    Massive Intravascular Hemolysis in a Patient Infected by a Clostridium perfringens

    Clostridium perfringens infection is a very rare cause of massive intravascular hemolysis, but it should always be kept in mind, since only early treatment can rescue patients from an otherwise rapidly fatal outcome. We report a case of a 78-year-old diabetic male who was admitted complaining of general fatigue, dark red urine, and vomiting. His blood revealed massive hemolysis. Computer tomography demonstrated huge liver abscess in the right lobe of the liver. About 1 h after admission, he suddenly fell into a critical condition. He died 3 h after admission in spite of intensive care and resuscitation. Clostridium perfringens was detected from the blood taken before death and from liver abscess by biopsy after death. We concluded that this patient died of acute massive intravascular hemolysis in septicemia caused by Clostridium perfringens infection

    Evidence for late glacial oceanic carbon redistribution and discharge from the Pacific Southern Ocean

    Southern Ocean deep-water circulation plays an important role in the global carbon cycle. On geological time-scales, upwelling along the Chilean continental margin likely contributed to the deglacial atmospheric carbon dioxide rise, but little quantitative evidence exists of carbon storage. Here, we use a new X-ray Micro-Computer-Tomography method to assess foraminiferal test dissolution as proxy for paleo-carbonate ion concentrations [CO3^2−]. Our subantarctic Southeast Pacific sediment core depth transect shows significant deep-water [CO3^2−] variations during the Last Glacial Maximum and Deglaciation (10 – 22 ka BP). We provide evidence for an increase in [CO3^2−] during the early deglacial period (15-19 ka BP), followed by a ca. 40 µmol kg^-1 reduction in Lower Circumpolar Deepwater (CDW). This decreased Pacific to Atlantic export of low-carbon CDW contributed to significantly lowered carbon storage within the Southern Ocean, highlighting the importance of a dynamic Pacific–Southern Ocean deep-water reconfiguration for shaping late-glacial oceanic carbon storage, and subsequent deglacial oceanic-atmospheric CO2 transfer

    Do Strong Winds Impact Water Mass, Nutrient, and Phytoplankton Distributions in the Ice‐Free Canada Basin in the Fall?

    In general, strong wind events can enhance ocean turbulent mixing, followed by episodic nutrient supply to the euphotic zone and phytoplankton blooms. However, it is unclear whether such responses to strong winds occur in the ice‐free Canada Basin, where the seasonal pycnocline is strong and the nutricline is deep. In the present study, we monitored a fixed‐point observation (FPO) station in the Canada Basin for about 3 weeks in the fall of 2014 to examine the oceanic and biological responses to strong winds. At the FPO site, oceanic microstructure measurements, hydrographic surveys, and water sampling were performed with high temporal resolution, recording internal wave propagation, eddy passage, and water mass changes. Strong winds and internal wave propagation significantly enhanced the mixing above and at the seasonal pycnocline, but their effects were diminished at the nutricline, which was much deeper than the seasonal pycnocline. Therefore, wind‐induced mixing did not increase the upward nutrient supply from the nutricline and did not impact phytoplankton (chlorophyll a) distribution in the surface layer of the FPO site. The temporal evolution of the chlorophyll a concentration was most closely related to water mass changes. We also observed prominent subsurface chlorophyll a maxima with abundant large‐sized phytoplankton that were likely carried by warm‐core eddies to the FPO site. Phytoplankton biomass may have been sustained by the high concentration of ammonium within the eddy and ammonium regeneration at the seasonal pycnocline, where particulate organic matter likely accumulated

    Regional changes of fractional anisotropy with normal aging using Statistical Parametric Mapping (SPM)

    Objective :There has been reports on the usefulness of diffusion tensor imaging (DTI) about age-or disease-related degradation. DTI is generally evaluated by the region of interest (ROI) methodology. In this study, we applied a statistical way using Statistical Parametric Mapping (SPM) to assess normal aging by DTI and compared results of these two methods. Methods : Ten young and ten senior normal volunteers were examined. On SPM, tensor images were changed into normalized tensor images. They were compared between the two groups by t-test. Results : In the senior group, fractional anisotropy (FA) values were higher on the basal ganglia, cingulated gyrus and other cortical gray matter, lower in the corona radiata, internal capsule, centrum semiovale and corpus callosum by using SPM. In the ROI method, the results were almost compatible except in the brain periphery. Conclusions : Aging changes on water diffusion anisotropy was clearly shown by SPM method which would be useful to evaluate change of water diffusion anisotropy without operator bias even in clinical setting instead of ROI measurement

    Eukaryotic Components Remodeled Chloroplast Nucleoid Organization during the Green Plant Evolution.

