    Decrease in shunt volume in patients with cryptogenic stroke and patent foramen ovale

    <p>Abstract</p> <p>Background</p> <p>In patients with patent foramen ovale (PFO) there is evidence supporting the hypothesis of a change in right-to-left shunt (RLS) over time. Proven, this could have implications for the care of patients with PFO and a history of stroke. The following study addressed this hypothesis in a cohort of patients with stroke and PFO.</p> <p>Methods</p> <p>The RLS volume assessed during hospitalisation for stroke (index event/T0) was compared with the RLS volume on follow-up (T1) (median time between T0 and T1 was 10 months). In 102 patients with a history of stroke and PFO the RLS volume was re-assessed on follow-up using contrast-enhanced transcranial Doppler/duplex (ce-TCD) ultrasound. A change in RLS volume was defined as a difference of ≥20 microembolic signals (MES) or no evidence of RLS during ce-TCD ultrasound on follow-up.</p> <p>Results</p> <p>There was evidence of a marked reduction in RLS volume in 31/102 patients; in 14/31 patients a PFO was no longer detectable. An index event classified as cryptogenic stroke (P < 0.001; OD = 39.2, 95% confidence interval 6.0 to 258.2) and the time interval to the follow-up visit (P = 0.03) were independently associated with a change in RLS volume over time.</p> <p>Conclusions</p> <p>RLS volume across a PFO decreases over time, especially in patients with cryptogenic stroke. These may determine the development of new strategies for the management in the secondary stroke prevention.</p

    Infrastructure-Based Digital Twins for Cooperative, Connected, Automated Driving and Smart Road Services

    Driving requires continuous decision making from a driver taking into account all available relevant information. Automating driving tasks also automates the related decisions. However, humans are very good at dealing with bad quality, fuzzy, informal and incomplete information, whereas machines generally require solid quality information in a formalized format. Therefore, the development of automated driving functions relies on the availability of machine-usable information. A digital twin contains quality controlled information collected and augmented from different sources, ready to be supplied to such an automated driving function. An information model that describes all conceivably relevant information is necessary. To this end, a list of requirements that such an information model should meet is proposed and each requirement is argued for. Based on the anticipated services and applications that such a system should support, a collection of requirements for system architecture is derived. Information modeling is performed for selected relevant information groups. A system architecture has been proposed and validated with three different implementations, addressing several different applications to support decisions at a highway tunnel construction site in Austria and throughout the Test Bed Lower Saxony in Germany

    End-to-End latency in HAD applications using cloud technology

    In the drive towards truly automated driving infrastructure data will play a substantial role, as it enhances the event horizon of the autonomous vehicle and enables the road operator to communicate strategic routing information. As infrastructure data is basically an aggregation of large data source systems, the guaranteed latency with which relevant information can be conveyed to the vehicle poses a challenge. This paper breaks up the downstream data chain from the infrastructure to the vehicle into its generic building blocks and focusses on the data throughput rate of the infrastructure database element. The achievable throughput rates are determined experimentally in a real life productive system during standard operation, the traffic information system of Austrian highway operator ASFINAG. The throughput rates through the main data gates have been made configurable and the timestamps for data passing through the individual software modules are recorded."br" Measurement results for the configuration with the highest throughput rate show a mean latency of 2 to 6 seconds for traffic messages from infrastructure into the vehicle, excluding the time for event detection. The concept will be expanded to eventually determine and monitor latency through all building blocks of the data chain

    Helicobacter pylori Employs a Unique Basolateral Type IV Secretion Mechanism for CagA Delivery.

    The Helicobacter pylori (Hp) type IV secretion system (T4SS) forms needle-like pili, whose binding to the integrin-β1 receptor results in injection of the CagA oncoprotein. However, the apical surface of epithelial cells is exposed to Hp, whereas integrins are basolateral receptors. Hence, the mechanism of CagA delivery into polarized gastric epithelial cells remains enigmatic. Here, we demonstrate that T4SS pilus formation during infection of polarized cells occurs predominantly at basolateral membranes, and not at apical sites. Hp accomplishes this by secreting another bacterial protein, the serine protease HtrA, which opens cell-to-cell junctions through cleaving epithelial junctional proteins including occludin, claudin-8, and E-cadherin. Using a genetic system expressing a peptide inhibitor, we demonstrate that HtrA activity is necessary for paracellular transmigration of Hp across polarized cell monolayers to reach basolateral membranes and inject CagA. The contribution of this unique signaling cascade to Hp pathogenesis is discussed
