    Background: Counseling about nutrition of tembang still not known in wide society especially adolescent hence therefore need socialization and delivery of Guidance of Balanced Nutrition. Efforts to improve knowledge about balanced nutrition in adolescents require strategic approaches to be achieved effectively and efficiently. Health education methods one approach that is often used to convey a message or information so that the information provided is acceptable and well understood by the audience. Various media used as a supporter and tool for extension methods one of them is visual media that can channel the message in the form of visual communication symbols that need to be understood in the form of leaflet and audiovisual which can provide real stimulation containing motion picture and sound element with duration a relatively short time that is shown in video form. Objective: To Analyze the Effectiveness of Media Leaflet and Video Use of Knowledge of Balanced Nutrition Guidance on Students of SMP Negeri 5 Yogyakarta. Method: The type of research used quasi experimental (Quasi Experimental) with pretest-postest design with control group design. This research was conducted at SMPN 5 Yogyakarta on 5-6 April 2018. Samples with 36 people from leaflet and video group. The data analysis uses paired t-test and independent t-test. Results: The mean score of knowledge of balanced nutrition before counseling with media leaflets was 8.94 and 9.44 videos. While after being given counseling using media leaflet 10.78 and video 12,11. The result of the research shows the effect of media giving to the change of knowledge (delta p = 0,002). Conclusion: There is a difference of effectiveness of leaflet and video media usage on knowledge of balanced nutrition guidance on students of SMPN 5 Yogyakarta.     Latar Belakang : Penyuluhan tentang gizi sembang masih belum dikenal di kalangan masyarakat luas khususnya remaja maka dari itu perlu adanya sosialisasi dan penyampaian Pedoman Gizi Seimbang. Upaya untuk meningkatkan pengetahuan tentang gizi seimbang pada remaja memerlukan cara pendekatan yang strategis agar tercapai secara efektif dan efisien. Metode penyuluhan kesehatan salah satu pendekatan yang sering digunakan untuk menyampaikan pesan atau informasi sehingga informasi yang diberikan dapat diterima dan dipahami dengan baik oleh audien. Berbagai media yang digunakan sebagai penunjang dan alat bantu untuk metode penyuluhan salah satunya adalah media visual yang dapat menyalurkan pesan yang berupa simbol-simbol komunikasi visual yang perlu dipahami dalam bentuk leaflet dan audiovisual yang dapat memberikan stimulasi secara nyata berisi gambar gerak dan unsur suara dengan durasi waktu relatif pendek yang ditayangkan dalam bentuk video. Tujuan : Menganalisis Keefektivitasan Penggunaan Media Leaflet dan Video Terhadap Pengetahuan Pedoman Gizi Seimbang Pada Siswa SMP Negeri 5 Yogyakarta. Metode : Jenis penelitian yang digunakan eksperimen semu (Quasi Experimental) dengan rancangan pretest-postest with control group design. Penelitian ini dilaksanakan di SMPN 5 Yogyakarta pada tanggal 5-6 April 2018. Sampel dengan jumlah 36 orang dari kelompok leaflet dan video. Analisis data menggunalan paired t-test dan independent t-test. Hasil : Rata-rata skor pengetahuan gizi seimbang sebelum penyuluhan dengan media leaflet adalah 8,94 dan video 9,44. Sedangkan setelah diberikan penyuluhan menggunakan media leaflet 10,78 dan video 12,11. Hasil penelitian menunjukan adanya pengaruh pemberian media terhadap perubahan pengetahuan (delta p=0,002). Kesimpulan : ada perbedaan efektivitas penggunaan media leaflet dan video terhadap pengetahuan pedoman gizi seimbang pada siswa SMPN 5 Yogyakarta.
