A kéz osteoid osteomáinak kivizsgálása és műtéti ellátása = Osteoid osteoma of the hand – Diagnostics and operative treatment
Absztrakt:
Bevezetés: Az osteoid osteoma az osteochondroma és a fibrosus
dysplasia után a harmadik leggyakoribb jóindulatú csonttumor, mely a legtöbb
esetben típusos tünetekkel, illetve radiológiai megjelenéssel jár. Jellemző az
éjszakai fájdalom, mely nemszteroid gyulladáscsökkentők adására szűnik.
Képalkotó vizsgálatokon a centralis „nidus” osteolyticus zónáját reaktív
sclerosis veszi körül. A kézen előfordulása ritka, jellemző tünetei
hiányozhatnak, illetve atípusos tünetekkel járhat. Röntgenfelvételeken
megjelenéséből szintén hiányozhatnak a típusos jellemzők. Diagnosztikája ennek
függvényében számos nehézségbe ütközhet. Klinikai/radiológiai gyanú esetén HRCT
(nagy felbontású CT) vizsgálat végzése javasolt. Célkitűzés:
Célunk, hogy összefoglaljuk ezen betegség patofiziológiáját, előfordulását,
klinikai jellemzőit, valamint a diagnózis felállításának nehézségeit a kézen
előforduló esetekben. Tárgyaljuk továbbá az ellátási lehetőségeket.
Módszer: Retrospektív vizsgálatot végeztünk a Semmelweis
Egyetem Ortopédiai Klinikáján 2014. március és 2018. december között.
Beválasztási kritérium volt: osteoid osteoma miatt ellátásban részesült betegek
a vizsgált periódus alatt. A kézen elhelyezkedő osteoid osteoma miatt műtéti
ellátásban részesült 6 beteg anyagát esetismertetésként mutatjuk be.
Eredmények: Ezen időszak alatt a klinikánkon kezelt osteoid
osteomák száma 112 volt, ebből 8 esetben (7%) fordult elő kézen az elváltozás. A
kézen előforduló elváltozások között 3 esetben hiányzott a típusos éjszakai
fájdalom, valamint 1 esetben a nemszteroid gyulladáscsökkentőkre szűnő fájdalom.
6 esetben végeztünk nyitott műtéti ellátást, excochleatiót, jó eredménnyel.
Következtetés: Az osteoid osteoma előfordulása a kézen
ritka, típusos tünetei elmaradhatnak, atípusos tünetek jelentkezhetnek.
Radiológiai megjelenése röntgenfelvételeken szegényes lehet, klinikai gyanú
esetén HRCT végzése javasolt. A rádiófrekvenciás termokoaguláció a kézen
jelentkező osteoid osteomák esetén nehézségekbe ütközhet, az arany standard
eljárás az excochleatio. Ezen műtét elvégzése esetén a panaszok enyhülését,

Abstract:
Introduction: Osteoid osteoma is a rare benign bone tumor
displaying typical clinical symptoms and radiological signs in most cases.
Characteristic symptoms are nocturnal pain alleviated by non-steroid
antiinflammatory drugs. Radiological findings are also characteristic, the
central osteolytic ‘nidus’ is surrounded by reactive sclerosis. These lesions
are rare in the hand, typical symptoms may be absent, furthermore, atypical
symptoms may occur. Characteristic radiographic signs may also be missing.
Therefore, diagnosis may prove difficult. In the case of radiological/clinical
suspicion, HRCT (high-resolution CT) is recommended. Aim: Our
aim is to summarize the pathophysiology, occurrence and clinical features of
these lesions and also the difficulties that accompany diagnosis on the hand.
