    Low Body Mass Index as a Risk Factor for the Onset of Porosity of the Mandibular Bone in the Elderly

    Objective: To investigate whether a low body mass index (BMI) has a significant relationship with mandibular bone porosity progression by conducting a mandibular inferior cortex (MIC) classification in elderly Japanese people. Material and Methods: A total of 266 study subjects, aged 70 at baseline, were recruited for this study, conducted from 1998 to 2007. The subjects were divided into two groups according to changes in the MIC on serial panoramic radiographs during this nine-year study period: a no change group (MIC-NC) and a change group (MIC-C). All subjects in the MIC-C showed changes trending toward greater fragility. We evaluated the BMI at baseline. Logistic regression analysis was performed to assess the relationship between the MIC condition during the nine-year period (0: MIC-NC, 1: MIC-C) and BMI (kg/m2) adjusted for gender, current health status (CHS), and smoking habit (SH) at baseline. Results: The mean and standard deviations of the BMI at baseline in the MIC-NC and MIC-C were 22.8 ± 2.1 and 21.8 ± 2.5 kg/m2 for males and 23.1 ± 2.9 and 21.9 ± 2.4 kg/m2 for females. There was a significant relationship between the MIC condition and the BMI in both males (p=0.04) and females (p=0.01). The logistic regression analysis revealed a significant association between the MIC condition over the nine-year period and the BMI (OR=0.84, p=0.003), which was adjusted depending on the gender (OR=5.18, p=0.000), CHS (OR=0.53, p=0.015), and SH (OR=4.15, p=0.002) at baseline. Conclusion: A low BMI carries a risk of developing mandibular bone porosity by measuring the MIC condition in panoramic radiographs

    Whole body counter surveys of Miharu-town school children for four consecutive years after the Fukushima NPP accident

    Comprehensive whole-body counter surveys of Miharu town school children have been conducted for four consecutive years, in 2011-2014. This represents the only long-term sampling-bias-free study of its type conducted after the Fukushima Dai-ichi accident. For the first time in 2014, a new device called the Babyscan, which has a low 134/137^{134/137}Cs MDA of <50< 50 Bq/body, was used to screen the children shorter than 130 cm. No child in this group was found to have detectable level of radiocesium. Using the MDAs, upper limits of daily intake of radiocesium were estimated for each child. For those screened with the Babyscan, the upper intake limits were found to be <1 Bq/day for 137^{137}Cs. Analysis of a questionnaire filled out by the children's parents regarding their food and water consumption shows that the majority of Miharu children regularly consume local and/or home-grown rice and vegetables.     Stepwise development of Hematopoietic stem Cells from Embryonic Stem Cells

    The cellular ontogeny of hematopoietic stem cells (HSCs) remains poorly understood because their isolation from and their identification in early developing small embryos are difficult. We attempted to dissect early developmental stages of HSCs using an in vitro mouse embryonic stem cell (ESC) differentiation system combined with inducible HOXB4 expression. Here we report the identification of pre-HSCs and an embryonic type of HSCs (embryonic HSCs) as intermediate cells between ESCs and HSCs. Both pre-HSCs and embryonic HSCs were isolated by their c-Kit(+)CD41(+)CD45(−) phenotype. Pre-HSCs did not engraft in irradiated adult mice. After co-culture with OP9 stromal cells and conditional expression of HOXB4, pre-HSCs gave rise to embryonic HSCs capable of engraftment and long-term reconstitution in irradiated adult mice. Blast colony assays revealed that most hemangioblast activity was detected apart from the pre-HSC population, implying the early divergence of pre-HSCs from hemangioblasts. Gene expression profiling suggests that a particular set of transcripts closely associated with adult HSCs is involved in the transition of pre-HSC to embryonic HSCs. We propose an HSC developmental model in which pre-HSCs and embryonic HSCs sequentially give rise to adult types of HSCs in a stepwise manner

