    Kikuchi-Fujimoto (KD) disease is the rare differential diagnoses of chronic cervical lymphadenopathy of unknown etiology. The findings of histopathology may be overlapping, in such condition immunohistochemistry has a definite role to play. Since Tubercular lymphadenopathy is the commonest cause for chronic cervical lymphadenopathy in developing and tropical country like India. Occasionally it is misdiagnosed, ignoring the other rare condition like KD if immunohistochemistry is not taken into consideration to differentiate. As a result the morbidity increases and cost of antitubercular treatment (ATT) in wrong diagnosed case is enormous. We report a similar case of misdiagnoses, non responsive to ATT, finally diagnosed as a case of Kikuchi-Fujimoto disease, did well after recommended treatment

    The trends of incidence, morbidity and mortality of tuberculosis in children have always remained unclear because of lack of a definitive diagnostic tool in mostly cases. The diagnosis of tuberculosis in children remains a major challenge as it is seldom confirmed and is mostly based on clinical signs, symptoms and special investigations e. g. TST, chest X-ray and sputum smear microscopy. Sputum smear microscopy is positive in less than 10-15% of children with TB and culture yields are nearly about 30-40%. In low burden countries, the triad of (a) known contact with an infectious source case, (b) a positive tuberculin skin test and (c) a suggestive CXR is usually used to establish a diagnosis of childhood TB. The research on childhood tuberculosis is often neglected due to technical difficulties like slow growth in culture, the difficulty of obtaining specimens and the diverse and relatively nonspecific clinical presentation of tuberculosis in this age group. The complexity in making an exact diagnosis of tuberculosis in children have led to the development of several diagnostic approaches like point scoring systems, diagnostic classifications and diagnostic algorithms. The aim of this study was to review and analyse the different diagnostic approaches to childhood tuberculosis. In this paper, the mostly used techniques like culture methods as well as the non-culture methods including serology, biochemistry and polymerase chain reaction and DNA fingerprinting for the diagnosis of tuberculosis will be discussed.     Double coin in esophagus at same location and same alignment - a rare occurrence: a case report

    Coin is the most common foreign body swallowed by pediatric age group. The multiple coin swallowing is extremely rare and very few cases had been reported in English literature. Most of them were present at different site and had different alignment in the esophagus. The location of the coin (trachea vs. esophagus) is commonly determined by the alignment of the coin on radiographic studies. A 4-year-girl was presented to us with history of coin ingestions one day back without any respiratory distress. On radiological study there was suspicion of two coins on same location and alignment. The diagnosis was confirmed after removal. The both coin was removed successfully by esophagoscopy. Unexpected second foreign bodies in pediatric esophageal coin ingestions are rare and it is mandatory to do post operative radiography after removal to exclude duplex coin or tracheal coin. We are presenting this case because of its rarity, difficulty in diagnosis especially when proper history is not available

    Thalassaemia is a blood disorder in which an abnormal form of haemoglobin is made due to genetic aberration. Although no national database exists to calculate the exact number of thalassaemics, but it is estimated that there are 9.8 million carriers of the disease in Pakistan [1]. Consanguineous marriage is the leading cause for this high prevalence rate due to which the thalassaemia gene is trapped within the affected families. These patients are dependent on regular blood transfusions to sustain life in addition to expensive chelation therapy and other medical management. As a result, thalassaemia is a major healthcare challenge and places great psychological and financial trauma on the affected families and is a huge burden on the national healthcare delivery system [2]