Treatment options will also be discussed. Method: Retrospective
analysis was performed at Semmelweis University, Department of Orthopedics,
between March 2014 and December 2018. Inclusion criteria were: patients who had
undergone treatment for osteoid osteoma during this period. Data from the 6
patients who have undergone open surgery for osteoid osteoma of the hand will be
presented as case reports. Results: During this period, a total
of 112 patients were treated for osteoid osteoma at our Department, 8 lesions
were found on the hand (7%). Among the cases presenting on the hand, typical
nocturnal pain was absent in 3 cases, and in 1 case the pain did not respond to
non-steroid antiinflammatory drugs. Open surgery and curettage was performed in
6 cases with good results. Conclusion: Osteoid osteoma on the
hand is rare, typical symptoms may be missing, and atypical symptoms may occur.
Radiographic findings may be nondescript, HRCT is recommended in the case of
clinical suspicion. Performing radiofrequency ablation for osteoid osteoma of
the hand may pose difficulties, curettage is the gold standard of treatment.

A De Quervain-féle tendinopathia kezelése konzervatív módszerekkel = Treatment of De Quervain’s tendinopathy with conservative methods
Absztrakt:
Bevezetés: A De Quervain-féle tendinopathia a csukló-kéz
régióját érintő betegség. A hüvelykujj mozgatása fájdalmassá válik, a kéz
funkciója jelentősen romlik. A betegség a legújabb kutatások szerint inkább
degeneratív, semmint gyulladásos eredetű. Első lépésként a kéz sínezése,
nemszteroid gyulladáscsökkentő szerek és különböző fizikoterápiás kezelések
alkalmazása javasolt. Hatástalanságuk esetén további lehetőség az ínhüvelybe
fecskendezett szteroidinjekció és a műtéti úton végzett ínhüvelybemetszés.
Célkitűzés: Kutatásunkban megvizsgáltuk, hogy az
excentrikus tréninggel kibővített konzervatív kezelés megfelelő alternatíváját
nyújthatja-e a jelenleg elfogadott kezelési lehetőségeknek.
Módszer: Az excentrikus tréning 8 hétig tartott, melyet
indokolt esetben 12 hetesre bővítettünk. A betegek (n = 9) a betanítást követően
naponta többször végezték a tréninget, amit a heti találkozók alkalmával
kontrolláltunk. Az 1., a 8., valamint a 12. heti találkozó során az inspekciót
követően mértük az ízületi mozgástartományt, az izomerőt, a fájdalmas régiók
számát, illetve elvégeztük a ’Numeric Pain Rating Scale’, a ’Quick Disabilities
of the Arm, Shoulder and Hand’, valamint a ’Patient-Rated Wrist Evaluation’
kérdőívek felvételét. A méréseket páros mintás t-teszttel és ismételt méréses
varianciaanalízissel elemeztük. Az elemzéseket IBM SPSS Statistics 25.0 és
Microsoft Office Excel Professional Plus 2016 programmal végeztük; p<0,05
esetén tekintettük statisztikailag szignifikánsnak eredményeinket.
Eredmények: Szignifikáns javulást mértünk a fájdalom
intenzitása (’Numeric Pain Rating Scale’ p = 0,005, n = 9) és a kéz, valamint a
csukló funkciója terén (’Quick Disabilities of the Arm, Shoulder and Hand
Outcome Measure’ kérdőív 1. rész p<0,001, 2. rész p<0,001, ’Patient-Rated
Wrist Evaluation’ kérdőív p<0,001; n = 9). Következtetés:
Eredményeink alapján megfelelő betegbeválasztás mellett az excentrikus
tréninggel kibővített konzervatív kezelés valós alternatívája lehet a jelenleg

Abstract:
Introduction: De Quervain’s tendinopathy affects the region of
the wrist and the hand. Thumb motion becomes painful. This illness is caused by
a degenerative process rather than inflammation. Primary treatment methods are
splinting, taking non-steroid anti-inflammatory drugs and different physical
therapeutic modalities, administration of a steroid injection into the tendon
sheath or surgical release of the tendon sheath may be performed.
