The relevance of reticulocytosis in the classification of hereditary spherocytosis: case report
A Esferocitose Hereditária Ă© uma doença hemolĂtica de origem genĂ©tica polissĂ´mica, podendo ser autossĂ´mica dominante ou recessiva. Seu quando clĂnico Ă© representado por anemia, reticulocitose, hiperbilirrubinemia indireta e presença de numerosos esferĂłcitos na corrente sanguĂnea, os quais se mostram alterados no teste de fragilidade osmĂłtica, revelando uma menor resistĂŞncia Ă lise. Os exames laboratoriais sĂŁo de grande importância na tomada de condutas, todavia, devem ser interpretados juntamente com a histĂłria clĂnica, posto que esta ainda revela-se soberana. Apesar do quadro de reticulocitose ser um critĂ©rio para classificar o paciente portador de esferocitose como grave, este deve ser avaliado com cautela juntamente com outros parâmetros clĂnicos e laboratoriais. É de grande importância o olhar holĂstico do mĂ©dico sobre o paciente, bem como a análise crĂtica do uso de protocolos clĂnicos.The Hereditary spherocytosis is a haemolytic disease of genetic origin polissĂ´mica and may be autosomal dominant or recessive. Its clinical status is represented by anemia, reticulocytosis, indirect hyperbilirubinemia and the presence of numerous spherocytes in the bloodstream, which are altered in the osmotic fragility test, revealing a lower resistance to lysis. The laboratory tests are of great importance in the taking of conducts, nevertheless, they must be interpreted together with the clinical history, since this still shows itself to be sovereign. Although reticulocytosis is a criterion for classifying patients with spherocytosis as severe, it should be carefully evaluated along with other clinical and laboratory parameters. It is of great importance the holistic look of the doctor on the patient, as well as the critical analysis of the use of clinical protocols
