Incidence of, predictors for, and mortality associated with malignant ventricular arrhythmias in non-ST elevation myocardial infarction patients.
BACKGROUND: The incidence of non-ST elevation myocardial infarction (NSTEMI) is increasing. Although life-threatening ventricular arrhythmias have been well-documented in patients with ST elevation MI (STEMI), their incidence and importance in NSTEMI have not been examined in similar detail. We examined the incidence, predictors, and mortality rates of ventricular arrhythmias in a cohort of NSTEMI patients undergoing an early invasive strategy.
METHODS: Consecutive patients admitted with NSTEMI who underwent cardiac catheterization within 48 h of admission were identified by chart review. Presence and type of ventricular arrhythmias and 30-day mortality were recorded. Malignant arrhythmias were defined as sustained ventricular tachycardia (VT, \u3e100 beats/min lasting \u3e30 s) or fibrillation (VF). Clinical risk factors, laboratory values, findings on electrocardiogram, echocardiogram, cardiac catheterization, and revascularization procedure data were recorded.
RESULTS: VT/VF occurred in 21 (7.6%) of 277 NSTEMI patients. Sixty percent of these events occurred within the first 48 h after hospital admission, with a median occurrence at 72 h. Twelve patients (4.3%) required defibrillation. Troponin levels were higher and left ventricular ejection fraction was lower in the VT/VF group. Multivariable analysis also identified the presence of left bundle branch block and need for urgent coronary artery bypass grafting as significant predictors of malignant ventricular arrhythmias. Thirty-day mortality was significantly higher in NSTEMI patients with malignant ventricular arrhythmias than without (38 vs. 3%, P\u3c0.001).
CONCLUSION: Despite an early invasive strategy, malignant ventricular arrhythmias are frequent in NSTEMI patients and are associated with increased 30-day mortality
First report of maize yellow mosaic virus infecting sugarcane (Saccharum spp.) and itch grass (Rottboellia cochinchinensis) in Nigeria

region of Nigeria, sugarcane and itch grass (Rottboellia cochinchinensis) plants showing virus-like
mosaic symptoms were encountered in farmers’ sugarcane fields in Kaduna State. Symptomatic
leaf tissue samples from five randomly selected plants (sugarcane = 4; itch grass = 1) were dried
and stored under CaCl2 at room temperature then shipped to Texas A&M AgriLife Research and
Extension Center, Weslaco, TX, with USDA-APHIS-PPQ permit (P526P-14-04321) for further
analysis. The MagMAX-96 viral RNA isolation kit (Thermo Fisher) was used to isolate total nucleic
acid (TNA) from each sample and from a sample subset consisting of pooled leaf tissue materials
from both plants. TNA aliquot from the pooled sample was subjected to ribosomal RNA depletion
and cDNA library construction using a TruSeq Stranded Total RNA with Ribo-Zero Plant kit
(Illumina), then sequenced on the Illumina NextSeq 500 platform. The raw high-throughput
sequencing (HTS) reads were analyzed as previously described (Alabi et al. 2015), generating

