    Investigation of Mannose-binding lectin level and deficiency in patients with Dermatophytosis

    Background: Dermatophytosis is a cutaneous fungal infection with a worldwide occurrence. In dermatophyte infections, the release of keratinocyte cytokines, in the presence of dermatophyte antigens, causes an acute phase response; subsequently, the acute-phase proteins are produced by hepatocytes. Mannose-binding lectin (MBL), an acute-phase protein, also acts as a kind of pattern recognition receptor. MBL deficiency plays a role in susceptible viral, bacterial, fungal and parasitic infections. Objectives: Some research has been conducted on the role of acute-phase proteins in dermatophyte infections. This study has been designed to determine the serum MBL levels in patients with dermatophytosis. Materials and Methods: This cross-sectional study, included 96 healthy individuals and 105 patients with dermatophytosis, in access sampling procedure. Microscopic examinations were conducted and cultivated to detect dermatophytes, and in the cases that the identification of different dermatophyte species was necessary, complementary examinations were conducted. Additionally, the enzyme-linked immunosorbent assay (ELISA) was used to determine the serum MBL levels of healthy individuals and patients. Various tests (Chi-square, Fisher exact, Mann - Whitney, Kruskal Wallis, Kendal tau correlation coefficient and ROC curve analysis) were used to examine the relationships between variables, when the P < 0.05 were considered as significant level. Results: The mean serum MBL level of healthy individuals and patients, was 1.53 ± 1.87 μg/mL and 1.97 ± 2.03 μg/mL (P = 0.039), respectively. Using ROC curve analysis, the MBL level was established as a significant predictor of dermatophytosis (P = 0.042). MBL deficiency (serum level < 1 μg/mL) was more common in healthy group (56.2) than the patients with dermatophytosis (41.0). Conclusions: The findings showed that the increased concentrations of serum MBL in patients with dermatophytosis play a role in this fungal infection. The high frequency of MBL deficiency in healthy individuals was compared with patients indicated that MBL deficiency is not a predisposing factor of this type of infection. © 2013, Ahvaz Jundishapur University of Medical Sciences; Published by Kowsar Corp

    Possible Brucellosis in an Early Hominin Skeleton from Sterkfontein, South Africa

    We report on the paleopathological analysis of the partial skeleton of the late Pliocene hominin species Australopithecus africanus Stw 431 from Sterkfontein, South Africa. A previous study noted the presence of lesions on vertebral bodies diagnosed as spondylosis deformans due to trauma. Instead, we suggest that these lesions are pathological changes due to the initial phases of an infectious disease, brucellosis. The macroscopic, microscopic and radiological appearance of the lytic lesions of the lumbar vertebrae is consistent with brucellosis. The hypothesis of brucellosis (most often associated with the consumption of animal proteins) in a 2.4 to 2.8 million year old hominid has a host of important implications for human evolution. The consumption of meat has been regarded an important factor in supporting, directing or altering human evolution. Perhaps the earliest (up to 2.5 million years ago) paleontological evidence for meat eating consists of cut marks on animal remains and stone tools that could have made these marks.     A comprehensive overview of radioguided surgery using gamma detection probe technology

    The concept of radioguided surgery, which was first developed some 60 years ago, involves the use of a radiation detection probe system for the intraoperative detection of radionuclides. The use of gamma detection probe technology in radioguided surgery has tremendously expanded and has evolved into what is now considered an established discipline within the practice of surgery, revolutionizing the surgical management of many malignancies, including breast cancer, melanoma, and colorectal cancer, as well as the surgical management of parathyroid disease. The impact of radioguided surgery on the surgical management of cancer patients includes providing vital and real-time information to the surgeon regarding the location and extent of disease, as well as regarding the assessment of surgical resection margins. Additionally, it has allowed the surgeon to minimize the surgical invasiveness of many diagnostic and therapeutic procedures, while still maintaining maximum benefit to the cancer patient.     Determination of Serum Levels of Interleukin-6, Interleukin-8, Interleukin-10, and Tumor Necrosis-Alpha and their Relationship With The Total Body Surface Area in Children

    There are few studies on the inflammatory processes and the role of cytokines involved in pediatric burn injuries. The present study aims to measure the serum levels of cytokines and their relationship with the degree of burn injury in children. Within the 48 hours of hospitalization, the serum samples were obtained to measure inflammatory cytokines (interleukin-6, interleukin-8, interleukin-10 IL-6, IL-8, and IL-10 and tumor necrosis factor-alpha TNF-α). The level of all of these cytokine factors was assessed by enzyme-linked immunosorbent assay technique. The mean levels of IL-6, IL-8, IL-10, and TNF-α was 18.15 ± 4.77 pg/ml, 59.54 ± 4.59 pg/ml, 8.41 ± 2.09 pg/ml, and 1.48 ± 0.15 pg/ml, respectively, which were higher than the normal range designated for the healthy pediatrics age group. The levels of TNF-α were higher in patients with sepsis (P = .03) and deceased patients (P = .001). There was a statistically significant difference in the levels of IL-8 in patients with second- (.001) and third-degree (.001) burn injuries in comparison to the first-degree burn injuries, and the level of IL-8 was statistically significantly higher in patients with electrical burn injuries in comparison to scald burn injuries (.01). IL-10 was statistically significantly higher in patients with contact burn injuries in comparison to scald (.001) and flame (.03) burn injuries. Cytokine levels in pediatric burn patients increased after severe burn injuries. There was a significant correlation between the levels of IL-8 and the degree of burn injuries. © American Burn Association 2019. All rights reserved.     Analog Pulse Shape Discrimination Based on Time Duration and Pulse Height

