    Management of papillary microcarcinomas of thyroid gland is subject to many discussions.Objective : Aim of this study: determinate lymph node status in these malignant tumors.Materials and methods : this study is about 20 cases of papillary microcarcinomas of thyroid gland, operated between 1994 and 2007,Results : Our study was about 18 women and 2 men with a middle-age of 46 years. We had found that 35% of microcarcinomas had a size higher than 0,5cm.Neck lymph node metastasis were present in 83% when the size of tumor was higher than 0,5 cm.Conclusion : Neck lymph node metastasis in micropapillary carcinomas is conditioned by many factors of which depend the therapeutic management.Keywords : Thyroid gland, papillary microcarcinoma, neck-lymph node dissection

    Cancers du larynx etude retrospective Ă  propos de 90 cas

    Objectif : Le cancer du larynx est parmi les premiers cancers de la sphĂšre oto-rhino laryngĂ©e. La dysphonie constitue le maĂźtre symptĂŽme rĂ©vĂ©lateur et peut ĂȘtre associĂ©e Ă  une dyspnĂ©e selon la localisation tumorale. Il s’agit essentiellement d’un carcinome Ă©pidermoide. L’alcoolo-tabagisme joue un rĂŽle important dans la genĂšse de ce cancer.MatĂ©riel et mĂ©thodes: Notre Ă©tude est rĂ©trospective Ă  propos de 90 cas colligĂ©s sur une pĂ©riode de 18 ans (1990-2007).RĂ©sultats : La prĂ©dominance du sexe masculin est nette avec une seule femme dans notre sĂ©rie. Le pic d’ñge est entre 60 et 70 ans. 98,9% de nos patients sont des fumeurs chroniques. 14,4% ont Ă©tĂ© suivis pour une laryngite chronique. La dysphonie a Ă©tĂ© retrouvĂ©e chez tous nos patients. Le dĂ©lai de consultation moyen est relativement long avec une moyennede 8 mois. Le point de dĂ©part de la tumeur a Ă©tĂ© le plan glottique dans 72,2% et sus glottique dans 23,4%. Le type histologique le plus frĂ©quent est le carcinome Ă©pidermoĂŻde dans 92,2%. Le bilan d’extension a systĂ©matiquement comportĂ© une endoscopie sous anesthĂ©sie gĂ©nĂ©rale avec une tomodensitomĂ©trie. La majoritĂ© de nos malades ont Ă©tĂ© classĂ©sT3-T4 et ceci dans 31,4%. Le taux de survie globale Ă  5 ans a Ă©tĂ© de 57,1%.Conclusion : Le cancer du larynx est un cancer du sujet de sexe masculin surtout alcoolo-tabagique. Le bilan endoscopique et radiologique essentiellement scanographique va permettre un bilan d’extension et guider ainsi la conduite thĂ©rapeutique. Le traitement est essentiellement chirurgical, couplĂ© parfois Ă  une radiothĂ©rapie ou une chimiothĂ©rapie.Le pronostic est le plus frĂ©quemment favorable surtout si le diagnostic est prĂ©coce.Mots-clĂ©s : Larynx, Cancer, TomodensitomĂ©trie,Traitemen

    Human Cellular Immune Response to the Saliva of Phlebotomus papatasi Is Mediated by IL-10-Producing CD8+ T Cells and Th1-Polarized CD4+ Lymphocytes

    Cutaneous leishmaniasis affects millions of people worldwide and is caused by protozoa of the genus Leishmania. The parasite is transmitted during sand fly bites. While probing the skin for a blood meal, vectors salivate into the host's skin. Sand fly saliva contains several components that increase hemorrhage and interfere with the host's inflammatory response. Data obtained in mice originally indicate that immunization against saliva protected from leishmaniasis supporting possibility that leishmaniasis could be prevented by a vaccine based on sand fly saliva. Herein we investigated the nature and the importance of the cellular immune response developed against sand fly saliva by individuals at risk of cutaneous leishmaniasis due to Leishmania major. We demonstrated that the immunity against saliva is dominated by the activation of lymphocytes producing a suppressive cytokine called IL-10. These data may preclude the protective effect of sand fly saliva pre-exposure in humans. Further experiments revealed that the production of IL-10 masked the presence of a second kind of lymphocytes producing IFN-Îł, a rather protective cytokine. The latter finding highlights the importance of the identification of the proteins activating the latter lymphocytes in order to develop vaccines based on selected proteins from the saliva of sand flies
