    Debridement with high speed burr reduce more colonies of Staphylococcus aureus in chronic osteomyelitis of rabbit’s tibia compare to debridement with curettage

    Background: Chronic osteomyelitis has been a difficult problem for the patient and for the treating physician. Chronic osteomyelitis is defined by most authors as osteomyelitis with symptoms that have been present for more than 1 month. Chronic osteomyelitis also defined by an infected dead bone within compromise soft tissue. An aggressive debridement and specific antibiotic treatment is first choice for chronic osteomyelitis. The purpose of this study was to assess the reduction in the number of colonies of Staphylococcus aureus after debridement with a high speed burr compared to debridement with curettage.Methods: This study is an experimental study was designed using a randomized post-test only group design. A total of 42 rabbits inoculated his right tibia with 0.1 ml Staphylococcus aureus 107 CFU / ml, and then inserted with K-wire intramedullary. Two weeks after inoculation, chronic osteomyelitis occurs, grade 3 and 4 radiologically. Then 10 rabbits were sacrificed without debridement, colonies of germs counted as control, 16 rabbits was performed debridement with curettage and 16 rabbits was performed debridement with high speed burr. Three weeks after debridement, 32 rabbits was sacrificed and counted for final germs colonies.Results: The results obtained from debridement with high speed burr and debridement with curettage, number of colonies of Staphylococcus aureus is less than the number of control’s colonies, with a value of p = 0.006 (p <0.05), debridement with a high speed burr also decrease the number of colonies of Staphylococcus aureus compared to debridement with curettage, with p = 0.023 (p<0.05), odds ratio of 9.000 (CI=95%). This is shows that this difference is statistically significant.Conclusions: The data showed that debridement with a high speed burr more likely to decrease the number of colonies of germs that can be recommended as a method of debridement for chronic osteomyelitis

    The Correlation Of Body Mass Index And Age With Disability Rate In Patients With Grade Ii-Iv Knee Osteoarthritis According To Kellgren-Lawrence At Rs Phc Surabaya

    Introduction: Knee osteoarthritis is a degenerative joint disease that is most common in adults and the elderly in many countries. Incidence of osteoarthritis is increased steadily over the last 15 years. Currently, there is still no simple therapy to cure osteoarthritis disease. Therefore, it is very important to find the risk factor of osteoarthritis to do etiology prevention. Aim: The purpose of this study is to discover the correlation of body mass index and age with disability rate in patients with Grade II-IV Knee Osteoarthritis. Methods: This research is an observational analytical study using cross sectional approach. The sampling techniques used was purposive sampling. Data collection was done by interviewing respondents using KOOS-PS questionnaire. Result: There is a significant, strong correlation between body mass index and disability rate in patients with knee joint OA (r=0.661). However, there is no significant correlation between age and knee joint OA disability (r=0.150). A moderate correlation is found, respectively, between body mass index and the grade of knee joint OA (r=0.424) and also between age and the grade of knee joint OA (r=0.352). Conclusion: There is a significant correlation between body mass index and disability rate of patients with knee joint OA

    Introduction: Posterior tibial tendon dysfunction is one of the most common, problems of the foot and ankle. Tenosynovitis of the posterior tibial tendon (PTT) is an often unrecognized form of PTT dysfunction. Case: A 54-year-old woman presented with left ankle pain that began while morning walk three days prior. She noted that the left ankle hurt with even light touch and the pain was unrelieved with sodium diclofenac. She denied any history of trauma. She was seen in the outpatient clinic for this condition. On examination, a three centimeter area of pain was found posterior to the medial malleolus and parallel to the PTT. She also had a stage I flat foot and mild soft tissue swelling around medial malleolus region on her radiograph examination. Ultrasound examination was done with the result of anechoic fluid visible in the peritendinous space around the PTT. The patient received diagnosis of PTT tenosynovitis, with the foot and ankle disability index (FADI) score was 58.7. Platelet rich plasma (PRP) injection was done twice with an interlude of two weeks. The pain subsided and the following FADI score was 84.6. Outcome: Patient showed improvement in her left ankle PTT tenosynovitis after two PRP injection. Conclusion: This case report highlights the efficacy of PRP as a modality in managing PTT tenosynovitis

    High expression levels of bone morphogenetic protein-2 and p-glycoprotein related with progressivity in patient with osteosarcoma

    Background: Osteosarcoma progression characterized by the presence of metastasis dan local recurrence. Several studies focused on bone morphogenetic protein-2 (BMP-2) and P-glycoprotein as protein that influence the progression of osteosarcoma, which are characterized by metastasis and local recurrence.Methods: This study was a cross sectional analytic study. This study looks at the relationship level of BMP-2 and P-glycoprotein with metastasis and local recurrence in osteosarcoma. Twenty three research subjects are paraffin blocks of osteosarcoma patients and secondary data was conducted through medical records to determine metastasis and local recurrence that occurs in these patients.Results: The result showed significant correlation between BMP-2 and P-glycoprotein with metastasis and local reccurence.Conclusions: BMP-2 and P-glycoprotein positivity as determined by immunohistochemistry, was a strong correlate of more rapid disease progression. It should be taken into consideration to identify a subgroup of osteosarcoma patients with poor outcome at the time of diagnosis. So it might be an important marker in planning innovative chemotherapeutic regimens
