabetic patients with nephropathy whose estimated glomerular filtration rate(eGFR)was not lower than 30 mL/min/1.73 m2. After 12 to 16 weeks, hemoglobin A1c decreased by 0.6%(p<0.001), body weight was reduced by 1.8 kg(p<0.01)and blood pressure was lowered by -10/-6 mmHg(p<0.001/p <0.001). This was accompanied by reductions in serum uric acid(-0.7 mg/dL, p<0.001), triglycerides (-25 mg/dL, p=0.028)and g-glutamyl transferase(-8 U/L, p=0.001). On the other hand, plasma B-type natriuretic peptide also decreased by 12%(p=0.020)and urinary albumin excretion was reduced by 23% (p=0.018)although the eGFR was not significantly changed. It is concluded that ipragliflozin is effective in lowering blood glucose even in patients with diabetic kidney disease and is beneficial in improving theaccompanying obesity and hypertension. In addition, ipragliflozin is thought to have favorable influences on the metabolisms of uric acid and lipids. These properties of ipragliflozin is expected to bring about protective effects against the progression of nephropathy and the development of cardiovascular disease resulting in the improvement of prognosis in diabetic patients with mild to moderate chronic kidney disease

    Tegafur-Uracil Plus Gemcitabine Combination Chemotherapy in Patients with Advanced Non-small Cell Lung Cancer Previously Treated with Platinum

    BackgroundAn open-label, single-arm prospective study was conducted to evaluate the efficacy and toxicity of the combination of gemcitabine and tegafur-uracil (UFT) in patients with advanced nonsmall-cell lung cancer (NSCLC) after the failure of previous platinum-containing regimens.Patients and MethodsPatients with advanced NSCLC received 200 mg/m2 of UFT twice daily from day 1 through 14 plus 900 mg/m2 of gemcitabine per day via intravenous injection on days 8 and 15. This regimen was repeated every 3 or 4 weeks.ResultsA total of 40 patients were enrolled. Eleven patients (28%; 95% confidence interval [CI], 15–44%) achieved a partial response. The median progression-free survival, median overall survival, and 1-year survival rate were 4.0 months (95% CI, 3.3–6.7 months), 12.6 months (95% CI, 7.0–22.3 months), and 51% (95% CI, 33–66%), respectively. The most common grade 3 or 4 toxicity was neutropenia (38%; 95% CI, 23–54%) and the rate of grade 3 or 4 nonhematologic toxicity remained at less than 5%. A multivariate Cox model showed that adenocarcinoma, nonsmoking history, and good performance status predicted better survival.ConclusionsCombination chemotherapy with UFT and gemcitabine showed a promising effectiveness and acceptable toxicity for patients with platinum-resistant NSCLC

    This study aims to investigate which and what motions of lifelike conversational agents play essential role to make the agents natural. Some preliminary experimental results and future plan are shown. Embodying conversational agents is intended to imitate paralinguistic channel or back channel which plays important role in human-human conversation. Although there have been not a few lifelike conversational agents developed, appearance and behaviour of the agents are far from being natural, and merits of embodying have not been realized that much. One of the main reasons is the lack of natural motions of the agents, including the motion of head, mouth, eyes and eye brows. To improve the quality of these motions, several pioneering works have been reported [1–3].     Low-Intensity Resistance Training with Moderate Blood Flow Restriction Appears Safe and Increases Skeletal Muscle Strength and Size in Cardiovascular Surgery Patients:A Pilot Study

    We examined the safety and the effects of low-intensity resistance training (RT) with moderate blood flow restriction (KAATSU RT) on muscle strength and size in patients early after cardiac surgery. Cardiac patients (age 69.6 +/- 12.6 years, n = 21, M = 18) were randomly assigned to the control (n = 10) and the KAATSU RT group (n = 11). All patients had received a standard aerobic cardiac rehabilitation program. The KAATSU RT group additionally executed low-intensity leg extension and leg press exercises with moderate blood flow restriction twice a week for 3 months. RT-intensity and volume were increased gradually. We evaluated the anterior mid-thigh thickness (MTH), skeletal muscle mass index (SMI), handgrip strength, knee extensor strength, and walking speed at baseline, 5-7 days after cardiac surgery, and after 3 months. A physician monitored the electrocardiogram, rate of perceived exertion, and the color of the lower limbs during KAATSU RT. Creatine phosphokinase (CPK) and D-dimer were measured at baseline and after 3 months. There were no side effects during KAATSU RT. CPK and D-dimer were normal after 3 months. MTH, SMI, walking speed, and knee extensor strength increased after 3 months with KAATSU RT compared with baseline. Relatively low vs. high physical functioning patients tended to increase physical function more after 3 months with KAATSU RT. Low-intensity KAATSU RT as an adjuvant to standard cardiac rehabilitation can safely increase skeletal muscle strength and size in cardiovascular surgery patients.</p

    Spread and yield loss mechanisms of rice stripe disease in rice paddies

    Rice stripe disease is an economically important disease of rice caused by the Rice stripe virus (RSV), which is transferred by the small brown planthopper (SBPH). The recent rapid increase in damage to rice crops throughout Japan caused by this disease makes it imperative to develop control methods as soon as possible. To obtain basic data for developing such methods, we studied how the disease causes damage and spreads within paddy fields. Our investigations revealed that diseased plants first appear in mid-June to early July, after which the disease spreads from affected plants to adjacent plants. This suggests that SBPH carrying RSV enter paddy fields, where they infect plants as they move about and lay eggs. Subsequently, hatched viruliferous nymphs infect surrounding plants, thereby spreading the disease. Our analysis of the damage caused by rice stripe disease showed that the earlier the onset of disease, the more extensive the damage caused, and that the disease reduces yield by reducing the number of healthy panicles. This suggests that to reduce damage caused by this disease, it is necessary to ensure the growth of a sufficient number of healthy panicles by controlling the vector insect during the crop’s early growth period. To be most effective, pest control efforts should be timed to target either the first-generation adults that colonize the paddy fields or the second-generation nymphs and adults that cause the rapid increase in the number of diseased plants within a field.Figures(.eps)はPDFに変換して本文PDFに組み入れた
