Study of the heart muscle in health and disease

This thesis does not purport to bring to light any startling new theories or indeed any new matter at all. It endeavours to examine all heart conditions from the standpoint of the heart muscle and it aims at emphasising the great importance of the myocardium in cardiac disease. Modern research has shown conclusively that whatever kind of heart-lesion may be present, the real factor of importance is the state of the heart muscle. A healthy myocardium can compensate for every defect. The actual condition of the heart muscle is the one guide to treatment and the only sure index in prognosis

Structures in Real Theory Application: A Study in Feasible Epistemology

This thesis considers the following problem: What methods should the epistemology of science use to gain insight into the structure and behaviour of scientific knowledge and method in actual scientific practice? After arguing that the elucidation of epistemological and methodological phenomena in science requires a method that is rooted in formal methods, I consider two alternative methods for epistemology of science. One approach is the classical approaches of the syntactic and semantic views of theories. I show that typical approaches of this sort are inadequate and inaccurate in their representation of scientific knowledge by showing how they fail to account for and misrepresent important epistemological structure and behaviour in science. The other method for epistemology of science I consider is modeled on the methods used to construct valid models of natural phenomena in applied mathematics. This new epistemological method is itself a modeling method that is developed through the detailed consideration of two main examples of theory application in science: double pendulum systems and the modeling of near-Earth objects to compute probability of future Earth impact. I show that not only does this new method accurately represent actual methods used to apply theories in applied mathematics, it also reveals interesting structural and behavioural patterns in the application process and gives insight into what underlies the stability of methods of application. I therefore conclude that for epistemology of science to develop fully as a scientific discipline it must use methods from applied mathematics, not only methods from pure mathematics and metamathematics as traditional formal epistemology of science has done

Knowing the wheat from the weeds in noisy speech

This paper introduces a word boundary detection algorithm that works in a variety of noise conditions including what is commonly called the 'cocktail party' situation. The algorithm uses the direction of the signal as the main criterion for differentiating between desired-speech and background noise. To determine the signal direction the algorithm calculates estimates of the time delay between signals received at two microphones. These time delay estimates together with estimates of the coherence function and signal energy are used to locate word boundaries. The algorithm was tested using speech embedded in different types and levels of noise including car noise, factory noise, babble noise, and competing talkers. The test results showed that the algorithm performs very well under adverse conditions and with SNR down to -14.5dB

Diversity indices applied in desert grassland communities of Otero Mesa, New Mexico

To describe plant community (alpha) diversity on rangelands, managers are confronted with a variety of commonly used indices. The choice, performance, and interpretations of these indices are often not clear. Biodiversity indices were computed for a variety of plant communities in a desert grassland of southern New Mexico. Data consisted of reported importance values, range transect data for both grazed and ungrazed pastures, and search-and-find data specifically addressed to plant community diversity. Occurrence of threatened and endangered plants was considered by a weighting procedure. Performance of each diversity index was evaluated by ranking plant communities from low to high and comparing the rankings yielded by the various indices. Data based upon importance or dominance that omit plant species of lesser importance or dominance should not be the basis of comparisons for alpha diversity. Communities described by range transect data ranked differently depending upon the index used. The most practical measure of plant species diversity may be the number of species found by search-and-find procedures

On the Importance of Registers for Computability

All consensus hierarchies in the literature assume that we have, in addition to copies of a given object, an unbounded number of registers. But why do we really need these registers? This paper considers what would happen if one attempts to solve consensus using various objects but without any registers. We show that under a reasonable assumption, objects like queues and stacks cannot emulate the missing registers. We also show that, perhaps surprisingly, initialization, shown to have no computational consequences when registers are readily available, is crucial in determining the synchronization power of objects when no registers are allowed. Finally, we show that without registers, the number of available objects affects the level of consensus that can be solved. Our work thus raises the question of whether consensus hierarchies which assume an unbounded number of registers truly capture synchronization power, and begins a line of research aimed at better understanding the interaction between read-write memory and the powerful synchronization operations available on modern architectures.Comment: 12 pages, 0 figure

Relaxation and exercise in lymphoma survivors (REIL study): a randomised clinical trial protocol.

Background: Lymphoma survivors commonly report ongoing complaints including fatigue, pain, depression and decreased quality of life (QoL) following treatment. Although evidence suggests that both relaxation and exercise can significantly improve such symptoms, there is no consensus on which intervention is more effective. This paper presents the REIL (Relaxation and Exercise In Lymphoma) Study protocol. The REIL study aims to compare the effect of two home-based interventions - relaxation and exercise - on QoL in lymphoma survivors. Methods: Eligible participants (n = 36) will be randomised to a relaxation or exercise programme to perform at least three times per week. The primary outcome measure is QoL, assessed by the European Organisation for Research and Treatment of Cancer QoL Questionnaire Core 30 (EORTC QLQ-C30). Secondary outcome measures include body composition, cardiovascular status, pulmonary function, grip strength, functional exercise capacity (six minute walk test), well-being assessed by the FACT-Lym questionnaire, and psychological status assessed by the Hospital Anxiety and Depression Scale. Total duration of the study will be twelve weeks and outcome measures will be assessed at baseline, six weeks and at the end of the study. Discussion: It is anticipated that results from this preliminary study will begin to highlight effective pathways to improve QoL following chemotherapy for this population. This will better inform healthcare professionals to optimise QoL of lymphoma patients, and enable a smooth transition from being a cancer patient to survivor. In January 1985 The Journal of Chinese Medicine published an edited transcript of a discussion called ‘Acupuncture in the West’ between Peter Deadman, Ted Kaptchuk, Giovanni Maciocia and Felicity Moir. It was a particularly popular article and to celebrate the 30th anniversary of the JCM we decided to convene another discussion, in London in March 2009. Present this time were Peter Deadman (PD), Hugh MacPherson (HM), Daniel Maxwell (DM), Felicity Moir (FM) and Volker Scheid (VS).
