    Structural basis of complement membrane attack complex formation

    In response to complement activation, the membrane attack complex (MAC) assembles from fluid-phase proteins to form pores in lipid bilayers. MAC directly lyses pathogens by a ‘multi-hit’ mechanism; however, sublytic MAC pores on host cells activate signalling pathways. Previous studies have described the structures of individual MAC components and subcomplexes; however, the molecular details of its assembly and mechanism of action remain unresolved. Here we report the electron cryo-microscopy structure of human MAC at subnanometre resolution. Structural analyses define the stoichiometry of the complete pore and identify a network of interaction interfaces that determine its assembly mechanism. MAC adopts a ‘split-washer’ configuration, in contrast to the predicted closed ring observed for perforin and cholesterol-dependent cytolysins. Assembly precursors partially penetrate the lipid bilayer, resulting in an irregular β-barrel pore. Our results demonstrate how differences in symmetric and asymmetric components of the MAC underpin a molecular basis for pore formation and suggest a mechanism of action that extends beyond membrane penetration

    The 4C framework for making reasonable adjustments for people with learning disabilities

    Background People with learning disabilities experience significant inequalities in accessing healthcare. Legal frameworks, such as the Equality Act 2010, are intended to reduce such disparities in care, and require organisations to make ‘reasonable adjustments’ for people with disabilities, including learning disabilities. However, reasonable adjustments are often not clearly defined or adequately implemented in clinical practice. Aim To examine and synthesise the challenges in caring for people with learning disabilities to develop a framework for making reasonable adjustments for people with learning disabilities in hospital. This framework would assist ward staff in identifying and managing the challenges of delivering person-centred, safe and effective healthcare to people with learning disabilities in this setting. Method Fourth-generation evaluation, collaborative thematic analysis, reflection and a secondary analysis were used to develop a framework for making reasonable adjustments in the hospital setting. The authors attended ward manager and matron group meetings to collect their claims, concerns and issues, then conducted a collaborative thematic analysis with the group members to identify the main themes. Findings Four main themes were identified from the ward manager and matron group meetings: communication, choice-making, collaboration and coordination. These were used to develop the 4C framework for making reasonable adjustments for people with learning disabilities in hospital. Discussion The 4C framework has provided a basis for delivering person-centred care for people with learning disabilities. It has been used to inform training needs analyses, develop audit tools to review delivery of care that is adjusted appropriately to the individual patient; and to develop competencies for learning disability champions. The most significant benefit of the 4C framework has been in helping to evaluate and resolve practice-based scenarios. Conclusion Use of the 4C framework may enhance the care of people with learning disabilities in hospital, by enabling reasonable adjustments to be made in these settings

    It is popular nowadays to bring techniques from bibliometrics and scientometrics into the world of digital libraries to analyze the collaboration patterns and explore mechanisms which underlie community development. In this paper we use the DBLP data to investigate the author's scientific career and provide an in-depth exploration of some of the computer science communities.     Accretion column eclipses in the X-ray pulsars GX 1+4 and RX J0812.4-3114

    Sharp dips observed in the pulse profiles of three X-ray pulsars (GX 1+4, RX J0812.4-3114 and A 0535+26) have previously been suggested to arise from partial eclipses of the emission region by the accretion column occurring once each rotation period. We present pulse-phase spectroscopy from Rossi X-ray Timing Explorer satellite observations of GX 1+4 and RX J0812.4-3114 which for the first time confirms this interpretation. The dip phase corresponds to the closest approach of the column axis to the line of sight, and the additional optical depth for photons escaping from the column in this direction gives rise to both the decrease in flux and increase in the fitted optical depth measured at this phase. Analysis of the arrival time of individual dips in GX~1+4 provides the first measurement of azimuthal wandering of a neutron star accretion column. The column longitude varies stochastically with standard deviation 2-6 degrees depending on the source luminosity. Measurements of the phase width of the dip both from mean pulse profiles and individual eclipses demonstrates that the dip width is proportional to the flux. The variation is consistent with that expected if the azimuthal extent of the accretion column depends only upon the Keplerian velocity at the inner disc radius, which varies as a consequence of the accretion rate Mdot.Comment: 7 pages, 5 figures, accepted by MNRAS. If the characters of these stories and plays share a commonality, it is that they all search for something to believe in, some theory or myth about their lives. Though these characters want little more than to count on the familiar, like the rising of the sun or mothers who love them, they find little to grasp hold of outside themselves
