    A comparison of motion sickness prevalence between seafarers and non-seafarers onboard naval platforms

    Background: Motion sickness may crucially affect the operational performance of soldiers at sea and this differs between individuals and environments. Objectives: To report on the prevalence and understand the risk factors for motion sickness among Singaporean sailors (seafarers) and attached army servicemen (nonseafarers) onboard naval platforms. Methodology: Cross sectional study using self-administered survey of 503 personnel over the monsoon period from January to April 2001. Results: The prevalence of motion sickness was distinctly higher in the army (59.2%) personnel compared with the navy (38.3%) over a series of sea states. The most common symptoms were headache, nausea and dizziness. The Motion Sickness Susceptibility Questionnaire was used to score susceptibility and appeared to correlate better among non-seafarers rather than seafarers. The discomfort experienced in one's environment was perceived to contribute towards onset and smoking appeared to be protective against motion sickness. Regular sailing appears to be an important factor in minimising motion sickness. Conclusion: While we understand motion sickness to be a continuum of physiological responses to the whole body vibration, it is especially apparent among the non-seafarers. Seafarers by themselves will become less susceptible with regular sailing and they are also more cognizant of the modalities available to alleviate symptoms

Many nursing education programs are seeking strategies for better educating students on technology utilization. Health care technology includes electronic health records, telemedicine, and clinical decision support systems. However, little focus has been placed on the role of social media in health care. This paper describes an educational workshop using standardized patients and hands-on experiences to introduce advanced practice nurses in a Doctor of Nursing Practice program to the role of social media in addressing issues inherent in the delivery of rural health care. The students explore innovative approaches for utilizing social media for patient and caregiver support as well as identify online resources that assist providers in a rural setting

    Enhanced detection of groundwater contamination from a leaking waste disposal site by microbial community profiles

    Groundwater biogeochemistry is adversely impacted when municipal solid waste leachate, rich in nutrients and anthropogenic compounds, percolates into the subsurface from leaking landfills. Detecting leachate contamination using statistical techniques is challenging because well strategies or analytical techniques may be insufficient for detecting low levels of groundwater contamination. We sampled profiles of the microbial community from monitoring wells surrounding a leaking landfill using terminal restriction fragment length polymorphism (T-RFLP) targeting the 16S rRNA gene. Results show in situ monitoring of bacteria, archaea, and the family Geobacteraceae improves characterization of groundwater quality. Bacterial T-RFLP profiles showed shifts correlated to known gradients of leachate and effectively detected changes along plume fringes that were not detected using hydrochemical data. Experimental sediment microcosms exposed to leachate-contaminated groundwater revealed a shift from a -Proteobacteria and Actinobacteria dominated community to one dominated by Firmicutes and δ-Proteobacteria. This shift is consistent with the transition from oxic conditions to an anoxic, iron-reducing environment as a result of landfill leachate-derived contaminants and associated redox conditions. We suggest microbial communities are more sensitive than hydrochemistry data for characterizing low levels of groundwater contamination and thus provide a novel source of information for optimizing detection and long-term monitoring strategies at landfill sites.     Educating Future Nursing Scientists: Recommendations for Integrating Omics Content in PhD Programs

    Preparing the next generation of nursing scientists to conduct high-impact, competitive, sustainable, innovative, and interdisciplinary programs of research requires that the curricula for PhD programs keep pace with emerging areas of knowledge and health care/biomedical science. A field of inquiry that holds great potential to influence our understanding of the underlying biology and mechanisms of health and disease is omics. For the purpose of this article, omics refers to genomics, transcriptomics, proteomics, epigenomics, exposomics, microbiomics, and metabolomics. Traditionally, most PhD programs in schools of nursing do not incorporate this content into their core curricula. As part of the Council for the Advancement of Nursing Science\u27s Idea Festival for Nursing Science Education, a work group charged with addressing omics preparation for the next generation of nursing scientists was convened. The purpose of this article is to describe key findings and recommendations from the work group that unanimously and enthusiastically support the incorporation of omics content into the curricula of PhD programs in nursing. The work group also calls to action faculty in schools of nursing to develop strategies to enable students needing immersion in omics science and methods to execute their research goals

The purpose of this Editorial Board Special Article is to provide PhD students and advisors with suggestions for making the most of their doctoral experience. Editorial Board members provide their individual insights on the skills and attributes students must acquire during the course of their doctoral education in order to succeed. The authors provide practical tips and advice on how to excel in a PhD program, including how to select an advisor and a dissertation committee, the importance of attending conferences to increase visibility and develop a network of colleagues, presenting and publishing research while still a student, and balancing work and personal life. Students who take full advantage of the opportunities available to them during the course of their doctoral programs will graduate well prepared to take on the multiple responsibilities of research, teaching, and leadership

    Enlarged Perivascular Spaces are Negatively Associated with Montreal Cognitive Assessment Scores in Older Adults

