    Tissue eosinophilia and eosinophil degranulation in Riedel's invasive fibrous thyroiditis.

    The etiology of Riedel's invasive fibrous thyroiditis (IFT) has remained obscure. This rare disorder has been confused in the past with the more common fibrous variant of Hashimoto's disease. The typical histological features of IFT, in particular the presence of an invasive fibrosclerotic process in conjunction with a prominent chronic inflammatory infiltrate, suggest that the release of fibrogenic cytokines and other factors from these cellular infiltrates may play an important role in the pathogenesis of this condition. Our observations in routinely processed tissue sections obtained from patients with documented IFT of striking tissue eosinophilia led us to hypothesize that eosinophils and their products may play a role in the evolution of this disease. Immunofluorescence staining with affinity-purified polyclonal rabbit antibody directed against human eosinophil granule major basic protein revealed marked tissue eosinophilia and abundant extracellular deposition of major basic protein in all specimens from 16 patients with IFT. By contrast, only occasional eosinophils and no extracellular major basic protein were detected in control thyroid tissues obtained from patients with multinodular goiter, Graves' disease, Hashimoto's disease, and normal thyroid tissue. The presence of marked eosinophil infiltration and extracellular major basic protein deposition in IFT and other associated fibrosclerotic conditions suggests a role for eosinophils and their products in propagating the fibrogenesis seen in IFT

    Incontinence-specific quality of life measures used in trials of treatments for female urinary incontinence: a systematic review.

    This systematic review examined the use of incontinence-specific QOL measures in clinical trials of female incontinence treatments, and systematically evaluated their quality using a standard checklist. Of 61 trials included in the review, 58 (95.1%) used an incontinence-specific QOL measure. The most commonly used were IIQ (19 papers), I-QoL (12 papers) and UDI (9 papers). Eleven papers (18.0%) used measures which were not referenced or were developed specifically for the study. The eight QOL measures identified had good clinical face validity and measurement properties. We advise researchers to evaluate carefully the needs of their specific study, and select the QOL measure that is most appropriate in terms of validity, utility and relevance, and discourage the development of new measures. Until better evidence is available on the validity and comparability of measures, we recommend that researchers consider using IIQ or I-QOL with or without UDI in trials of incontinence treatments

    The N-end rule pathway controls multiple functions during Arabidopsis shoot and leaf development

    The ubiquitin-dependent N-end rule pathway relates the in vivo half-life of a protein to the identity of its N-terminal residue. This proteolytic system is present in all organisms examined and has been shown to have a multitude of functions in animals and fungi. In plants, however, the functional understanding of the N-end rule pathway is only beginning. The N-end rule has a hierarchic structure. Destabilizing activity of N-terminal Asp, Glu, and (oxidized) Cys requires their conjugation to Arg by an arginyl–tRNA–protein transferase (R-transferase). The resulting N-terminal Arg is recognized by the pathway's E3 ubiquitin ligases, called “N-recognins.” Here, we show that the Arabidopsis R-transferases AtATE1 and AtATE2 regulate various aspects of leaf and shoot development. We also show that the previously identified N-recognin PROTEOLYSIS6 (PRT6) mediates these R-transferase-dependent activities. We further demonstrate that the arginylation branch of the N-end rule pathway plays a role in repressing the meristem-promoting BREVIPEDICELLUS (BP) gene in developing leaves. BP expression is known to be excluded from Arabidopsis leaves by the activities of the ASYMMETRIC LEAVES1 (AS1) transcription factor complex and the phytohormone auxin. Our results suggest that AtATE1 and AtATE2 act redundantly with AS1, but independently of auxin, in the control of leaf development

    Testing Einstein's time dilation under acceleration using M\"ossbauer spectroscopy