    Chloroplast (cp) DNA is thought to originate from the ancestral endosymbiont genome and is compacted to form nucleoprotein complexes, cp nucleoids. The structure of cp nucleoids is ubiquitously observed in diverse plants from unicellular algae to flowering plants and is believed to be a multifunctional platform for various processes, including cpDNA replication, repair/recombination, transcription, and inheritance. Despite its fundamental functions, the protein composition for cp nucleoids in flowering plants was suggested to be divergent from those of bacteria and algae, but the evolutionary process remains elusive. In this research, we aimed to reveal the evolutionary history of cp nucleoid organization by analyzing the key organisms representing the three evolutionary stages of eukaryotic phototrophs: the chlorophyte alga Chlamydomonas reinhardtii , the charophyte alga Klebsormidium flaccidum , and the most basal land plant Marchantia polymorpha . To clarify the core cp nucleoid proteins in C. reinhardtii , we performed an LC-MS/MS analysis using highly purified cp nucleoid fractions and identified a novel SAP domain-containing protein with a eukaryotic origin as a constitutive core component. Then, homologous genes for cp nucleoid proteins were searched for in C. reinhardtii , K. flaccidum , and M. polymorpha using the genome databases, and their intracellular localizations and DNA binding activities were investigated by cell biological/biochemical analyses. Based on these results, we propose a model that recurrent modification of cp nucleoid organization by eukaryotic factors originally related to chromatin organization might have been the driving force for the diversification of cp nucleoids since the early stage of green plant evolution

    HLA-Haploidentical Peripheral Blood Stem Cell Transplantation with Post-Transplant Cyclophosphamide after Busulfan-Containing Reduced-Intensity Conditioning

    AbstractAllogeneic hematopoietic stem cell transplantation (allo-SCT) using post-transplant cyclophosphamide (PTCy) is increasingly performed. We conducted a multicenter phase II study to evaluate the safety and efficacy of PTCy-based HLA-haploidentical peripheral blood stem cell transplantation (PTCy-haploPBSCT) after busulfan-containing reduced-intensity conditioning. Thirty-one patients were enrolled; 61% patients were not in remission and 42% patients had a history of prior allo-SCT. Neutrophil engraftment was achieved in 87% patients with a median of 19 days. The cumulative incidence of grades II to IV and III to IV acute graft-versus-host disease (GVHD) and chronic GVHD at 1 year were 23%, 3%, and 15%, respectively. No patients developed severe chronic GVHD. Day 100 nonrelapse mortality (NRM) rate was 19.4%. Overall survival, relapse, and disease-free survival rates were 45%, 45%, and 34%, respectively, at 1 year. Subgroup analysis showed that patients who had a history of prior allo-SCT had lower engraftment, higher NRM, and lower overall survival than those not receiving a prior allo-SCT. Our results suggest that PTCy-haploPBSCT after busulfan-containing reduced-intensity conditioning achieved low incidences of acute and chronic GVHD and NRM and stable donor engraftment and low NRM, particularly in patients without a history of prior allo-SCT

    A case of von Recklinghausen’s disease with coincident malignant peripheral nerve sheath tumor and gastrointestinal stromal tumor

    18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET/CT) was performed to evaluate a left femoral subcutaneous mass in a patient with von Recklinghausen’s disease (vRd) that gradually enlarged, causing pain and numbness. The left femoral mass showed intense FDG uptake with the maximum standardized uptake value (SUVmax) of 9.0. Other subcutaneous masses considered benign and neurogenic in nature also showed FDG uptake (SUVmax around 3 or less), but the degree of FDG uptake differed considerably from the left femoral mass. This observation suggested that the degree of FDG uptake may be a useful indicator of malignant transformation. Incidentally, PET/CT also showed an asymptomatic large abdominal mass with intense FDG uptake (SUVmax 8.8). The abdominal mass was resected and confirmed as gastrointestinal stromal tumor (GIST) of the small intestine. Three months later, the left femoral mass was operated on and pathologically diagnosed as a malignant peripheral nerve sheath tumor (MPNST). Various malignant diseases are known to occur with high frequency in vRd. Therefore, vRd patients need to have periodical examinations including PET/CT. We present a rare case of a patient with vRd with a MPNST of the left femur and coincidental GIST of the small intestine

    Temporal characteristics of facial ensemble in individuals with autism spectrum disorder: examination from arousal and attentional allocation

    IntroductionIndividuals with Autism Spectrum Disorder (ASD) show atypical recognition of facial emotions, which has been suggested to stem from arousal and attention allocation. Recent studies have focused on the ability to perceive an average expression from multiple spatially different expressions. This study investigated the effect of autistic traits on temporal ensemble, that is, the perception of the average expression from multiple changing expressions.MethodsWe conducted a simplified temporal-ensemble task and analyzed behavioral responses, pupil size, and viewing times for eyes of a face. Participants with and without diagnosis of ASD viewed serial presentations of facial expressions that randomly switched between emotional and neutral. The temporal ratio of the emotional expressions was manipulated. The participants estimated the intensity of the facial emotions for the overall presentation.ResultsWe obtained three major results: (a) many participants with ASD were less susceptible to the ratio of anger expression for temporal ensembles, (b) they produced significantly greater pupil size for angry expressions (within-participants comparison) and smaller pupil size for sad expressions (between-groups comparison), and (c) pupil size and viewing time to eyes were not correlated with the temporal ensemble.DiscussionThese results suggest atypical temporal integration of anger expression and arousal characteristics in individuals with ASD; however, the atypical integration is not fully explained by arousal or attentional allocation
