Mental health, attachment and breastfeeding: implications for adopted children and their mothers
Breastfeeding an adopted child has previously been discussed as something that is nice to do but without potential for significant benefit. This paper reviews the evidence in physiological and behavioural research, that breastfeeding can play a significant role in developing the attachment relationship between child and mother. As illustrated in the case studies presented, in instances of adoption and particularly where the child has experienced abuse or neglect, the impact of breastfeeding can be considerable. Breastfeeding may assist attachment development via the provision of regular intimate interaction between mother and child; the calming, relaxing and analgesic impact of breastfeeding on children; and the stress relieving and maternal sensitivity promoting influence of breastfeeding on mothers. The impact of breastfeeding as observed in cases of adoption has applicability to all breastfeeding situations, but may be especially relevant to other at risk dyads, such as those families with a history of intergenerational relationship trauma; this deserves further investigation
Cluster-based interactive volume rendering with Simian
technical reportCommodity-based computer clusters offer a cost-effective alternative to traditional largescale, tightly coupled computers as a means to provide high-performance computational and visualization services. The Center for the Simulation of Accidental Fires and Explosions (C-SAFE) at the University of Utah employs such a cluster, and we have begun to experiment with cluster-based visualization services. In particular, we seek to develop an interactive volume rendering tool for navigating and visualizing large-scale scientific datasets. Using Simian, an OpenGL volume renderer, we examine two approaches to cluster-based interactive volume rendering: (1) a ?cluster-aware? version of the application that makes explicit use of remote nodes through a message-passing interface, and (2) the unmodified application running atop the Chromium clustered rendering framework. This paper provides a detailed comparison of the two approaches by carefully considering the key issues that arise when parallelizing Simian. These issues include the richness of user interaction; the distribution of volumetric datasets and proxy geometry; and the degree of interactivity provided by the image rendering and compositing schemes. The results of each approach when visualizing two large-scale C-SAFE datasets are given, and we discuss the relative advantages and disadvantages that were considered when developing our cluster-based interactive volume rendering application
Emergency preparedness for those who care for infants in developed country contexts
Emergency management organisations recognise the vulnerability of infants in emergencies, even in developed countries. However, thus far, those who care for infants have not been provided with detailed information on what emergency preparedness entails. Emergency management authorities should provide those who care for infants with accurate and detailed information on the supplies necessary to care for them in an emergency, distinguishing between the needs of breastfed infants and the needs of formula fed infants. Those who care for formula fed infants should be provided with detailed information on the supplies necessary for an emergency preparedness kit and with information on how to prepare formula feeds in an emergency. An emergency preparedness kit for exclusively breastfed infants should include 100 nappies and 200 nappy wipes. The contents of an emergency preparedness for formula fed infants will vary depending upon whether ready-to-use liquid infant formula or powdered infant formula is used. If ready-to-use liquid infant formula is used, an emergency kit should include: 56 serves of ready-to-use liquid infant formula, 84 L water, storage container, metal knife, small bowl, 56 feeding bottles and teats/cups, 56 zip-lock plastic bags, 220 paper towels, detergent, 120 antiseptic wipes, 100 nappies and 200 nappy wipes. If powdered infant formula is used, an emergency preparedness kit should include: two 900 g tins powdered infant formula, 170 L drinking water, storage container, large cooking pot with lid, kettle, gas stove, box of matches/lighter, 14 kg liquid petroleum gas, measuring container, metal knife, metal tongs, feeding cup, 300 large sheets paper towel, detergent, 100 nappies and 200 nappy wipes. Great care with regards hygiene should be taken in the preparation of formula feeds. Child protection organisations should ensure that foster carers responsible for infants have the resources necessary to formula feed in the event of an emergency. Exclusive and continued breastfeeding should be promoted as an emergency preparedness activity by emergency management organisations as well as health authorities. The greater the proportion of infants exclusively breastfed when an emergency occurs, the more resilient the community, and the easier it will be to provide effective aid to the caregivers of formula fed infants
EVALUATION OF ACOUSTICAL HOLOGRAPHY FOR THE INSPECTION OF LIGHT WATER REACTOR WELD ASSEMBLIES
The primary objective of this program was the evaluation of acoustical holography techniques for characterization of the light water reactor weld surface signatures in the nuclear safeguards program. The accurate characterization of weld surface irregulari ties and vertical deviations was achieved using acoustical holographic interferometric techniques. Preselected weld surfaces were inspected and the vertical deviations characterized by phase measurements or fringe densities in the image. Experimental results on Sandia samples verify depth deviation sensitivities of 0.11 {micro}m to 0.16 {micro}m. The two point interferogram technique is recommended for surveillance of the weld surface associated wi th fuel rod removal in the nuclear safeguard program. The use of this unique holographic signal processing provides essentially a fail-safe method for surveillance of clandestine fuel rod removal. Statistical analysis indicates 99.99% (weld surface deviation) confidence interval between 2~m and 3~m can be achieved. These results illustrate the extremely high resolution capabilities of the surveillance technique employing coherent signal processing
Milk sharing: from private practice to public pursuit