    Emerging evidence suggests that enlarged perivascular spaces (ePVS) may be a clinically significant neuroimaging marker of global cognitive function related to cerebral small vessel disease (cSVD). We tested this possibility by assessing the relationship between ePVS and both a standardized measure of global cognitive function, the Montreal Cognitive Assessment (MoCA), and an established marker of cSVD, white matter hyperintensity volume (WMH) volume. One hundred and eleven community-dwelling older adults (56–86) underwent neuroimaging and MoCA testing. Quantification of region-specific ePVS burden was performed using a previously validated visual rating method and WMH volumes were computed using the standard ADNI pipeline. Separate linear regression models were run with ePVS as a predictor of MoCA scores and whole brain WMH volume. Results indicated a negative association between MoCA scores and both total ePVS counts (P ≤ 0.001) and centrum semiovale ePVS counts (P ≤ 0.001), after controlling for other relevant cSVD variables. Further, WMH volumes were positively associated with total ePVS (P = 0.010), basal ganglia ePVS (P ≤ 0.001), and centrum semiovale ePVS (P = 0.027). Our results suggest that ePVS burden, particularly in the centrum semiovale, may be a clinically significant neuroimaging marker of global cognitive dysfunction related to cSVD

    Crew factors in flight operations 6: Psychophysiological responses to helicopter operations

    Thirty-two helicopter pilots were studied before, during, and after 4-5 day trips providing support services from Aberdeen, Scotland, to rigs in the North Sea oil fields. Early on-duty times obliged subjects to wake up 1.5 hours earlier on trip days than on pretrip days. Consequently, they slept nearly an hour less per night on trips. They reported more fatigue on post-trip days than on pretrip days, suggesting a cumulative effect of duty-related activities and sleep loss. Fatigue and negative affect were higher, and activation lower, by the end of trip days than by the end of pretrip days. The earlier a subject went on duty, the lower his activation by the end of the day. Caffeine consumption increased 42 percent on trip days. The incidence of headache doubled, of back pain increased twelve fold, and of burning eyes quadrupled. In the aircraft studied, thermal discomfort and high vibration levels were common. The longer pilots remained on duty, the more negative their mood became

    Water Exchange Rate across the Blood-Brain Barrier Is Associated with CSF Amyloid-β 42 in Healthy Older Adults

    INTRODUCTION: We tested if water exchange across the blood-brain barrier (BBB), estimated with a noninvasive magnetic resonance imaging (MRI) technique, is associated with cerebrospinal fluid (CSF) biomarkers of Alzheimer\u27s disease (AD) and neuropsychological function. METHODS: Forty cognitively normal older adults (67–86 years old) were scanned with diffusion‐prepared, arterial spin labeling (DP‐ASL), which estimates water exchange rate across the BBB (kw). Participants also underwent CSF draw and neuropsychological testing. Multiple linear regression models were run with kw as a predictor of CSF concentrations and neuropsychological scores. RESULTS: In multiple brain regions, BBB kw was positively associated with CSF amyloid beta (Aβ)42 concentration levels. BBB kw was only moderately associated with neuropsychological performance. DISCUSSION: Our results suggest that low water exchange rate across the BBB is associated with low CSF Aβ42 concentration. These findings suggest that kw may be a promising noninvasive indicator of BBB Aβ clearance functions, a possibility which should be further tested in future research

    Macrolide‐resistant Mycoplasma pneumoniae pneumonia in transplantation: Increasingly typical?

    Mycoplasma pneumoniae is one of the most common bacterial causes of pneumonia. Macrolide‐resistant M pneumoniae (MRMP) was documented in 7.5% of isolates in the United States. Resistance portends poor outcomes to macrolide therapy, yet patients respond well to fluoroquinolones or tetracyclines such as minocycline. However, MRMP may be under‐appreciated because M pneumoniae generally causes relatively mild infections in non‐immunosuppressed adults that may resolve without effective therapy and because microbiological confirmation and susceptibility are not routinely performed. We report two cases of pneumonia due to MRMP in kidney transplant recipients. Both patients required hospital admission, worsened on macrolide therapy, and rapidly defervesced on doxycycline or levofloxacin. In one case, M pneumoniae was only identified by multiplex respiratory pathogen panel analysis of BAL fluid. Macrolide resistance was confirmed in both cases by real‐time PCR and point mutations associated with macrolide resistance were identified. M pneumoniae was isolated from both cases, and molecular genotyping revealed the same genotype. In conclusion, clinicians should be aware of the potential for macrolide resistance in M pneumoniae, and may consider non‐macrolide‐based therapy for confirmed or non‐responding infections in patients who are immunocompromised or hospitalized.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/163484/2/tid13318.pdfhttp://deepblue.lib.umich.edu/bitstream/2027.42/163484/1/tid13318_am.pd
