    Impact of Remote Monitoring on Standardized Outcomes in Nephrology-Peritoneal Dialysis

    Introduction: This study aimed to evaluate the association between the use of remote patient monitoring (RPM) in patients on automated peritoneal dialysis (APD) and the Standardized Outcomes in Nephrology in peritoneal dialysis (SONG-PD) clinical outcomes. Methods: A prospective and multicenter cohort study was conducted on patients with advanced chronic kidney disease on APD, recruited at 16 Spanish Hospitals, between June 1 and December 31, 2021. Patients were divided into 2 cohorts, namely patients on APD with RPM (APD-RPM) and patients on APD without RPM. The primary endpoints were the standardized outcomes of the SONG-PD clinical outcomes: PD-associated infection, cardiovascular disease (CVD), mortality rate, technique survival, and life participation (assessed as health-related quality of life [QoL]). Propensity score matching (PSM) was used to evaluate the association of RPM exposure with the clinical outcomes. Results: A total of 232 patients were included, 176 (75.9%) in the APD-RPM group and 56 (24.1%) in the APD-without-RPM group. The mean patient follow-up time was significantly longer in the APD-RPM group than in the APD-without-RPM group (10.4 ± 2.8 vs. 9.4 ± 3.1 months, respectively; P = 0.02). In the overall study sample, the APD-RPM group was associated with a lower mortality rate (hazard ratio [HR]: 0.08; 95% confidence interval [CI]: 0.01 to 0.69; P = 0.020) and greater technique survival rate (HR: 0.25; 95% CI: 0.11 to 0.59; P = 0.001). After PSM, APD-RPM continued to be associated with better technique survival (HR: 0.23; 95% CI: 0.06 to 0.83; P = 0.024). Conclusion: The use of RPM programs in patients on APD was associated with better survival of the technique and lower mortality rates. However, after PSM, only technique survival was significant

    El libro es el resultado de un esfuerzo académico derivado de las investigaciones llevadas a cabo al interior de la línea de investigación sobre mecanismos de protección de los derechos humanos de la Facultad de Jurisprudencia de la Universidad del Rosario, en asocio con la Fundación Hanns Seidel. Esta obra presenta una reflexión sobre los avances y retos que tienen las acciones constitucionales en nuestro país y busca destacar su importancia en el ordenamiento colombiano, como herramientas para el ciudadano en la defensa de sus derechos, del interés público y de los derechos humanos. Así mismo, también se busca comprenderlas como mecanismos de participación ciudadana al ser herramientas que el Constituyente creó para controlar las actuaciones del Estado y, por último, revisar los avances y los vacíos o aspectos que se deben mejorar en su aplicación. Esperamos que este libro sea de interés y utilidad para quienes, en estos casi treinta años de existencia de las acciones, han puesto su mirada en la importancia de estas, lo que representan en términos de progresividad del ordenamiento en cuanto a la garantía de derechos y para quienes quieren adentrarse en el análisis de estos mecanismos de defensa de derechos, que cada día son más relevantes en términos de participación ciudadana y ejercicio de derechos políticos del ciudadano en el marco de un Estado social de derecho.The book is the result of an academic effort derived from a research carried out in the line of research on human rights protection mechanisms at the Faculty of Jurisprudence of the Universidad del Rosario, in association with the Hanns Seidel Foundation. This work presents a reflection on the progress and challenges of constitutional actions in our country and aims to highlight their importance in Colombian laws, as tools for citizens to defend their rights, public interest, and human rights. Likewise, it seeks to understand them as mechanisms of citizen participation since they are tools created by the Constituent to control State actions; and, finally, to review the progress and gaps or aspects that should be improved in their application. We hope that this book is of interest and utility to those who, during almost thirty years of their existence, have paid attention to the importance of these actions and what they represent in terms of the progressiveness of laws as a guarantee of rights, as well as to those who want to learn in more depth about these rights protection mechanisms, which are becoming more relevant every day in terms of citizen participation and the exercise of citizens’ political rights within the framework of a social state of law

    Improving equity in access to early diagnosis of cancer in different healthcare systems of Latin America: protocol for the EquityCancer-LA implementation-effectiveness hybrid study

    Introduction Healthcare fragmentation, a main cause for delay in cancer diagnosis and treatment, contributes to high mortality in Latin America (LA), particularly among disadvantaged populations. This research focuses on integrated care interventions, which have been limitedly implemented in the region. The objective is to evaluate the contextual effectiveness of scaling-up an integrated care intervention to improve early diagnosis of frequent cancers in healthcare networks of Chile, Colombia and Ecuador.Methods and analysis This research is two pronged: (A) quasi-experimental design (controlled before and after) with an intervention and a control healthcare network in each LA country, using an implementation-effectiveness hybrid approach to assess the intervention process, effectiveness and costs; and (B) case study design to analyse access to diagnosis of most frequent cancers. Focusing on the most vulnerable socioeconomic population, it develops in four phases: (1) analysis of delays and barriers to early diagnosis (baseline); (2) intervention adaptation and implementation (primary care training, fast-track referral pathway and patient information); (3) intracountry evaluation of intervention and (4) cross-country analysis. Baseline and evaluation studies adopt mixed-methods qualitative (semistructured individual interviews) and quantitative (patient questionnaire survey) methods. For the latter, a sample size of 174 patients with cancer diagnosis per healthcare network and year was calculated to detect a proportions difference of 15%, before and after intervention (α=0.05; β=0.2) in a two-sided test. A participatory approach will be used to tailor the intervention to each context, led by a local steering committee (professionals, managers, policy makers, patients and researchers).Ethics and dissemination This study complies with international and national legal stipulations on ethics. It was approved by each country’s ethical committee and informed consent will be obtained from participants. Besides the coproduction of knowledge with key stakeholders, it will be disseminated through strategies such as policy briefs, workshops, e-tools and scientific papers