Association between obesity and infarct size: insight into the obesity paradox.
Abstract:
Background: In patients with coronary heart disease, being overweight or obese is associated with better outcomes, a phenomenon known as the \u27obesity paradox\u27. Despite the high prevalence of obesity in the United Sates, its effects on infarct size are largely unexplored.
Methods: Prospective cross-sectional study of 102 consecutive patients admitted with acute myocardial infarction (MI). Standardized forms were used to collect data on body mass index (BMI), waist circumference (WC), cardiovascular risk factors, and medications. Peak troponin I and creatinine phosphokinase (CPK) were used to estimate infarct size. Epicardial and pericardial fat were measured by echocardiography. We used univariate and multivariate analyses to assess whether obesity was associated with infarct size. Correlations between BMI, WC and cardiac fat with cardiac biomarkers were also performed.
Results: Mean age was 62±12 years, and 55% were men. Obesity was diagnosed in 69%. On multivariate analysis, obesity was associated with greater infarct size in non-ST elevation MI (p=0.02). A positive correlation was observed between BMI and peak troponin I (rho=0.24, p=0.03), and both, BMI and WC had a positive correlations with CPK levels (rho=0.28, & rho=0.28, both p=0.02). However, in ST elevation MI, obesity was associated with smaller infarct size (p=0.05). Epicardial fat + pericardial fat had a negative correlation with peak CPK levels (rho=-0.36, p=0.05).
Conclusions: We observed an opposite association between obesity and infarct size depending on the type of MI. These results were unexpected and may provide insight into the pathophysiology of the obesity paradox
Treatment of Cryptococcal Meningitis with Combination Amphotericin B and Flucytosine for Four as Compared with Six Weeks
Abstract
One hundred ninety-four patients with cryptococcal meningitis were enrolled in a multicenter, prospective, randomized clinical trial to compare the efficacy and toxicity of four as compared with six weeks of combination amphotericin B and flucytosine therapy. Among 91 patients who met preestablished criteria for randomization, cure or improvement was noted in 75 percent of those treated for four weeks and in 85 percent of those treated for six weeks. The estimated relapse rate for the four-week regimen was higher — 27 as compared with 16 percent — whereas the incidence of toxic effects for the two regimens was similar — 44 as compared with 43 percent. Among 23 transplant recipients, 4 of 5 treated for four weeks relapsed, leading to the decision to treat the rest of the group for six weeks. Only 3 of the 18 treated for six weeks relapsed. In a third group of 80 patients, the protocol was not followed during the initial four weeks, and these patients were not randomized. Thirty-eight died or relapsed. These and other findings in this study are consistent with the view that the four-week regimen should be reserved for patients who have meningitis without neurologic complications, underlying disease, or immunosuppressive therapy; a pretreatment cerebrospinal fluid white-cell count above 20 per cubic millimeter and a serum cryptococcal antigen titer below 1:32; and at four weeks of therapy, a negative cerebrospinal fluid India ink preparation and serum and cerebrospinal fluid cryptococcal-antigen titers below 1:8. Entanglement, Mixedness, and Spin-Flip Symmetry in Multiple-Qubit Systems
A relationship between a recently introduced multipartite entanglement
measure, state mixedness, and spin-flip symmetry is established for any finite
number of qubits. It is also shown that, within those classes of states
invariant under the spin-flip transformation, there is a complementarity
relation between multipartite entanglement and mixedness. A number of example

Diffuse light and galaxy interactions in the core of nearby clusters
The kinematics of the diffuse light in the densest regions of the nearby
clusters can be unmasked using the planetary nebulae (PNs) as probes of the
stellar motions. The position-velocity diagrams around the brightest cluster
galaxies (BCGs) identify the relative contributions from the outer halos and
the intracluster light (ICL), defined as the light radiated by the stars
floating in the cluster potential. The kinematics of the ICL can then be used
to asses the dynamical status of the nearby cluster cores and to infer their
formation histories. The cores of the Virgo and Coma are observed to be far
from equilibrium, with mergers currently on-going, while the ICL properties in
the Fornax and Hydra clusters show the presence of sub-components being
accreted in their cores, but superposed to an otherwise relaxed population of
stars. Finally the comparison of the observed ICL properties with those
predicted from Lambda-CDM simulations indicates a qualitative agreement and
provides insights on the ICL formation. Both observations and simulations
indicate that BCG halos and ICL are physically distinct components, with the
``hotter" ICL dominating at large radial distances from the BCGs halos as the

