    Machine learning and multidrug-resistant gram-negative bacteria: An interesting combination for current and future research

    The dissemination of multidrug-resistant Gram-negative bacteria (MDR-GNB) is associated with increased morbidity and mortality in several countries. Machine learning (ML) is a branch of artificial intelligence that consists of conferring on computers the ability to learn from data. In this narrative review, we discuss three existing examples of the application of ML algorithms for assessing three different types of risk: (i) the risk of developing a MDR-GNB infection, (ii) the risk of MDR-GNB etiology in patients with an already clinically evident infection, and (iii) the risk of anticipating the emergence of MDR in GNB through the misuse of antibiotics. In the next few years, we expect to witness an increasingly large number of research studies perfecting the application of ML techniques in the field of MDR-GNB infections. Very importantly, this cannot be separated from the availability of a continuously refined and updated ethical framework allowing an appropriate use of the large datasets of medical data needed to build efficient ML-based support systems that could be shared through appropriate standard infrastructures

    Surgical management of rhinosinusitis in onco-hematological patients

    ObjectivesIn onco-hematological diseases, the incidence of paranasal sinuses infection dramatically increase and requires a combination of medical and surgical therapy. Balloon dilatation surgery (DS) is a minimally invasive, tissue preserving procedure. The study evaluates the results of DS for rhinosinusitis in immunocompromised patients.MethodsA retrospective chart review was conducted in 110 hematologic patients with rhinosinusitis. Twenty-five patients were treated with DS technique and 85 patients with endoscopic sinus surgery (ESS). We considered the type of anesthesia and the extent of intra- and postoperative bleeding. Patients underwent Sino-Nasal Outcome Test (SNOT-20) to evaluate changes in subjective symptoms and global patient assessment (GPA) questionnaire to value patient satisfaction.ResultsLocal anesthesia was employed in 8 cases of DS and in 15 of ESS. In 50 ESS patients, an anterior nasal packing was placed and in 12 cases a repacking was necessary. In the DS group, nasal packing was required in 8 cases and in 2 cases a repacking was placed (P=0.019 and P=0.422, respectively). The SNOT-20 change score showed significant improvement of health status in both groups. However the DS group showed a major improvement in 3 voices: need to blow nose, runny nose, and facial pain/pressure. The 3-month follow-up GPA questionnaire showed an higher satisfaction of DS group.ConclusionBalloon DS represents a potentially low aggressive treatment and appears to be relatively safe and effective in onco-hematologic patients. All these remarks may lead the surgeon to consider a larger number of candidates for surgical procedure

    Dating long thrust systems on Mercury: new clues on the thermal evolution of the planet

    The global tectonics of Mercury is dominated by contractional features mainly represented by lobate scarps, high relief ridges, and wrinkle ridges. These structures are the expression of thrust faults and are linear or arcuate features widely distributed on Mercury. Locally, these structures are arranged in long systems characterized by a preferential orientation and non-random spatial distribution. In this work we identified five thrust systems, generally longer than 1000 km. They were named after the main structure or crater encompassed by the system as: Thakur, Victoria, Villa Lobos, Al-Hamadhani, and Enterprise. In order to gain clues about their formation, we dated them using the buffered crater counting technique, which can be applied to derive the ages of linear landforms such as faults, ridges and channels. To estimate the absolute age for the end of the thrust system's activity, we applied both Le Feuvre and Wieczorek Production Function and Neukum Production Functions. Moreover, to further confirm the results obtained with the buffered crater counting method, the classic stratigraphic approach has been adopted, in which a faulted and an unfaulted craters were dated for each system. The results gave consistent ages and suggested that the most movements along major structures all over Mercury most likely ended at about 3.6–3.8 Ga. This gives new clues to better understand the tectonics of the planet and, therefore, its thermal evolution. Indeed, the early occurrence of tectonic activity in the planet's history, well before than predicted by the thermophysical models, coupled with the orientation and spatial distribution of the thrust systems, suggests that other processes beside global contraction, like mantle downwelling or tidal despinning, could have contributed to the first stage of the planet's history.     Criteria for the Evaluation of Italian Scientific Institutes for Research, Hospitalization and Healthcare (IRCCS): Comparison with European Standards and Certification Models

