Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training. Using proportional assist ventilation to wean a prolonged mechanically ventilated patient with chronic obstructive pulmonary disease

Long-term mechanical ventilation (MV) increases heath care cost. Difficult-to-wean patients represent a challenge to health care providers and patient′s family. Patient with chronic obstructive pulmonary diseases (COPDs) are usually difficult to wean off mechanical ventilators for many pathophysiological reasons. Modes of ventilation used in MV weaning have produced variable results. The main shortcoming of currently available modes of weaning is inability to meet patient′s changing ventilator demand. Proportional assist ventilation (PAV) is a new mode of ventilation designed to keep up with patient′s breathing demand and unload work of breathing. In this case report, we have shown that PAV was able to wean off a patient with COPD who had been on the ventilator for 37 days

Aim: This article aims to explore best practices and innovative approaches in infection control within healthcare environments, focusing on epidemiological methods and isolation protocols. Methods: The article reviews fundamental epidemiological principles, including disease frequency, distribution, and determinants. It discusses various study designs such as case reports, cross-sectional studies, and randomized controlled trials, highlighting their application in infection control. The article also examines isolation practices recommended by the Centers for Disease Control and Prevention (CDC) and provides an overview of standard and transmission-based precautions. Results: The review underscores the importance of accurate disease quantification through prevalence and incidence metrics. It details the strengths and limitations of different epidemiological study designs, emphasizing their role in understanding and controlling infections. Additionally, the article outlines the CDC's guidelines for isolation, noting the implementation challenges and the need for updated practices to address evolving infectious threats

The emergency department services on the monitoring of direct oral anticoagulants

Background: Direct oral anticoagulants (DOACs) such as dabigatran, rivaroxaban, apixaban, and edoxaban are increasingly used due to their benefits over warfarin, including reduced risk of intracranial hemorrhage and fewer dietary and drug interactions. Despite these advantages, the management of DOACs involves complexities like dose adjustments for renal or hepatic impairment, potential drug-drug interactions, and high costs, leading to prevalent off-label use. Aim: This study aims to evaluate the effect of Anticoagulation management services (AMS) on the management of DOAC therapy compared to non-AMS management, focusing on healthcare utilization and anticoagulation-related outcomes in the emergency department. Methods: A retrospective cohort analysis was conducted at the University of Utah Health, analyzing data from adult patients prescribed DOACs. Patients were categorized into AMS and non-AMS groups based on management approach. The study assessed the frequency and types of encounters, interventions related to DOAC therapy, and clinical outcomes such as thromboembolic events, bleeding, and mortality. Results: Patients managed by AMS had more frequent interactions regarding DOAC therapy but did not show significant improvement in clinical outcomes compared to non-AMS managed patients. The AMS group had slightly higher bleeding events, which might be attributed to higher comorbidity scores and poorer renal function.&nbsp

Emerging infectious diseases and their impact on emergency medical services: Role of emergency and paramedic

Background: Emerging infectious diseases (EIDs) pose significant threats to global health due to their potential for widespread outbreaks and severe impacts on public health systems. These diseases can emerge from zoonotic sources, environmental changes, or resistance to antibiotics. Approximately 15% of human pathogens are considered to be emerging, reflecting their critical role in public health. Aim: This review aims to evaluate the impact of emerging infectious diseases on emergency medical services, with a focus on the role of zoonotic diseases, vector-borne infections, and the application of advanced surveillance technologies in managing these threats. Methods: A comparative review of literature and data sources, including global surveillance systems and recent outbreak case studies, was conducted to assess the impact of EIDs on emergency medical services. The study also examines the effectiveness of the One Health approach and the role of advanced technologies in disease monitoring. Results: Emerging infectious diseases, including zoonotic and vector-borne infections, have significantly impacted emergency medical services by increasing the demand for urgent care and resource allocation. Replacing protein via enteral nutrition in a stepwise approach in critically ill patients: the REPLENISH randomized clinical trial protocol

Abstract Background Protein intake is recommended in critically ill patients to mitigate the negative effects of critical illness-induced catabolism and muscle wasting. However, the optimal dose of enteral protein remains unknown. We hypothesize that supplemental enteral protein (1.2 g/kg/day) added to standard enteral nutrition formula to achieve high amount of enteral protein (range 2–2.4 g/kg/day) given from ICU day 5 until ICU discharge or ICU day 90 as compared to no supplemental enteral protein to achieve moderate amount enteral protein (0.8–1.2 g/kg/day) would reduce all-cause 90-day mortality in adult critically ill mechanically ventilated patients. Methods The REPLENISH (Replacing Protein Via Enteral Nutrition in a Stepwise Approach in Critically Ill Patients) trial is an open-label, multicenter randomized clinical trial. Patients will be randomized to the supplemental protein group or the control group. Patients in both groups will receive the primary enteral formula as per the treating team, which includes a maximum protein 1.2 g/kg/day. The supplemental protein group will receive, in addition, supplemental protein at 1.2 g/kg/day starting the fifth ICU day. The control group will receive the primary formula without supplemental protein. The primary outcome is 90-day all-cause mortality. Other outcomes include functional and quality of life assessments at 90 days. The trial will enroll 2502 patients. Discussion The study has been initiated in September 2021. Interim analysis is planned at one third and two thirds of the target sample size. The study is expected to be completed by the end of 2025.
