    Gastric Diseases by H. Pylori and the Role of Mast Cells

    H. Pylori deemed as one of the causes of gastric pathologies (mainly chronic superficial gastritis, chronic atrophic gastritis, gastroduodenal ulcer, etc.) which can progress to gastric atrophy and cancer of the stomach. Although still not well-known pathogenesis and Physiopathology of damage by H. Pylori has recently attracted attention histological framework of these diseases. In particular, mast cells are found frequently and in a number of pathologies, such increased at a significant correlation what makes think that mast cells have a manifest impact in the pathogenesis of these pathologies. We wanted to verify whether any correlations exist between HP positivity, type of gastritis and presence of mast cells. We examined 90 bioptic fragments of gastric mucosa obtained by diagnostic esophagogastroduodenoscopy. The study population consisted of 22 patients with chronic superficial gastritis and 8 with chronic atrophic gastritis (20 males and 10 females). HP detection was performed on Giemsa-stained preparations. Mast cell detection was performed on Giemsa- and PAS-stained preparations. Sixty-eight percent of chronic superficial gastritis cases were found to be HP-positive and mast cell was present in 87% of these and absent in 13%. In chronic atrophic gastritis cases, 50% were HP-positive and mast cell was present in 50% and absent in the rest. On the basis of our data, a significant correlation was found (86%) between HP-positivity and presence of mast cells in the inflammatory gastric mucosa.Keywords: Helicobacter pylori, mast cell, gastritis

    Healthcare Workforce Motivation as Crucial Element of Healthcare Reform Success

    Health care reforms are initiated in order to improve the quality of care aiming main objective, healthy population. Regardless goals of healthcare reforms, main actors implementing all activities, are the most important element of human resources, professional workers. Human resources are the most important asset of the healthcare system and their motivation has key importance for sectoral reforms. Human resource motivation is the process that assures utilization of employeeā€™s, hence either healthcare institution or an employee get most high profit. Primary health care reform started in the year 2007 had financial, organizational and juridical dimensions, which couldnā€™t succeed without the motivation of healthcare personnel in the process. Health Insurance Institute (HII) was found in 1995 as a single purchase in Albania. Active purchasing strategies can drive better quality of care by financial rewarding best practices and improve outcome, and create incentives for providers to be more efficient or more responsive to consumers. This was the key mechanism used by HII in contracting Health Centers (HC) since the beginning of health care reform in primary health care. New financing formula: 85% of fix budget for normal running of HC, 10% of the budget according to the performance of family doctors and 5% of budget based on process indicators in order to measure results of health care in the catchment area of HC. According to Herzbergā€™s hygiene factors, if you address dissatisfying job factors like work conditions, salary, status, and security you may create peace, but not necessarily enhance performance. HC have the right to distribute financial remuneration twice per year to health staff based on financial results, in order to motivate them to provide better quality service. On the other hand sign of working contract between HC on one side and healthcare employ on the other side, tended to make employees feel more secure in working place. It is known that the first quarter of this period is sensitive because it is precisely this period in which take originated all the structures of the developing organism. Fall into contact with substances assigned in this period, resulting in serious consequences in the future of the individual to be born. In some cases turns out that the substances are considered as medications may have teratogenic effects.
