    Asthma, Food Allergy, and How They Relate to Each Other

    The association between atopic diseases is well known, and previous research has shown that having one atopic disease can predispose to having another. The link between asthma and food allergy has been well researched, but the exact relationship between the two atopic conditions is not fully understood. Food allergic infants are at increased risk for the development of asthma and are at risk of food-induced asthmatic episodes and also anaphylaxis. Having a diagnosis of both food allergy and asthma has also been shown to have an effect on the severity of a patient’s disease including being at greater risk of severe asthmatic episodes. Therefore, understanding the relationship between these two conditions in order to treat and manage these children safely is crucial to clinicians

    The architectural development of Acacia karroo conforms to Troll’s model. Growth of the branches is modular and sympodial with heteroblastic leaves on all long shoots of the tree, including the seedling. Axillary buds tend to proliferate especially on flowering shoots where they form fascicles consisting of up to 10 inflorescences arranged in two parallel serial rows per leaf axil. Most axillary buds are sylleptic and basal buds which give rise to short shoots, each producing two to five cataphylls each season, but no flowers. Inflorescences are only produced on long shoots (modules) of the current season. After flowering the terminal part of the module aborts, trees are usually andromonoecious with capitate inflorescences containing 40 to 100 flowers each, with some male and some hermaphrodite. Some trees produce only male flowers. Anthesis in the same inflorescence, the same tree as well as amongst trees of the same community are synchronised and occur at intermittent intervals, each lasting three or more days at a time. Flowers are protogynous and pollen is produced in polyads, each consisting of 16 pollen grains. Ovaries contain 10 to 14 ovules each. The concave stigma has space for only one polyad which can fertilise all ovules in the ovary after a single pollination event.     IgE-mediated fish allergy in children

    Abstract: Fish allergy constitutes a severe problem worldwide. Its prevalence has been calculated as high as 7% in paediatric populations, and in many cases, it persists into adulthood with life-threatening signs and symptoms. The following review focuses on the epidemiology of Immunoglobulin E (IgE)-mediated fish allergy, its pathogenesis, clinical manifestations, and a thorough approach to diagnosis and management in the paediatric population. The traditional approach for managing fish allergy is avoidance and rescue medication for accidental exposures. Food avoidance poses many obstacles and is not easily maintained. In the specific case of fish, food is also not the only source of allergens; aerosolisation of fish proteins when cooking is a common source of highly allergenic parvalbumin, and elimination diets cannot prevent these contacts. Novel management approaches based on immunomodulation are a promising strategy for the future of these patients

    Biomarkers of severity and threshold of allergic reactions during oral peanut challenges

    Background: oral food challenge (OFC) is the criterion standard to assess peanut allergy (PA), but it involves a risk of allergic reactions of unpredictable severity.Objective: our aim was to identify biomarkers for risk of severe reactions or low dose threshold during OFC to peanut.Methods: we assessed Learning Early about Peanut Allergy study, Persistance of Oral Tolerance to Peanut study, and Peanut Allergy Sensitization study participants by administering the basophil activation test (BAT) and the skin prick test (SPT) and measuring the levels of peanut-specific IgE, Arachis hypogaea 2–specific IgE, and peanut-specific IgG4, and we analyzed the utility of the different biomarkers in relation to PA status, severity, and threshold dose of allergic reactions to peanut during OFC.Results: when a previously defined optimal cutoff was used, the BAT diagnosed PA with 98% specificity and 75% sensitivity. The BAT identified severe reactions with 97% specificity and 100% sensitivity. The SPT, level of Arachis hypogaea 2–specific IgE, level of peanut-specific IgE, and IgG4/IgE ratio also had 100% sensitivity but slightly lower specificity (92%, 93%, 90%, and 88%, respectively) to predict severity. Participants with lower thresholds of reactivity had higher basophil activation to peanut in vitro. The SPT and the BAT were the best individual predictors of threshold. Multivariate models were superior to individual biomarkers and were used to generate nomograms to calculate the probability of serious adverse events during OFC for individual patients.Conclusions: the BAT diagnosed PA with high specificity and identified severe reactors and low threshold with high specificity and high sensitivity. The BAT was the best biomarker for severity, surpassed only by the SPT in predicting threshold. Nomograms can help estimate the likelihood of severe reactions and reactions to a low dose of allergen in individual patients with PA
