    A retrospective study on the course and outcome of fetal ventriculomegaly

    AbstractObjectiveTo evaluate the outcomes associated with fetal ventriculomegaly.Materials and methodsReports of women who underwent ultrasound scanning between 18 and 36 weeks of gestation during the period from January 1, 2000, to December 31, 2010, were reviewed. According to the defined severity of ventriculomegaly of affected fetuses, the women were divided into the following groups: (1) mild ventriculomegaly (Group A); (2) moderate ventriculomegaly (Group B); and (3) severe ventriculomegaly (Group C). The women were classified into the “gray zone” group if the fetal lateral ventricle measured between 7 mm and <10 mm. All cases were followed up with additional ultrasound scans. Postnatal information was obtained from the computer database or the medical charts.ResultsA total of 41 cases were recruited for this analysis. Four (9.8%) cases had an abnormal karyotype. Twelve women (29.3%) opted for termination of pregnancy. Of the 29 women who delivered, 56.1% (N = 23) were from Group A, 14.6% (N = 6) were from Group B, and none was from Group C. All children in Group A had normal neurological development. Three children in Group B had normal neurological development, whereas the other three had neurologic deficits. A total of 432 cases were classified into the “gray zone” group. Of these cases, 2.8% (N = 12) progressed to ventriculomegaly.ConclusionCases of isolated and mild ventriculomegaly without additional structural anomalies or chromosomal aberrations had good prognoses. However, the parents of fetuses with moderate or severe ventriculomegaly should be counseled regarding related risks. If the ventricular size of the fetus falls within the “gray zone”, at least one additional exam in the third trimester should be performed, for early detection of ventriculomegaly and other related abnormalities. It is important to make the parents of these fetuses aware of these risks, from a medico-legal point of view

    Protein and microbial determinations on worn contact lenses cleaned conventionally using the Lens2® automatic lens cleaner

    The accumulation of tear film proteins as well as microbes colonization onto worn contact lenses can be eliminated conventionally by mechanical rubbing during the cleaning process. Lens2® functions in rotation manner to loosen the deposits on the contact lens and has antimicrobial coating to keep lenses away from contamination. The objective of this study was to determine the efficiency of Lens2® to remove deposited protein and reduce microbial contamination compared to conventional method. Twenty-eight subjects each wore a pair of contact lens FDA Group 1 (Polymacon, SoftLens® 38, Bausch & Lomb) for one month and c l eaned them us ing mul t ipurpos e solut ion (COMPLETE ® MoisturePLUS TM , Advanced Medical Optics) separately using two different methods. The right lens was cleaned conventionally while the left lens were cleaned using the Lens2® The control group of thirteen subjects each . wore a pair of contact lens for the same period and cleaned both conventionally. These lenses and its cases were then analyzed for protein deposition using Bichinchoninic Acid Assay (BCA) Kit (Sigma, USA) in 96-well plate. Microbial contamination was determined by culturing the samples on nutrient agar for bacteria and fungi and non-nutrient agar for amoeba isolation. The mean of total protein on control lenses (17.014 ± 13.246 µg/mL) was not significantly different from those on the Lens2® (21.623 ± 19.127 µg/mL). There were also low growth numbers of amoeba in each group of samples. Interestingly, there were no growths of amoeba from all Lens2® samples collected. There was also low growth numbers of bacteria in each sample group whereby Lens2® had the lowest growth of bacteria. No growth of fungi was obtained from all samples. The automatic lens cleaner, Lens2® was found to be as efficient as the conventional cleaning method.     A review of teenage pregnancy research in Malaysia

    Objective: To summarise the published research on teenage pregnancy in Malaysia, discuss the impact of the findings on clinical practice, and identify gaps in teenage pregnancy research in Malaysia. Methods: There were 31 articles related to teen pregnancy found after searching a database dedicated to indexing all original clinical research data published in Malaysia from year 2000 to 2014. Twenty-seven articles (including reports from the National Obstetrics Registry) were selected and reviewed on the basis of clinical relevance and future research implications. This literature review has been divided into eight sections: epidemiology, age at first marriage, adolescent fertility rate, unmarried childbearing, risk factors, maternal risks and neonatal outcome, future plan after delivery, and contraceptive use. Results: More than 19,000 births to teenage mothers were recorded each year between 2009 and 2011. Adolescent fertility rates were recorded at 6 births per 1000 women ages 15-19 years in 2013. Many of these births were from unwed pregnancies, which accounted for 1.99% of total deliveries. A majority of young mothers were willing to take care of their baby, although some of them planned to put their baby up for adoption. Risk factors for teenage pregnancy were found to be similar to those published in studies worldwide. Conclusion: More research is needed to better understand the issue of teen pregnancy. For the best results, collaborative studies among nationwide hospitals and institutions should be the way forward

    The Relationship Between Tear Ferning Patterns and Non-invasive Tear Break-up Time in Normal Asian Population