Quasars and their host galaxies
This review attempts to describe developments in the fields of quasar and
quasar host galaxies in the past five. In this time period, the Sloan and 2dF
quasar surveys have added several tens of thousands of quasars, with Sloan
quasars being found to z>6. Obscured, or partially obscured quasars have begun
to be found in significant numbers. Black hole mass estimates for quasars, and
our confidence in them, have improved significantly, allowing a start on
relating quasar properties such as radio jet power to fundamental parameters of
the quasar such as black hole mass and accretion rate. Quasar host galaxy
studies have allowed us to find and characterize the host galaxies of quasars
to z>2. Despite these developments, many questions remain unresolved, in
particular the origin of the close relationship between black hole mass and
galaxy bulge mass/velocity dispersion seen in local galaxies.Comment: Review article, to appear in Astrophysics Update
Active Galactic Nuclei at the Crossroads of Astrophysics
Over the last five decades, AGN studies have produced a number of spectacular
examples of synergies and multifaceted approaches in astrophysics. The field of
AGN research now spans the entire spectral range and covers more than twelve
orders of magnitude in the spatial and temporal domains. The next generation of
astrophysical facilities will open up new possibilities for AGN studies,
especially in the areas of high-resolution and high-fidelity imaging and
spectroscopy of nuclear regions in the X-ray, optical, and radio bands. These
studies will address in detail a number of critical issues in AGN research such
as processes in the immediate vicinity of supermassive black holes, physical
conditions of broad-line and narrow-line regions, formation and evolution of
accretion disks and relativistic outflows, and the connection between nuclear

Fitting the integrated Spectral Energy Distributions of Galaxies
Fitting the spectral energy distributions (SEDs) of galaxies is an almost
universally used technique that has matured significantly in the last decade.
Model predictions and fitting procedures have improved significantly over this
time, attempting to keep up with the vastly increased volume and quality of
available data. We review here the field of SED fitting, describing the
modelling of ultraviolet to infrared galaxy SEDs, the creation of
multiwavelength data sets, and the methods used to fit model SEDs to observed
galaxy data sets. We touch upon the achievements and challenges in the major
ingredients of SED fitting, with a special emphasis on describing the interplay
between the quality of the available data, the quality of the available models,
and the best fitting technique to use in order to obtain a realistic
measurement as well as realistic uncertainties. We conclude that SED fitting
can be used effectively to derive a range of physical properties of galaxies,
such as redshift, stellar masses, star formation rates, dust masses, and
metallicities, with care taken not to over-interpret the available data. Yet
there still exist many issues such as estimating the age of the oldest stars in
a galaxy, finer details ofdust properties and dust-star geometry, and the
influences of poorly understood, luminous stellar types and phases. The
challenge for the coming years will be to improve both the models and the
observational data sets to resolve these uncertainties. The present review will
be made available on an interactive, moderated web page (sedfitting.org), where
the community can access and change the text. The intention is to expand the

Rhinologic Issues in Pregnancy
The diagnosis and treatment of rhinitis, sinusitis, and epistaxis during pregnancy present unique challenges to the otolaryngologist. Poorly controlled sinonasal disease may have significant adverse effects on the mother's quality of life and pregnancy outcomes and the lack of adequately controlled safety data limits the clinician's ability to make informed decisions about management. At the conclusion of this discussion, the reader should be familiar with the available literature and evidence-based guidelines regarding the safety and indications for radiographic imaging, clinical testing, medical intervention, and surgical treatment of sinonasal disease in pregnant patients. A review was performed of pertinent guidelines regarding the management of gestational rhinitis, sinusitis, and epistaxis, including the diagnostic and therapeutic limitations and physiological changes specific to pregnancy. A study population of four patients was analyzed to highlight the steps of management by reviewing the patient charts including pertinent history, physical examination, clinical course, and operative reports. Two patients with epistaxis and two patients with rhinosinusitis ranging from 27 to 38 years of age and between 16 and 35 weeks gestation were analyzed. The treatment of sinonasal disease during pregnancy is challenging and a thorough knowledge of the available medical evidence and treatment guidelines is necessary to optimize pregnancy outcomes. When the severity of disease precludes the possibility of delaying treatment, the clinician should provide a limited intervention that optimizes the mother's health without placing the fetus at significant risk