    Pulse shape discrimination is a name of a group of techniques used to detect and distinguish between different types of radiation interactions. Analog pulse shape discrimination methods can be more suitable than digital methods, for high-speed scintillators both from rate and power consumption perspectives. Common analog discrimination methods are based on pulse-height and pulse-energy discrimination techniques. Other techniques rely on the time difference in the pulse width such as the ZeroCrossing methods. Neither of the above combine both amplitude and time methods. We present a novel analog pulse shape discrimination topology that combines both height and time domain. The topology is based on discrimination according to the pulse duration in time combined with compensation function of the pulse height. Amplitude of the pulse is used as a restraining factor. Subsequently, our topology correctly identifies fast pulses that are prolonged in time due to their high amplitude. The topology yields superior discrimination capabilities, under degraded light collection conditions, with an uncertainty gap smaller than 1 ns in pulse width. The ability to control both the time and the amplitude parameters individually, provides tailored adjustment for various detectors and pulse shape discrimination applications

    The Incidence of Co-occurrence of Chlamydial Cervicitis with Bacterial Vaginosis

    Background and Objectives: Bacterial vaginosis is caused by an imbalance in normal vaginal bacterial flora mainly caused by the introduction of pathogenic bacteria. Failure to properly treat this condition can not only induce abortion but also increase the chance of acquiring other serious infections such as AIDS, gonorrhea and chlamydiosis. Chlamydia trchomatis is one of the causative agents of cervicitis of which 70% is totally asymptomatic. Untreated cases can lead to salpengititis, pelvic inflammatory diseases, infertility, pelvic area pains and other complications. The purpose of this study was to determine the co-occurrence of these two conditions.Methods: A total of 137 patients were examined for both Chlamydial cervicitis and for bacterial vaginosis. Gram stain was used to detect bacterial vaginosis and anti-chlamydial antibodies were titered by microimmunofluoresence (MIF) assay. Results: According to the MIF results, 10 patients(7.3%) had elevated anti-chlamydial IgG and 3 patients (2.2%) showed high IgM titers. Gardnerella vaginalis was detected in 6 patients(4.7%) as the causative agent of vaginosis. There were 3 cases of co-occurrence of chlamydial cervicitis and bacterial vaginosis (30%). Conclusion: Due to the fact that bacterial vaginosis can provide the pre-disposing conditions for cervicitis and its chronicity and the similarity of the cilinical singns of these two conditions, Infections with Chlamydia are often overlooked. It therefore seems necessary to check any patient with bacterial vaginosis for chlamydial co-infection

    Spectral Enhancement of a SiPM Array-Based Radiation Detector

    Silicon Photomultipliers (SiPMs) have many advantages when used in radiation detectors. Low bias voltage, compactness and immunity to electromagnetic interference are among their prominent benefits. However, due to their small size, usually an array of SiPM components is required in order to cover the coupling surface area of a scintillator. Since the SiPM is a semiconductor, biased in a reversed voltage, gain variation and strong temperature dependence are introduced. As a result, SiPM-based detectors, particularly an array of SiPMs, undergo spectral signal to noise ratio reduction. This work studies the effect of the SiPM breakdown voltage variation on the obtained energy spectrum and proposes an electronic approach to overcome this technological drawback. This developed technology provides an adequate temperature-dependent, commonly distributed high bias voltage and an individual offset-voltage fine tuning that enables adjustment of all the SiPM components to their optimum operating points. Powerwise it is beneficial to operate SiPM at lower voltages, where undesirable gain variation is more dominant. The proposed solution enables working at lower bias voltages, which provides lower power consumption and better radiation hardness, while yielding an enhanced spectrum resolution. The proposed electronic approach enhances the obtained spectra, reducing the noise threshold by 16 % when working at 1 V overvoltage. Hence provides an enhanced signal to noise ratio over the traditional biasing methods

    Recurrent vulvovaginal candidiasis (RVVC) is a common opportunistic, mucosal fungal infection, predominantly caused by the fungus Candida albicans. Mannose-binding lectin (MBL) is an acute-phase protein that plays a key role in the innate immunity defence against infectious disease. This study was conducted to evaluate the relationship between the MBL serum level and the relative expression of MBL mRNA in RVVC using real-time PCR for the first time. The case-control study included 40 female participants suffering from RVVC and 40 healthy individuals. The MBL serum level was measured using a commercial ELISA kit. The relative mRNA expression of the MBL gene was quantified using real-time PCR. Data analysis was carried out by spss software. The MBL concentration was significantly higher in the participants suffering from RVVC compared to the control group (0.330 ng/mL vs 0.253 ng/mL). The prognostic value (P <.001) for RVVC diagnosis has been calculated. Quantitative RT-PCR results from 35 samples showed a low to significant values for mRNA levels corresponding to MBL gene expression (1-352 folds) (P <.001). The results of this study suggest that MBL plays a main role in the innate immunity and it is also affected by environmental factors and other genetic variations. Therefore, the MBL gene expression profile does not reflect precise phenotypic levels in the serum. © 2018 Blackwell Verlag Gmb
