Maternal methadone use in pregnancy : factors associated with the development of neonatal abstinence syndrome and implications for healthcare resources
The objectives of this study were to investigate factors associated with the development of neonatal abstinence syndrome (NAS) and to assess the implications for healthcare resources of infants born to drug-misusing women. Design. Retrospective cohort study from 1 January 2004 to 31 December 2006. Setting. Inner-city maternity hospital providing dedicated multidisciplinary care to drug-misusing women. Four hundred and fifty singleton pregnancies of drugmisusing women prescribed substitute methadone in pregnancy. Development of NAS and duration of infant hospital stay. 45.5% of infants developed NAS requiring pharmacological treatment. The odds ratio of the infant developing NAS was independently related to prescribed maternal methadone dose rather than associated polydrug misuse. Breastfeeding was associated with reduced odds of requiring treatment for NAS (OR 0.55, 95% CI 0.34-0.88). Preterm birth did not influence the odds of the infant receiving treatment for NAS. 48.4% infants were admitted to the neonatal unit (NNU) 40% of these primarily for treatment of NAS. The median total hospital stay for all infants was 10 days (interquartile range 7-17 days). Infants born to methadone-prescribed drug-misusing mothers represented 2.9% of hospital births, but used 18.2% of NNU cot days. Higher maternal methadone dose is associated with a higher incidence of NAS. Pregnant drug-misusing women should be encouraged and supported to breastfeed. Their infants are extremely vulnerable and draw heavily on healthcare resources
An investigation into the combined effects of β-amyloid toxicity and cerebral ischemia on the pathological expression of gangliosides.
Identifying mechanisms underlying the synergistic pathological interaction between stroke and Alzheimerâs disease (AD) can effectively guide future therapeutic strategies for these highly co-morbid conditions. Aberrant ganglioside expression marked by the pathological accumulation of ganglioside GM3 is common to stroke and AD, yet it is unclear whether GM3 is synergistically enhanced in a comorbid model, or if GM3 is a viable therapeutic target. Adult male Wistar rats received a unilateral ischemic striatal infarct via endothelin-1 (ET-1) injection alone or in combination with bilateral intracerebroventricular injection of the β-Amyloid 25-35 peptide (Aβ) to induce generalized Aβ toxicity (Aβ/ET-1). Animals were sacrificed after 3 or 21 days and assessed via immunohistochemistry for neuroinflammation, neurodegeneration, and ganglioside expression. Other animals were also treated with chloroquine (CQ) systemically for seven days beginning 3 days before surgery as a novel therapeutic approach to prevent the accumulation of GM3 by inhibiting degradation of complex gangliosides. Our results showed that Aβ/ET-1 synergistically enhanced GM3 accumulation with concomitant reductions in complex gangliosides and neuronal survival at 21 days. CQ reduced inflammation and GM3 accumulation at 21 days, and improved neuronal survival. These findings suggest GM3 accumulation is a point of synergism between stroke and Aβ toxicity, and CQ may be a viable therapeutic approach for stroke

We find substantial mixing of the connected and disconnected contributions so
that the lattice result that the disconnected contribution to the nucleon is
large does not imply that the content is large. This has

What are the strategies, modalities and aspirations of island-based, stateless nationalist and regionalist parties in the twenty-first century? Political independence is now easier to achieve, even by the smallest of territories; yet, it is not so likely to be pursued with any vigour by the world's various persisting sub-national (and mainly island) jurisdictions. Theirs is a pursuit of different expressions of sub-national autonomy, stopping short of independence. And yet, a number of independence referenda are scheduled, including one looming in Scotland in autumn 2014
Accurate Reaction-Diffusion Operator Splitting on Tetrahedral Meshes for Parallel Stochastic Molecular Simulations
Spatial stochastic molecular simulations in biology are limited by the
intense computation required to track molecules in space either in a discrete
time or discrete space framework, meaning that the serial limit has already
been reached in sub-cellular models. This calls for parallel simulations that
can take advantage of the power of modern supercomputers; however exact methods
are known to be inherently serial. We introduce an operator splitting
implementation for irregular grids with a novel method to improve accuracy, and
demonstrate potential for scalable parallel simulations in an initial MPI
version. We foresee that this groundwork will enable larger scale, whole-cell
