    Complementary Therapy with Traditional Chinese Medicine for Treating Atherosclerosis-Related Diseases

    Atherosclerosis-related diseases are the leading cause of morbidity or mortality in the world. They result in serious outcomes such as sudden cardiac death, unstable angina pectoris, acute myocardial infarction, stroke, or intermittent claudication due to vessel obliteration or plaque rupture with subsequent thrombosis. There are some limitations with standard treatments such as antiplatelet drugs, angiotensin-converting enzyme inhibitors, beta-blockers, coronary artery bypass surgery, and percutaneous transluminal coronary angioplasty. Therefore, complementary and alternative medicine is necessary for medication. Traditional Chinese medicine is the main complementary therapy used in the Chinese community. This article aims to explore complementary therapy with traditional Chinese medication for atherosclerosis-related diseases. There is some scientific evidence to support that traditional Chinese medicine could treat atherosclerosis and its associated conditions. Acupuncture through needling on ST36, ST40, PC6, or BL15 could alleviate atherosclerosis-related cardiovascular diseases. Tai chi and meditation have beneficial effects for mental and physical health. In addition, extracts or compounds of single Chinese herbs such as Salvia miltiorrhiza, Panax notoginseng, Ginkgo biloba, Curcuma longa, Crataegus pinnatifida, Paeonia lactiflora, Prunella vulgaris, Polygonum multiflorum, Coptis chinensis, and red yeast rice also could treat atherosclerosis-related diseases through their endothelial protective, antioxidative, anti-inflammatory, inhibiting of smooth muscle cells proliferation, and lipid-lowering effects. In accordance with evidence-based medicine, well-designed and conducted clinical studies such as randomized control clinical trials will be necessary in the future

    Complementary Therapy with Traditional Chinese Medicine for Neonatal Hypoxic Ischemic Encephalopathy

    Hypoxic ischemic encephalopathy (HIE) is one of the most significant causes of morbidity, mortality, and lifelong disability in newborns. The diagnosis of neonatal HIE is based on the dysfunction of neurogenic signs and classification according to the Sarnat staging system, which evaluates conscious level, neuromuscular control, complex reflexes, autonomic function, seizures, electroencephalogram readings, and duration of neurologic sign. There is no standard treatment for neonatal HIE, but it is widely accepted that hypothermia therapy is a safe and effective method for treating neonates with HIE. Traditional Chinese medicine (TCM) has recently been used to treat cases of neonatal HIE, especially herbal medicine prescriptions. Acupuncture is a common method used in TCM and is another promising therapy for neonatal HIE due to its demonstrated effective treatment of the disease in animal models. While there is a lack of direct evidence in clinical practice, we have observed acupuncture to be useful in adult HIE and in animal studies; therefore, we believe a clinical trial designed to evaluate the effectiveness of acupuncture in neonatal HIE treatment is worthwhile. Taken together, TCM is a promising technique that can be integrated into the conventional therapies for neonatal HIE

    This is the online appendix, which includes theoretical and numerical supplements containing some technical details and three additional numerical examples, which could not fit in the main body due to page limits by the journal for a technical note. The abstract for the main body is as follows: In this note, we study a simulation optimization problem of selecting the alternative with the best performance from a finite set, or a so-called ranking and selection problem, in a special low-confidence scenario. The most popular sampling allocation procedures in ranking and selection do not perform well in this scenario, because they all ignore certain induced correlations that significantly affect the probability of correct selection in this scenario. We propose a gradient-based myopic allocation policy (G-MAP) that takes the induced correlations into account, reflecting a trade-off between the induced correlation and the two factors (mean-variance) found in the optimal computing budget allocation formula.     Complementary Therapy with Traditional Chinese Medicine for Childhood Asthma

    Asthma is a heterogeneous disease that is typically characterized by chronic airway inflammation and obstruction of airflow; it frequently presents in early childhood and is the leading chronic disease in children in the western world. This review presents a brief description of the pathophysiology of asthma and summarizes recent research results on the mechanisms of action of anti-asthma Chinese herbal medicine commonly used in clinical practice. Other interventions of traditional Chinese medicine (TCM), such as acupuncture, tai chi, and meditation are also briefly discussed. We believe that this contribution is theoretically and practically relevant because the prevalence of asthma is increasing and, in addition to standard treatment, the use of complementary therapy is increasing and there is increasing scientific evidence demonstrating that TCM has potential for the treatment of childhood asthma

