Ten Economic Facts About Crime and Incarceration in the United States

    Get PDF
Crime and high rates of incarceration impose tremendous costs on society, with lasting negative effects on individuals, families, and communities. Rates of crime in the United States have been falling steadily, but still constitute a serious economic and social challenge. At the same time, the incarceration rate in the United States is so high -- more than 700 out of every 100,000 people are incarcerated -- that both crime scholars and policymakers alike question whether, for nonviolent criminals in particular, the social costs of incarceration exceed the social benefits

Information provision in cervical screening in Australia

    Get PDF
    • The National Cervical Screening Program, and associated state and territory organisations, are responsible for promoting cervical screening. Communication via multiple media channels encourages women to be screened. • Some communications are not clear about misrepresent the risk of cervical cancer and the protective capacity and reliability of the Pap smear. The potential harms of screening are rarely presented. • Participation is a key performance indicator for the Program. • Women receive Pap tests from clinicians, mostly general practitioners (GPs) who often screen opportunistically during already-busy appointments. Incentive Payments encourage high screening rates. • Consent is an important ethical principle in the delivery of all health care. Provision of material information is one of the elements of valid consent. • The combination of arguably ambiguous misleading communications, screening participation targets, and opportunistic testing under time pressure seems likely to undermine opportunities for women participating in the cervical cancer screening program to be informed. • Of particular concern are women who are less likely to benefit, women who are more likely to experience harm, and some groups of disadvantaged women. • Improved communications could include the absolute risk of cervical cancer and the morbidity and mortality benefits and harms of screening. Screening programs internationally have begun providing such information. • Performance indicators could value evidence of discussion or informing. • Areas for further research include the appropriate roles of the program, screeners, and individuals in providing and seeking information. Such work would identify the optimum method for informing women in the screening process

‘Organised’ cervical screening 45 years on: How consistent are organised screening practices?

Organised screening programmes have been remarkably successful in reducing incidence and mortality from cervical cancer, while opportunistic screening varies in its effectiveness. Experts recommend that cervical screening or HPV testing be carried out only in the context of an organised programme. We sought to answer the following study questions: What does it mean for a cervical screening programme to be organised? Is there a place for opportunistic screening (in an organised programme)? We reviewed 154 peer-reviewed papers on organised and opportunistic approaches to cervical screening published between 1970 and 2014 to understand how the term ‘organised’ is used, formally and in practice. We found that despite broad recognition of a prescriptive definition of organisation, in practice the meaning of organisation is much less clear. Our review revealed descriptions of organised programmes that differ significantly from prescribed norms and from each other, and a variety of ways that opportunistic and organised programmes intersect. We describe the breadth of the variation in cervical cancer screening programmes and examine the relationships and overlaps between organised and opportunistic screening. Implications emerging from the review include the need to better understand the breadth of organisation in practice, the drivers and impacts of opportunistic screening and the impact of opportunistic screening on population programme outcomes. Appreciation of the complexity of cervical screening programmes will benefit both screeners and women as programmes are changed to reflect a partially vaccinated population, new evidence and new technologies

Quantification of Ion Migration in CH3NH3PbI3 Perovskite Solar Cells by Transient Capacitance Measurements

Solar cells based on organic-inorganic metal halide perovskites show efficiencies close to highly-optimized silicon solar cells. However, ion migration in the perovskite films leads to device degradation and impedes large scale commercial applications. We use transient ion-drift measurements to quantify activation energy, diffusion coefficient, and concentration of mobile ions in methylammonium lead triiodide (MAPbI3) perovskite solar cells, and find that their properties change close to the tetragonal-to-orthorhombic phase transition temperature. We identify three migrating ion species which we attribute to the migration of iodide (I-) and methylammonium (MA+). We find that the concentration of mobile MA+ ions is one order of magnitude higher than the one of mobile I- ions, and that the diffusion coefficient of mobile MA+ ions is three orders of magnitude lower than the one for mobile I- ions. We furthermore observe that the activation energy of mobile I- ions (0.29 eV) is highly reproducible for different devices, while the activation energy of mobile MA+ depends strongly on device fabrication. This quantification of mobile ions in MAPbI3 will lead to a better understanding of ion migration and its role in operation and degradation of perovskite solar cells

GROWTH CHARACTERISTICS OF NEWLY ISOLATED INDONESIAN MICROALGAE UNDER DIFFERENT SALINITY

The aim of this study was to investigate the growth characteristics of microalgae strains isolated from Kendari Bay and the Wanggu River estuary, Indonesia. The growth of the isolates, denoted as Kb1-2, Kb1-3, Kb1-5, and Kb2-6, were evaluated under controlled conditions. A batch culture experiment of these strains except Kb2-6 was conducted for 15 days under salinity levels of 20, 25, 30 and 35 gL-1. Tetraselmis chui, Tisochrysis lutea and Chaetocero sneogracile were also culture and used as the growth references. Cell density was measured every day and cell size was measured from 50 live cells during the logarithmic phase. The cell sizes of three of the four Indonesian microalgae ranged from 1.2-11.8 µm, considered suitable for shrimp larvae. The Indonesian strains started the logarithmic phase of growth at all salinities tested from day 0 to day 3 after inoculation except for Kb1-3 that started the phase after a 3-day lag. Increased cell density over the culture period and division rate of Indonesian microalgae during the logarithmic phase of growth were similar at all salinities tested and similar to T. chui, Ti. lutea and C. neogracile. However, the final biomasses after 15 days of culture of all microalgal strains were affected by culture salinities tested. Indonesian microalgal strains showed similar dry weight and ash free dry weight to smaller-cell strains, Ti. lutea and C. neogracile. Indonesian strains (other than Kb2-6) are suggested as suitable live food candidates for mass culture in shrimp hatcheries based on their cell size, ability to survive long culture periods, and wide salinity tolerance

Randomized double-blind placebo-controlled trial of 40 mg/day of atorvastatin in reducing the severity of sepsis in ward patients (ASEPSIS Trial)

    Get PDF
Introduction: Several observational studies suggest that statins modulate the pathophysiology of sepsis and may prevent its progression. The aim of this study was to determine if the acute administration of atorvastatin reduces sepsis progression in statin naïve patients hospitalized with sepsis. Methods: A single centre phase II randomized double-blind placebo-controlled trial. Patients with sepsis were randomized to atorvastatin 40 mg daily or placebo for the duration of their hospital stay up to a maximum of 28-days. The primary end-point was the rate of sepsis progressing to severe sepsis during hospitalization. Results: 100 patients were randomized, 49 to the treatment with atorvastatin and 51 to placebo. Patients in the atorvastatin group had a significantly lower conversion rate to severe sepsis compared to placebo (4% vs. 24% p = 0.007.), with a number needed to treat of 5. No significant difference in length of hospital stay, critical care unit admissions, 28-day and 12-month readmissions or mortality was observed. Plasma cholesterol and albumin creatinine ratios were significantly lower at day 4 in the atorvastatin group (p < 0.0001 and p = 0.049 respectively). No difference in adverse events between the two groups was observed (p = 0.238). Conclusions: Acute administration of atorvastatin in patients with sepsis may prevent sepsis progression. Further multi-centre trials are required to verify these findings. Trial Registration: International Standard Randomized Control Trial Registry ISRCTN64637517