    The Einstein time dilation formula was tested in several experiments. Many trials have been made to measure the transverse second order Doppler shift by M\"{o}ssbauer spectroscopy using a rotating absorber, to test the validity of this formula. Such experiments are also able to test if the time dilation depends only on the velocity of the absorber, as assumed by Einstein's clock hypothesis, or the present centripetal acceleration contributes to the time dilation. We show here that the fact that the experiment requires Îł\gamma-ray emission and detection slits of finite size, the absorption line is broadened; by geometric longitudinal first order Doppler shifts immensely. Moreover, the absorption line is non-Lorenzian. We obtain an explicit expression for the absorption line for any angular velocity of the absorber. The analysis of the experimental results, in all previous experiments which did not observe the full absorption line itself, were wrong and the conclusions doubtful. The only proper experiment was done by K\"{u}ndig (Phys. Rev. 129 (1963) 2371), who observed the broadening, but associated it to random vibrations of the absorber. We establish necessary conditions for the successful measurement of a transverse second order Doppler shift by M\"{o}ssbauer spectroscopy. We indicate how the results of such an experiment can be used to verify the existence of a Doppler shift due to acceleration and to test the validity of Einstein's clock hypothesis.Comment: 11 pages, 4 figure

    Treatment-seeking rates in malaria endemic countries

    BACKGROUND: The proportion of individuals who seek treatment for fever is an important quantity in understanding access to and use of health systems, as well as for interpreting data on disease incidence from routine surveillance systems. For many malaria endemic countries (MECs), treatment-seeking information is available from national household surveys. The aim of this paper was to assemble sub-national estimates of treatment-seeking behaviours and to predict national treatment-seeking measures for all MECs lacking household survey data. METHODS: Data on treatment seeking for fever were obtained from Demographic and Health Surveys, Malaria Indicator Surveys and Multiple Cluster Indicator Surveys for every MEC and year that data were available. National-level social, economic and health-related variables were gathered from the World Bank as putative covariates of treatment-seeking rates. A generalized additive mixed model (GAMM) was used to estimate treatment-seeking behaviours for countries where survey data were unavailable. Two separate models were developed to predict the proportion of fever cases that would seek treatment at (1) a public health facility or (2) from any kind of treatment provider. RESULTS: Treatment-seeking data were available for 74 MECs and modelled for the remaining 24. GAMMs found that the percentage of pregnant women receiving prenatal care, vaccination rates, education level, government health expenditure, and GDP growth were important predictors for both categories of treatment-seeking outcomes. Treatment-seeking rates, which varied both within and among regions, revealed that public facilities were not always the primary facility type used. CONCLUSIONS: Estimates of treatment-seeking rates show how health services are utilized and help correct reported malaria case numbers to obtain more accurate measures of disease burden. The assembled and modelled data demonstrated that while treatment-seeking rates have overall increased over time, access remains low in some malaria endemic regions and utilization of government services is in some areas limited

    Multi-parameter analysis in eddy current inspection of aircraft engine components

    One of the major problems limiting the life of critical aircraft engine components, such as compressor discs and spacers, is the formation of low cycle fatigue (LCF) cracks in the fastener bolt holes. Such cracks are often initiated from corners and their surfaces are oxidized during the engine operation. Eddy current techniques using rotating probes are considered to be the most appropriate for detecting bolt hole cracks. Inspection according to damage tolerance criteria requires repeatable detection (90% probability of detection with 95% confidence) of cracks of the order of 0.125 mm (0.005″). If only threshold setting methods are used by a human analyst or implemented by means of electronic instrumentation, detectability can be low since it is difficult to distinguish between the actual flaw signal and noise in the eddy current signal as both are of similar amplitude. However, in certain cases, searching for structure in the noisy waveform can provide indications of defects that escape detection by threshold setting techniques. One way of achieving this is by using multi-parameter signal analysis and pattern recognition methods

    Acoustic observations of vertical and horizontal swimming velocities of a diel migrator

    A strong sound scattering layer which underwent diel vertical migration was observed over 176 days using a bottom-mounted 600 kHz ADCP at a depth of approximately 285 m on the upper continental slope. Average observed descent times of the scatterers were within 12 minutes of sunrise and average ascent times were within 13 minutes of sunset. Average ascent speeds progressively increased away from the bed and approached 6 cm/s. Average descent speeds similarly reached a maximum of ∌6 cm/s. Horizontal velocities of the scatterers during vertical migration were found to be smaller than ambient velocities (by up to 3 cm/s), and it is inferred that the migrators were swimming horizontally against the flow. Horizontal velocities of the scatterers during time intervals when the layer was near the seafloor were found to be southwest (up to 3 cm/s), and onshelf (up to 1.7 cm/s) relative to the flow above the layer. Swimming velocities were independent of alongshelf flow direction, counter to the hypothesis that the scatterers sought to maintain their position by swimming against the flow
