Evidence regarding clinical use of microvolt T-wave alternans [Accuracy of microvolt T-wave alternans testing]
Background: Microvolt T-wave alternans (MTWA) testing in many studies has proven to be a highly accurate predictor of ventricular tachyarrhythmic events (VTEs) in patients with risk factors for sudden cardiac death (SCD) but without a prior history of sustained VTEs (primary prevention patients). In some recent studies involving primary prevention patients with prophylactically implanted cardioverter-defibrillators (ICDs), MTWA has not performed as well.
Objective: This study examined the hypothesis that MTWA is an accurate predictor of VTEs in primary prevention patients without implanted ICDs, but not of appropriate ICD therapy in such patients with implanted ICDs.
Methods: This study identified prospective clinical trials evaluating MTWA measured using the spectral analytic method in primary prevention populations and analyzed studies in which: (1) few patients had implanted ICDs and as a result none or a small fraction (≤15%) of the reported end point VTEs were appropriate ICD therapies (low ICD group), or (2) many of the patients had implanted ICDs and the majority of the reported end point VTEs were appropriate ICD therapies (high ICD group).
Results: In the low ICD group comprising 3,682 patients, the hazard ratio associated with a nonnegative versus negative MTWA test was 13.6 (95% confidence interval [CI] 8.5 to 30.4) and the annual event rate among the MTWA-negative patients was 0.3% (95% CI: 0.1% to 0.5%). In contrast, in the high ICD group comprising 2,234 patients, the hazard ratio was only 1.6 (95% CI: 1.2 to 2.1) and the annual event rate among the MTWA-negative patients was elevated to 5.4% (95% CI: 4.1% to 6.7%). In support of these findings, we analyzed published data from the Multicenter Automatic Defibrillator Trial II (MADIT II) and Sudden Cardiac Death in Heart Failure Trial (SCD-HeFT) trials and determined that in those trials only 32% of patients who received appropriate ICD therapy averted an SCD.
Conclusion: This study found that MTWA testing using the spectral analytic method provides an accurate means of predicting VTEs in primary prevention patients without implanted ICDs; in particular, the event rate is very low among such patients with a negative MTWA test. In prospective trials of ICD therapy, the number of patients receiving appropriate ICD therapy greatly exceeds the number of patients who avert SCD as a result of ICD therapy. In trials involving patients with implanted ICDs, these excess appropriate ICD therapies seem to distribute randomly between MTWA-negative and MTWA-nonnegative patients, obscuring the predictive accuracy of MTWA for SCD. The chemical structure is given of a film formed by plasma polymerization from pyridine monomers. The film has a hydrophilic chemical structure, its molecular weight is 900, and the molecular system is C55H50N10O3. The electrical characteristics of a plasma polymerized film are described. The film has good insulating properties and was successfully applied as video disc coating. Etching resistance properties make it possible to use the film as a resist in etching. The characteristics of plasma polymer formed from monomers containing tetramethyltin are discussed. The polymer is in film form, displays good adhesiveness, is similar to UV film UV 35 in light absorption and is highly insulating
Low Mach number effect in simulation of high Mach number flow
In this note, we relate the two well-known difficulties of Godunov schemes:
the carbuncle phenomena in simulating high Mach number flow, and the inaccurate
pressure profile in simulating low Mach number flow. We introduced two simple
low-Mach-number modifications for the classical Roe flux to decrease the
difference between the acoustic and advection contributions of the numerical
dissipation. While the first modification increases the local numerical
dissipation, the second decreases it. The numerical tests on the double-Mach
reflection problem show that both modifications eliminate the kinked Mach stem
suffered by the original flux. These results suggest that, other than
insufficient numerical dissipation near the shock front, the carbuncle
phenomena is strongly relevant to the non-comparable acoustic and advection
contributions of the numerical dissipation produced by Godunov schemes due to

XANES study of rare-earth valency in LRu4P12 (L = Ce and Pr)
Valency of Ce and Pr in LRu4P12 (L = Ce and Pr) was studied by L2,3-edge
x-ray absorption near-edge structure (XANES) spectroscopy. The Ce-L3 XANES
spectrum suggests that Ce is mainly trivalent, but the 4f state strongly
hybridizes with ligand orbitals. The band gap of CeRu4P12 seems to be formed by
strong hybridization of 4f electrons. Pr-L2 XANES spectra indicate that Pr
exists in trivalent state over a wide range in temperature, 20 < T < 300 K. We
find that the metal-insulator (MI) transition at TMI = 60 K in PrRu4P12 does
not originate from Pr valence fluctuation.Comment: 4 page
Patterning of dielectric nanoparticles using dielectrophoretic forces generated by ferroelectric polydomain films
A theoretical study of a dielectrophoretic force, i.e. the force acting on an
electrically neutral particle in the inhomogeneous electric field, which is
produced by a ferroelectric domain pattern, is presented. It has been shown by
several researchers that artificially prepared domain patterns with given
geometry in ferroelectric single crystals represent an easy and flexible method
for patterning dielectric nanoobjects using dielectrophoretic forces. The
source of the dielectrophoretic force is a strong and highly inhomogeneous
(stray) electric field, which exists in the vicinity of the ferroelectric
domain walls at the surface of the ferroelectric film. We analyzed
dielectrophoretic forces in the model of a ferroelectric film of a given
thickness with a lamellar 180 domain pattern. The analytical formula
for the spatial distribution of the stray field in the ionic liquid above the
top surface of the film is calculated including the effect of free charge
screening. The spatial distribution of the dielectrophoretic force produced by
the domain pattern is presented. The numerical simulations indicate that the
intersection of the ferroelectric domain wall and the surface of the
ferroelectric film represents a trap for dielectric nanoparticles in the case
of so called positive dielectrophoresis. The effects of electrical neutrality
of dielectric nanoparticles, free charge screening due to the ionic nature of
the liquid, domain pattern geometry, and the Brownian motion on the mechanism
of nanoparticle deposition and the stability of the deposited pattern are

Invariant construction of solutions to Einstein's field equations - LRS perfect fluids II
The properties of LRS class II perfect fluid space-times are analyzed using
the description of geometries in terms of the Riemann tensor and a finite
number of its covariant derivatives. In this manner it is straightforward to
obtain the plane and hyperbolic analogues to the spherical symmetric case. For
spherically symmetric static models the set of equations is reduced to the
Tolman-Oppenheimer-Volkoff equation only. Some new non-stationary and
inhomogeneous solutions with shear, expansion, and acceleration of the fluid
are presented. Among these are a class of temporally self-similar solutions
with equation of state given by , and a class of
solutions characterized by . We give an example of geometry
where the Riemann tensor and the Ricci rotation coefficients are not sufficient
to give a complete description of the geometry. Using an extension of the