Aim: The aim of the present study was to investigate
whether conservative treatment complemented by eccentric training could provide
an adequate alternative to the currently accepted treatment options.
Method: The eccentric training lasted for 8 weeks (if
necessary for 12 weeks). Following the introduction to exercises, patients (n =
9) repeated the training several times a day, which was controlled during weekly
meetings. At the 1st, 8th and 12th meetings, inspection and the following
measurements were performed: range of motion, muscle strength, evaluation and
number of painful regions including the completion of patient questionnaires.
Data were analysed with paired samples t-tests and repeated measures ANOVA. IBM
SPSS Statistics 25.0 and Microsoft Office Excel Professional Plus 2016 programs
were used. Results were regarded significant at level of p<0.05.
Results: Significant improvements were found in the
intensity of pain (Numeric Pain Rating Scale p = 0.005, n = 9) and in the
functionality of the hand and wrist (Quick Disabilities of the Arm, Shoulder and
Hand questionnaire part 1. p<0.001, part 2. p<0.001, Patient-Rated Wrist
Evaluation questionnaire p<0.001; n = 9). Conclusion: With
careful patient selection, conservative treatment complemented by eccentric
training could be an alternative to current treatment options. Orv Hetil. 2020;

Hatástanulmány a nyeregízületi arthrosis intraarticularis hialuronsavas kezeléséről Sinoval HL® injekcióval = Study on intra-articular hyaluronic acid treatment of saddle joint arthrosis with Sinoval HL® injection
Saddle joint arthrosis is a common disease in the aging patient population, associated with significant pain and reduced hand function. In the treatment of this progressive disease several questions remain unanswered; however the interdisciplinary literature agrees that the maximum utilization of conservative therapy is recommended. Sinovial HL® intra-articular injection contains a mixture of light and heavy molecular weight hyaluronic acid chains and has anti-inflammatory and chondroprotective properties when injected into the saddle joint. The aim of the research was to investigate the clinical effectiveness of the injection. Patients treated for arthrosis of the saddle joint were selected for the research, those who also had radiological changes appropriate for intraarticular viscosupplementation (Eaton–Littler stage II–IV) and whose complaints did not improve after 6–8 weeks of initial first-line conservative therapy. Depending on their complaints, the patients received one–three injections into the saddle joint. Out of the 21 selected patients, the complaints showed a significant improvement in 20 cases, additional surgical treatment became necessary in 1 case only. Sinovial HL® injection can be used effectively in the conservative treatment of saddle joint arthrosis
„Greater arc injuries” - irodalmi áttekintés egy saját eset bemutatása kapcsán = "Greater arc injuries" - literature review in connection with a case report
Perilunate dislocations and fracture–dislocations are rare injuries, however, even if proper operative treatment and postoperative aftercare is performed, it is difficult to achieve good results. Greater arc injury is a fracture-dislocation pattern of injury involving the perilunate carpal bones and in some cases the radius with ligament damage; thereby damaging the biomechanical complex of the wrist. However, given that these injuries affect the young population active in work and sports, the aim is to restore the biomechanics, the range of motion and the loading capacity of the wrist, without leaving any pre-arthrotic elements behind. The authors review presents the etiology, incidence and main diagnostic (X-ray and CT) signs of the disorder, and its surgical treatment, postoperative fixation and rehabilitation methods. In addition to the case presentation of a 24-year-old male patient, questionable points of the care process and points that can be optimized are discussed in relation to his daily care
Négyes arthrodesissel elért eredményeink a SNAC és a SLAC okozta csukló arthrosisok megoldásában = Our results with four corner fusion in the treatment of wrist arthrosis caused by SNAC and SLAC