    Purpose: To investigate the relationship between tear ferning patterns (TFP) and non-invasive tear break-up time (NIBUT) in normal Asian subjects. Methods: One hundred and forty-five adults with no ocular surface disorders were recruited. TFP and NIBUT were determined. Tears were collected using a capillary tube and allowed to air dry at room temperature for 10 min. TFP was later observed using a light microscope and classified according to Rolando's classification. Measurement for NIBUT was obtained using a Tearscope with the slit lamp magnification. Results: It was found that there is no significant difference between gender in TFP (Z=−1.77, P>.05) and NIBUT (Z=−1.475, P>.05). There is also no significant difference between Malay, Chinese, Indian, and other races in TFP, (H(3)=4.85, P>.05) and NIBUT (H(3)=2.18, P>.05). However, there is a significant difference between age groups of 20–29, 30–39, 40–49,and 50–60 years old in both TFP (H(3)=28.25, P<.01) and NIBUT (H(3)=36.50, P<.001). Spearman's correlation showed there was a significant relationship between TFP and NIBUT (r=−0.55, P<.001), age and NIBUT (r=−0.50, P<.001), age and TFP (r=0.41, P<.001), McMonnies score and NIBUT (r=−0.40, P<.001), McMonnies score and TFP (r=0.31, P<.001), as well as age and McMonnies score (r=0.52, P<.001). Conclusion: TFP and NIBUT was age dependent but not gender and race dependent. Older subjects had higher grade of TFP and McMonnies questionnaire score but lower NIBUT value. TFP and NIBUT can be used to assess the tear film quality

    Route optimization via RSSI APPs in indoor proxy mobile IPv6 test-bed

    Traditionally IP mobility support mechanisms, Mobile IPv4 or Mobile IPv6 are based on the host-based solution to keep on going sessions despite the movement. The current trend is towards networks-based solutions where mobility support is based on network operation. Proxy Mobile IPv6 (PMIPv6) has been developed by the Internet Engineering Task Force (IETF) as a network-based mobility management protocol. The development is aimed to guarantee and support mobility for IP devices or called as Mobile Node (MN) without the MN involving in any IP layer mobility related to signaling as stated in RFC5213. PMIPv6 in RFC5213 address the needed to enable Localized Routing (LR) but it not specify a complete procedure to establish Route Optimization (RO). Data packets belong to MN need to travel to Local Mobility Anchor (LMA) via bidirectional tunnel between Mobile Access Gateway (MAG) and LMA. This result to long end to end delay. This un-optimized route result in increasing signaling data delay, huge handover latency, large transport cost and many more. Therefore, it important to optimized the data path so that the data destined to MN will be traverse using the shorter path or directly change traffic tunnel between MAG. As for this, the aims of this study are to reduce the handover latency and optimized data route by the illustration of development the PMIPv6 test-bed with implementation of RO via RSSI APPs. Several experiment will be discuss to see the performance of RO algorithm via RSSI APPs with non-optimized RO

    Purpose. An open-label pilot study of virgin coconut oil (VCO) was conducted to determine the safety of the agent as ocular rewetting eye drops on rabbits. Methods. Efficacy of the VCO was assessed by measuring NIBUT, anterior eye assessment, corneal staining, pH, and Schirmer value before instillation and at 30 min, 60 min, and two weeks after instillation. Friedman test was used to analyse any changes in all the measurable variables over the period of time. Results. Only conjunctival redness with instillation of saline agent showed significant difference over the period of time (P<0.05). However, further statistical analysis had shown no significant difference at 30 min, 60 min, and two weeks compared to initial measurement (P>0.05). There were no changes in the NIBUT, limbal redness, palpebral conjunctiva redness, corneal staining, pH, and Schirmer value over the period of time for each agent (P>0.05). Conclusion. VCO acts as safe rewetting eye drops as it has shown no significant difference in the measurable parameter compared to commercial brand eye drops and saline. These study data suggest that VCO is safe to be used as ocular rewetting agent on human being

    Route optimization in proxy mobile IPv6 test-bed via RSSI APPs

    Proxy Mobile IPv6 is the new protocol, but there is a problem on Localized Routing (LR) algorithm which it is not ready build in PMIPv6 protocol. As for this in PMIPv6 in RFC5213 address the needed to enable Localized Routing (LR) but it not specify a complete procedure to establish Route Optimization (RO). RFC6279 state the problem statement on LR issue with several scenarios to tackle down. LR is important especially to minimize data delay and decrease handover latency due to the un-optimized data route. Data packets on PMIPv6 protocol without LR always need to travel to Local Mobility Anchor (LMA) with result in end-to-end delay. Therefore, this paper propose the new LR algorithm to optimized data route for selected scenarios to reduce handover and data packet delay. This paper also will illustrate the setting up of the PMIPv6 and test-bed performance