    Superior Mesenteric Artery Syndrome: A Single-institution Experience

    Background:Superior mesenteric artery syndrome (SMAS) is a rare disease in adult. SMAS is characterized by acute, or, more commonly, chronic nonspecific symptoms due to duodenal obstruction and severe malnutrition with reduced arterio-mesenteric angle and distance. Surgical treatment may be necessary in most cases with chronic symptoms or when conservative treatment fails in SMAS.Methods:A retrospective chart review was performed on patients who underwent operation for SMAS from January 2008 to August 2020 in Cardinal Tien Hospital. Patients’ clinical presentations, surgical intervention, and outcomes.Results:Data from a total of 14 patients diagnosed with SMAS were analyzed, of which seven were diagnosed with SMAS by abdominal computed tomography and upper gastrointestinal series with water-soluble barium contrast. Six of the confirmed cases underwent surgery, namely, gastric decompression using a nasogastric tube, andcorrection of electrolyte imbalance. The nasoduodenal tube was placed through the obstructed duodenum to provide a high-nutrient fluid supplement. After conservative treatment failure, the patients underwent surgery. Of the six patients, four underwent duodenojejunostomy, one underwent a mini-laparotomy duodenojejunostomy bypass, and the last one underwent Roux-en-Y duodenojejunal bypass with duodenal feeding tube insertion.Conclusion:Patients with SMAS should initially be treated conservative. Surgical intervention should be considered in patients in whom conservative treatments were not effective.Complete resolution of all symptoms may not always be guaranteed after surgical intervention. Laparoscopy is currently widely used. In well-selected patients, minimally invasive or mini-laparotomy duodenojejunostomy is a safe and effective treatment for SMAS. The main advantages of mini-laparotomy duodenojejunostomy over other surgical approaches include half-length surgical incision and a shorter operative time. Duodenojejunostomy is rapidly becoming the standard procedure of this condition, and it has excellent outcomes comparable with those of open surgery

    Logos are one of the most important graphic design forms that use an abstracted shape to clearly represent the spirit of a community. Among various styles of abstraction, a particular golden-ratio design is frequently employed by designers to create a concise and regular logo. In this context, designers utilize a set of circular arcs with golden ratios (i.e., all arcs are taken from circles whose radii form a geometric series based on the golden ratio) as the design elements to manually approximate a target shape. This error-prone process requires a large amount of time and effort, posing a significant challenge for design space exploration. In this work, we present a novel computational framework that can automatically generate golden ratio logo abstractions from an input image. Our framework is based on a set of carefully identified design principles and a constrained optimization formulation respecting these principles. We also propose a progressive approach that can efficiently solve the optimization problem, resulting in a sequence of abstractions that approximate the input at decreasing levels of detail. We evaluate our work by testing on images with different formats including real photos, clip arts, and line drawings. We also extensively validate the key components and compare our results with manual results by designers to demonstrate the effectiveness of our framework. Moreover, our framework can largely benefit design space exploration via easy specification of design parameters such as abstraction levels, golden circle sizes, etc

    Rapid Sensing of Hidden Objects and Defects using a Single-Pixel Diffractive Terahertz Processor

    Terahertz waves offer numerous advantages for the nondestructive detection of hidden objects/defects in materials, as they can penetrate through most optically-opaque materials. However, existing terahertz inspection systems are restricted in their throughput and accuracy (especially for detecting small features) due to their limited speed and resolution. Furthermore, machine vision-based continuous sensing systems that use large-pixel-count imaging are generally bottlenecked due to their digital storage, data transmission and image processing requirements. Here, we report a diffractive processor that rapidly detects hidden defects/objects within a target sample using a single-pixel spectroscopic terahertz detector, without scanning the sample or forming/processing its image. This terahertz processor consists of passive diffractive layers that are optimized using deep learning to modify the spectrum of the terahertz radiation according to the absence/presence of hidden structures or defects. After its fabrication, the resulting diffractive processor all-optically probes the structural information of the sample volume and outputs a spectrum that directly indicates the presence or absence of hidden structures, not visible from outside. As a proof-of-concept, we trained a diffractive terahertz processor to sense hidden defects (including subwavelength features) inside test samples, and evaluated its performance by analyzing the detection sensitivity as a function of the size and position of the unknown defects. We validated its feasibility using a single-pixel terahertz time-domain spectroscopy setup and 3D-printed diffractive layers, successfully detecting hidden defects using pulsed terahertz illumination. This technique will be valuable for various applications, e.g., security screening, biomedical sensing, quality control, anti-counterfeiting measures and cultural heritage protection.Comment: 23 Pages, 5 Figure