    Background: The demographics of patients with congenital heart disease (CHD) and atrial fibrillation (AF) differ significantly from the general population. The etiology and treatment strategy for AF in CHD patients have been investigated but are to date inconclusive. Methods: To determine the etiology of AF in CHD and to seek a better treatment strategy, we retrospectively evaluated the atrial overload in 42 complex CHD cases with normal atrial arrangements and AF (age 25; range, 9-66 years) and the impact of a reduction in the atrial overload on the atrial rhythm. Results: Cardiac defect diagnoses varied, with 17% of the patients having a persistent left superior vena cava (PLSVC). In regard to the volume overload, the frequencies of an overload in the right atrium (RA), left atrium (LA), or both, were 50 %, 23%, and 10%, respectively (p = 0.015). Other sustained supraventricular tachycardias were observed in 29 patients (69%) before and after the onset of AF. Among these 29 patients, 26 had intra-atrial reentrant tachycardia. Fifteen patients (36%), 10 of whom had chronic AF, died during the follow-up including 3 with arrhythmias and 10 because of heart failure. Fourteen (33%) patients had no AF at the last follow-up due to medical interventions, 8 of which underwent solely an RA-sided catheter ablation and/or surgical RA overload reduction. Conclusions: AF in complex CHD with a normal atrial arrangement correlates with a higher RA-sided overload than an LA-sided and exhibits a high incidence of PLSVCs, high comorbidity of intra-atrial reentrant tachycardias, and high mortality rate. In a substantial number of patients, RA-sided interventions were effective in controlling AF.     Oral health status: relationship to nutrient and food intake among 80‐year‐old Japanese adults

    Objectives The aim of this cross‐sectional study was to investigate the relationship of oral health status defined on the basis of presence of posterior occluding pairs ( POP s) and adequacy of removable denture fit as determined by self‐report to nutrient and food intake among older Japanese. Methods The subjects were 353 Japanese aged 80 years in 2008 and were classified into four groups according to the number of POP s, defined as pairs of occluding natural, restored, or fixed prosthetic postcanine teeth (range: 0–8) and removable denture status. The groups were: (i) good dentition ( n  =   56; 8 POP s and no removable prosthesis), (ii) well‐fitting dentures ( n  =   158; <8 POP s with self‐reported good‐fitting dentures), (iii) ill‐fitting dentures ( n  =   70; <8 POP s with self‐reported ill‐fitting dentures), and (iv) compromised dentition ( n  =   69; <8 POP s and no removable prosthesis). Multivariable analysis of the differences in nutrient and food intake outcome variables which were collected via validated food frequency questionnaire among the four oral health status groups was conducted using general linear models. Results Intake of multiple nutrients was significantly ( P  <   0.05) lower in the group with ill‐fitting dentures or compromised dentition than in the good dentition group. Vegetable, fish, and shellfish consumption was significantly lower in the ill‐fitting dentures or compromised dentition groups. No significant differences were seen in dietary intake between the well‐fitting dentures and good dentition groups. Conclusions Dietary intake was poorer in those with self‐perceived ill‐fitting dentures or fewer POP s than among those having all POP s. Regular dental care to maintain intact dentition, as well as dental treatment to replace missing teeth and ensure adequate denture fit and function, may be important to the diet intake and subsequent nutritional status of older Japanese.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/108691/1/cdoe12100.pd

    Effect of lifestyle on 6‐year periodontitis incidence or progression and tooth loss in older adults

    AimTo evaluate the longitudinal association of combined healthy lifestyle factors with incidence or progression of periodontitis and tooth loss in older adults.Materials and methodsThis 6‐year study included 374 Japanese 70‐year olds with 7,157 teeth, from a source eligible baseline population of 554 individuals. Four lifestyle factors—cigarette smoking, physical activity, relative weight, and dietary quality—were scored as healthy (1 point) or unhealthy (0 point). Adding the individual scores generated the “healthy lifestyle score” (0–4 points). Multilevel mixed‐effects logistic regression models were applied to evaluate tooth‐specific associations between the baseline healthy lifestyle score and the incidence or progression of periodontitis (increase in clinical attachment loss ≥3 mm) and tooth loss.ResultsAfter 6 years, 19.0% of the teeth exhibited periodontitis incidence or progression and 8.2% were lost. Compared with a healthy lifestyle score of 0–1 (least healthy), the highest score (4 points) was associated with a significantly lower tooth‐specific risk of periodontitis (adjusted odds ratio = 0.32; 95% confidence interval: 0.16–0.62) and tooth loss (adjusted odds ratio = 0.42; 95% confidence interval: 0.23–0.77).ConclusionsSimultaneous adherence to multiple healthy lifestyle factors significantly lowers the risk of incidence or progression of periodontitis and tooth loss in older adults.Peer Reviewedhttps://deepblue.lib.umich.edu/bitstream/2027.42/145573/1/jcpe12920_am.pdfhttps://deepblue.lib.umich.edu/bitstream/2027.42/145573/2/jcpe12920.pd

    Long-term prognostic value of cardiac autonomic nervous activity in postoperative patients with congenital heart disease