After only six months, a commerce-free internet-based milk-sharing model is operating in nearly 50 countries, connecting mothers who are able to donate breast milk with the caregivers of babies who need breast milk. Some public health authorities have condemned this initiative out of hand. Although women have always shared their milk, in many settings infant formula has become the "obvious" alternative to a mother's own milk. Yet an internationally endorsed recommendation supports mother-to-mother milk sharing as the best option in place of a birth mother's milk. Why then this rejection? Several possibilities come to mind: 1) ignorance and prejudice surrounding shared breast milk; 2) a perceived challenge to the medical establishment of a system where mothers exercise independent control; and 3) concern that mother-to-mother milk sharing threatens donor milk banks. We are not saying that milk sharing is risk-free or that the internet is an ideal platform for promoting it. Rather, we are encouraging health authorities to examine this initiative closely, determine what is happening, and provide resources to make mother-to-mother milk sharing as safe as possible. Health authorities readily concede that life is fraught with risk; accordingly, they promote risk-reduction and harm-minimisation strategies. Why should it be any different for babies lacking their own mothers' milk? The more that is known about the risks of substituting for breast milk, the more reasonable parental choice to use donor milk becomes. We believe that the level of intrinsic risk is manageable through informed sharing. If undertaken, managed and evaluated appropriately, this made-by-mothers model shows considerable potential for expanding the world's supply of human milk and improving the health of children
Emergency preparedness for infant and young child feeding in emergencies (IYCF-E) : an Australian audit of emergency plans and guidance
BACKGROUND: Australia experiences a high incidence of natural emergencies and Australian governments have committed significant investment into emergency preparedness and response. Amongst the population groups most vulnerable to emergencies are infants and young children with their vulnerability centering around their specific food and fluid needs. For this reason, the World Health Assembly has urged all member states to develop and implement infant and young child feeding in emergency (IYCF-E) plans in line with international guidance. This study aimed to determine the degree to which Australia has complied with this direction by conducting an audit of Australian emergency plans and guidance. METHODS: Australian Federal, State/Territory and a sample of Local government emergency plans and guidance were located via web searches. Documents were searched for key words to identify content dealing with the needs of infants and young children. Plans and guidance were also searched for content dealing with the needs of animals as a comparison. RESULTS: While plans and guidance contained numerous pointers to the desirability of having plans that address IYCF-E, there was a dearth of planning at all levels of government for the needs of infants and young children. Guidance related to heat waves contained information that could prove dangerous to infants. No agency at Federal or State/Territory had designated responsibility for IYCF-E or children in general. This was in stark contrast to the situation of animals for which there was widespread and comprehensive planning at all levels of government with clear designation of organisational responsibility. CONCLUSIONS: Lack of planning for IYCF-E in Australia places infants and young children at serious risk of adverse health consequences in emergencies. Australian Federal, State/Territory and Local governments need to take action to ensure that IYCF-E plans and guidance are developed and deployed in line with international standards. The pathway to successful integration of animal welfare plans provides a method for a similar integration of IYCF-E plans. Government health authorities are best placed to lead and be responsible for IYCF-E in Australia. National governments internationally should similarly take action to ensure that their youngest, most vulnerable citizens are protected in emergencies
'Want to Help the Children? Help the Parents': Challenges and Solutions from the Babies and Young Children in the Black Summer (BiBS) Study
This report presents the findings of the Babies and Young Children in the Black Summer (BiBS) Study. It considers the experiences of caregivers of very young children (0-4 years) of the 2019-20 Black Summer Bushfires and of emergency responders that supported families with very young children in this and other emergencies. Recommendations are made to improve emergency planning and response to better support very young children and their caregivers in Australian disasters
Facilitators and barriers of wet nursing from antiquity to the present : a narrative review with implications for emergencies
Background: If maternal breastfeeding is not possible, wet nursing allows infants to receive the nutrition and protection against disease that breastfeeding provides. Such protection may be particularly valuable in emergencies. However, while wet nursing is recommended in the Operational Guidance on Infant and Young Child Feeding in Emergencies it is underutilized. This narrative review aimed to develop an understanding of wet nursing practice across time and location, and why wet nursing is currently so little supported to inform interventions to support wet nursing in emergencies. Method: Medline and Embase were searched for "wet nursing," "cross-nursing," "shared breastfeeding," and "non-maternal nursing". Included articles were inductively analyzed to identify positive and negative factors associated with previous wet nursing practice. Results: This review included 74 records. Our analysis of the wet nursing literature includes historical and contemporary themes with milk kinship and wet nursing as risky being shared themes across time periods. Our analysis revealed that it is how wet nursing is undertaken that influences whether it benefits women, children, and societies or not. Facilitators and barriers to wet nursing in emergencies related to infant mortality rates, cultural and individual support for wet nursing, availability of wet nurses, and resources to support wet nursing. Conclusion: Understanding the conditions under which wet nursing has positive outcomes and what can be done to facilitate beneficial wet nursing practices will assist in enabling wet nursing to be more commonly implemented in emergencies