A csuklót érő hatások esetén gyakran találkozhatunk a proximalis kéztőcsontsorban található sajkacsontot vagy környezetét érintő sérülésekkel. Az itt található összetett biomechanikai rendszer bármilyen okból történő megszakadása a csontok közötti konstrukció felbomlásához vezet, ami az arthrotikus folyamatok megindulását is magába foglalja. A sajkacsont törése vagy a scapholunaris szalag sérülése során létrejövő szerkezeti instabilitás két speciális arthrosis menetet hoz létre: a Scaphoid Nonunion Advanced Collapse (SNAC) és Scapholunate Advanced Collapse (SLAC) wrist-et. Mindkét deformitás esetében számos műtéti technika áll rendelkezésre, a különböző stádiumoktól függően. A műtéti megoldások közül lehetőleg kerülendő a teljes arthrodesis, mert a betegek számára a mozgástartomány teljes hiánya az életminőség romlásához vezet. Éppen ezért a cél mindig a mozgásmegtartó műtétek előtérbe helyezése, amelyek közé a négyes arthrodesis is tartozik. A 4CF alternatíva lehet a SNAC és SLAC wrist II. és III. stádiumában. A Semmelweis Egyetem Ortopédiai Klinika és az Országos Sportegészségügyi Intézet Sportsebészeti Osztály beteganyagának, illetve a szakirodalomban található eredmények alapján a négyes arthrodesis eredményeit vizsgáltuk a mindennapi tevékenységekhez, munkához szükséges mozgástartomány, erő és a funkcionális kézpanaszok tekintetében, releváns kérdőívek és mérések elvégzésével
Ízületi szögmérő használatának jelentősége Dupuytren betegségben = Importance of using a goniometer in Dupuytren's disease
There is currently no definitive therapeutic option for the treatment of Dupuytren's disease - a condition that leads to progressive decrease regarding range of motion of the fingers. Therapeutic decisions made on several occasions for each patient during their lifetime. The disease characterized by joint range of motion values or more precisely the degree of extension deficit. There are a myriad of diseases and post-traumatic conditions of the hand where functions described by the degree of limitation in joint motions to characterize the extent of damage of the healing process itself. Precision or lack there of is dependent on measurement technique – this may well be demonstrated through the example of Dupyutren’s disease, as in the cases therapeutic planning is based in the accurate measurement of progression and recurrences. The use of a goniometer may ensure accurate data collection, but its use is not consistent in clinical practice. The purpose of our study was to assess the degree of inaccuracy when determining joint angles when foregoing the use of a goniometer through the example of Dupuytren's disease. During the data collection, we showed 97 colleagues of different professional experience images depicting contractures, on which the degree of joint flexion had gauged by visual assessment only. Specialists with significant work experience estimated more accurately on average compared to their resident and candidate specialist colleagues, while regarding field of profession, hand surgeons were the most successful. The assessments of colleagues who used a goniometer most often during the examination of patients were more reliable compared to those who never or rarely used this instrument. Abandoning the use of a goniometer leads to inaccuracy. In most cases, precise assessment of the course of the disease (progression, recurrences) is not possible without an instrument, but by visual estimation only, we suggest the use of a goniometer to achieve necessary precision
Effects of Vitamin D on Fertility, Pregnancy and Polycystic Ovary Syndrome—A Review
Polycystic ovary syndrome (PCOS) is one of the most common endocrine reproductive disorders in women. Vitamin D deficiency is also quite common in this condition. The degree of vitamin D deficiency correlates with the severity of PCOS. Both male and female vitamin D levels play a role in fertility and affect the outcomes of in vitro fertilization (IVF). Moreover, fertility and IVF indicators are improved by vitamin D not only in healthy women but in those diagnosed with PCOS. Both vitamin D deficiency and PCOS increase pregnancy-related complications. Vitamin D supplementation and optimal vitamin D levels decrease both maternal and fetal risk for complications and adverse events. Furthermore, vitamin D supplementation may ameliorate or even prevent pregnancy-related reversible bone loss in mothers. This review emphasizes the roles of vitamin D deficiency and vitamin D supplementation and their correlation with PCOS regarding reproductive health