    Background: Abnormal cardiac autonomic nervous activity (CANA) is not uncommon in postoperative patients with congenital heart disease (CHD). Methods and results: We attempted to clarify the prognostic value of the CANA variables in postoperative CHD patients and prospectively evaluated the CANA variables in 292 consecutive biventricular and 91 Fontan repair patients. The CANA variables included the heart rate variability, arterial baroreflex sensitivity (BRS), washout ratio of the myocardial metaiodobenzylguanidine scintigraphy, and plasma norepinephrine level. With a follow-up of 10 ± 2 years, 98 total events that required hospitalization, including 13 deaths and 48 unscheduled cardiac events (UCEs), occurred. In all the CHD patients, all the CANA indices predicted the total events and UCEs. Of those, the NE level (p = 0.0004) and BRS (p = 0.0373) predicted the mortality. In a multivariate analysis, the BRS was an independent CANA-predictor for the total events (p = 0.007). In the biventricular patients, the plasma NE level, heart rate variability, and BRS predicted the total events and UCEs and the BRS was the only independent CANA-predictor for the total events (p = 0.0329). In the Fontan patients, the plasma NE level was the only predictor for the UCEs (p = 0.0242) and no other CANA variables were independent predictors of the total events or UCEs. Conclusions: All CANA variables, especially the BRS, were useful predictors for future clinical events in biventricular CHD patients, whereas no CANA variables, except for the plasma NE level, predicted future clinical events in the Fontan patients

    Indication for Endoscopic Resection of Submucosal Colorectal Carcinoma: Special Reference to Lymph Node Metastasis

    We investigated the relationship between histological factors and lymph node metastasis in 77 lesions with submucosally invasive colorectal carcinomas to establish useful criteria for lesions in which endoscopic treatment alone results in cure of malignancy. There were positive correlations between histological factors, including the level of invasion, the histologic grade, presence or absence of lymphatic invasion, presence or absence of budding, and lymph node metastasis (p < 0.05, p < 0.05, p < 0.005, p < 0.01). The presence or absence of venous invasion did not influence lymph node metastasis. Laparoscopic surgery involving lymph node dissection should be indicated for sm1 carcinoma lesions with unfavorable histological factors. In lesions diagnosed as sm2 or sm3 prior to resection, intestinal resection involving lymph node dissection by laparoscopic surgery should be directly performed without endoscopic resection

    Hyper-luminous Dust Obscured Galaxies discovered by the Hyper Suprime-Cam on Subaru and WISE

    We present the photometric properties of a sample of infrared (IR) bright dust obscured galaxies (DOGs). Combining wide and deep optical images obtained with the Hyper Suprime-Cam (HSC) on the Subaru Telescope and all-sky mid-IR (MIR) images taken with Wide-Field Infrared Survey Explorer (WISE), we discovered 48 DOGs with iKs>1.2i - K_\mathrm{s} > 1.2 and i[22]>7.0i - [22] > 7.0, where ii, KsK_\mathrm{s}, and [22] represent AB magnitude in the ii-band, KsK_\mathrm{s}-band, and 22 μ\mum, respectively, in the GAMA 14hr field (\sim 9 deg2^2). Among these objects, 31 (\sim 65 %) show power-law spectral energy distributions (SEDs) in the near-IR (NIR) and MIR regime, while the remainder show a NIR bump in their SEDs. Assuming that the redshift distribution for our DOGs sample is Gaussian, with mean and sigma zz = 1.99 ±\pm 0.45, we calculated their total IR luminosity using an empirical relation between 22 μ\mum luminosity and total IR luminosity. The average value of the total IR luminosity is (3.5 ±\pm 1.1) ×\times 101310^{13} L_{\odot}, which classifies them as hyper-luminous infrared galaxies (HyLIRGs). We also derived the total IR luminosity function (LF) and IR luminosity density (LD) for a flux-limited subsample of 18 DOGs with 22 μ\mum flux greater than 3.0 mJy and with ii-band magnitude brighter than 24 AB magnitude. The derived space density for this subsample is log ϕ\phi = -6.59 ±\pm 0.11 [Mpc3^{-3}]. The IR LF for DOGs including data obtained from the literature is well fitted by a double-power law. The derived lower limit for the IR LD for our sample is ρIR\rho_{\mathrm{IR}} \sim 3.8 ×\times 107^7 [L_{\odot} Mpc3^{-3}] and its contributions to the total IR LD, IR LD of all ultra-luminous infrared galaxies (ULIRGs), and that of all DOGs are >> 3 %, >> 9 %, and >> 15 %, respectively.Comment: 15 pages, 15 figures, and 3 tables, accepted for publication in PASJ (Subaru special issue