    The definition of the title 'Istituto di Ricovero e Cura a Carattere Scientifico' (IRCCS) and how this title is given by the Italian Ministry of Health is presented. Specifically, the first assessment of a commission concerning the essential information for the accreditation process is introduced. Moreover, the two years review process that aims to collect last updated information of the IRCCS, to identify level of excellence and critical aspects, is also explained. The present Italian forms and international manuals like Joint Commission, OECI and HCERES were schematized using UML diagrams. The current IRCCS accreditation forms are presented with the suggested updates organized in some levels of structuring. We compared the Italian forms with the manuals required to obtain international certifications (Joint Commission and OECI) and we analyzed the criteria for the evaluation of research units in France (the HCERES standards). Although it is a preliminary study, the use of UML diagrams allows to schematize a new accreditation model, in line with European guidelines and the most important international certifications

    On the number of limit cycles of the Lienard equation

    In this paper, we study a Lienard system of the form dot{x}=y-F(x), dot{y}=-x, where F(x) is an odd polynomial. We introduce a method that gives a sequence of algebraic approximations to the equation of each limit cycle of the system. This sequence seems to converge to the exact equation of each limit cycle. We obtain also a sequence of polynomials R_n(x) whose roots of odd multiplicity are related to the number and location of the limit cycles of the system.Comment: 10 pages, 5 figures.     PRELIMINARY TESTS OF A NEW LOW-COST PHOTOGRAMMETRIC SYSTEM

    This paper presents preliminary tests of a new low-cost photogrammetric system for 4D modelling of large scale areas for civil engineering applications. The system consists of five stand-alone units. Each of the units is composed of a Raspberry Pi 2 Model B (RPi2B) single board computer connected to a PiCamera Module V2 (8 MP) and is powered by a 10 W solar panel. The acquisition of the images is performed automatically using Python scripts and the OpenCV library. Images are recorded at different times during the day and automatically uploaded onto a FTP server from where they can be accessed for processing. Preliminary tests and outcomes of the system are discussed in detail. The focus is on the performance assessment of the low-cost sensor and the quality evaluation of the digital surface models generated by the low-cost photogrammetric systems in the field under real test conditions. Two different test cases were set up in order to calibrate the low-cost photogrammetric system and to assess its performance. First comparisons with a TLS model show a good agreement

    Pneumocystis jirovecii pneumonia (PJP) is an opportunistic fungal infection that may affect patients with immunosuppression. In order to improve the diagnosis accuracy for PJP, facilitating the collection of data across Europe to reliably assess the performance of diagnostic tests for PJP is essential to improve the care of critically ill patients developing this severe condition. Such large data can be collected thanks to the contribution of several European hospitals in the compilation of a dedicated electronic Case Report Form (eCRF). The main focus of this work is to create an interface with high ergonomics both in the compilation and in the subsequent validation of the records

    Temporal-spatial frequency rockfall data from open-pit highwalls using a low-cost monitoring system

    In surface mining, rockfall can seriously threaten the safety of personnel located at the base of highwalls and cause serious damage to equipment and machinery. Close-range photogrammetry for the continuous monitoring of rock surfaces represents a valid tool to efficiently assess the potential rockfall hazard and estimate the risk in the affected areas. This work presents an autonomous terrestrial stereo-pair photogrammetric monitoring system developed to observe volumes falling from sub-vertical rock faces located in surface mining environments. The system has the versatility for rapid installation and quick relocation in areas often constrained by accessibility and safety issues and it has the robustness to tolerate the rough environmental conditions typical of mining operations. It allows the collection of synchronised images at different periods with high-sensitivity digital single-lens reflex cameras, producing accurate digital surface models (DSM) of the rock face. Comparisons between successive DSMs can detect detachments and surface movements during defined observation periods. Detailed analysis of the changes in the rock surface, volumes and frequency of the rocks dislodging from the sub-vertical rock surfaces can provide accurate information on event magnitude and return period at very reasonable cost and, therefore, can generate the necessary data for a detailed inventory of the rockfall spatial-temporal occurrence and magnitude. The system was first validated in a trial site, and then applied on a mine site located in NSW (Australia). Results were analysed in terms of multi-temporal data acquired over a period of seven weeks. The excellent detail of the data allowed trends in rockfall event to be correlated to lithology and rainfall events, demonstrating the capability of the system to generate useful data that would otherwise require extended periods of direct observation
