    Tonsillar metastasis of gastric cancer

    Metastasis from a malignant tumor to the palatine tonsils is rare, with only 100 cases reported in the English-language literature. Tonsillar metastasis from a gastric cancer is very rare. We report here a case of palatine tonsillar metastasis after gastric cancer surgery. The patient was an 88-year-old woman who had gastric cancer with abdominal wall invasion. She had undergone a distal gastrectomy with abdominal wall resection and D2 lymph node dissection. Histologically, the tumor was primarily a moderately differentiated adenocarcinoma. It was stage IV (T4, N1, M0) using TNM clinical classification. The patient developed pharyngeal discomfort and abdominal pain and was hospitalized during the follow-up period, 1 year 9 months post-operatively. Multiple lung metastases, Virchow’s lymph node metastasis, and adrenal metastasis were observed. A mass of 2.5 cm was also observed in the right palatine tonsil. It was diagnosed as a moderately differentiated adenocarcinoma, a metastasis from gastric cancer. There was a concern of asphyxiation due to hemorrhage of the tumor; however, the tumor dislodged approximately 10 days after biopsy and tonsillar recurrence was not observed. The patient died 1 year 10 months post-operatively. In the literature there are cases with tonsillar metastases where surgical treatment, radiotherapy, and chemotherapy were performed and extension of survival was seen. Tonsillar metastasis is a form of systemic metastasis of a malignant tumor, and there is a high risk for asphyxiation from tumor dislodgement or hemorrhage. Thus, it is important to recognize tonsillar metastasis as an oncologic emergency

    Il volume analizza la vita e l'opera di Francesco Paolo Neglia, musicista siciliano vissuto fra Italia e Germania a cavallo del XIX e XX secolo. Strumentista, didatta, direttore d'orchestra, Neglia si dedic\uf2 in modo completo alla musica, componendo soprattutto lavori strumentali (sinfonie, quartetti, pezzi per pianoforte), musica sacra ed un'unica opera, intitolata "Zelia". Ad Amburgo fond\uf2 un Conservatorio che prese il suo nome, ma che dovette abbandonare per amor di patria, a causa dello scoppio della prima guerra mondiale. Dopo una diffusa panoramica sulla vita e sulla musica strumentale, il testo si incentra sulla genesi e sulla drammaturgia di "Zelia", chiudendosi con un capitolo sulla fortuna critica e con un'appendice contenente il libretto dell'opera

Si sottolinea l'influenza di importanti direttori coevi (in particolare Felix Weingartner) e il contributo di Neglia alla diffusione e valorizzazione di compositori quali Anton Bruckner, Hugo Wolf, Josef Suk, Feliks Nowowiejski.     Il presente lavoro affronta lo studio delle attivit\ue0 musicali presso le istituzioni ecclesiastiche di Palermo lungo il XVI e la prima met\ue0 del XVII secolo. Partendo dall'analisi delle fonti d'archivio, si intende cogliere la componente concreta della produzione e della prassi musicale, incentrando l'attenzione sulle reti di contatto fra istituzioni. Grazie al contributo di diverse discipline e approcci metodologici, vengono sottolineate le principali iniziative di committenza musicale in relazione a dinamiche sociali e di consolidamento del potere istituzionale. L'indagine, dunque, si articola secondo alcune direttive privilegiate: organizzazione e finanziamento delle iniziative musicali; personale impiegato nelle singole istituzioni e cappelle; pratiche esecutive di maggiore diffusione; rapporto fra musica e contesto urbano; attivit\ue0 di promozione svolta dai pi\uf9 importanti ordini religiosi. Alla fine del percorso si colloca la fonte musicale, con particolare attenzione ad autori e repertori di cui oggi rimangono musiche a stampa.     The effects of wearing a removable-partial-denture on the bite forces: a cross-sectional study

    Background: Removable partial dentures are a frequently used prosthetic treatment in the elderly population, but different types or RPDs might guarantee different chewing capabilities. In many studies, the relationship between chewing and aging has been reported and it has been shown that efficient chewing can improve the overall quality of life. Objectives: In the present study, the relationship between maximum bite force (MBF) and RPDs was studied. A relationship between the body mass index (BMI) and the type of prosthesis was also analyzed. Methods: 240 elderly patients, 120 males and 120 females, with bilateral posterior edentulism (class 1 of Kennedy classification) who had been wearing an RPD for at least a year, were recruited. Patients were divided into two groups: Group 1: male (n = 60) and female (n = 60) patients with bilateral edentulous areas located posterior to the remaining natural teeth and natural teeth in the opposite dental arch. Group 2: male (n = 60) and female (n = 60) patients with maxillary and mandibular bilateral edentulous areas located posterior to the remaining natural teeth. Their Body Mass Index (BMI) and Maximum bite force (MBF) were measured and compared according to the material and design of their RPD. Re-sults: In both Groups, patients wearing cobalt-chrome alloy RPDs (Co-Cr-RPD) (Group 1: 20.25 ± 6.7 MBF, p < 0.001; Group 2: 16.0 ± 5.7 MBF, p < 0.001) had an increased MBF when compared to polymethylmethacrylate RPD (PMMA-RPD) (Group 1: 12.9 ± 3.36 MBF; Group 2: 10.4 + 2.8 MBF), and Valplast RPD (V-RPD) (Group 1: 14.3 ± 4.7 MBF; Group 2: 11.3 ± 3.4 MBF) users. There were no significant differences in bite force between patients wearing PMMA-RPD and V-RPD in both Groups. Patients in Group 2 showed a lower MBF than those in Group 1 (Group 1: 16.05 ± 6.13 MBF; Group 2: 12.6 ± 4.84 MBF; p < 0.001). Conclusions: A reduction in chewing force can lead to choosing softer foods for nutrition, which can lead to an increase in BMI. Our results show that only CoCr-RPD wearers were able to chew consistent food, whereas PMMA-RPD and V-RPD, due to the properties of the materials, their instability, and the possibility of causing pain during mastication, de-termined a limitation in the choice of food for many of the participants
